The Board APPROVED the above Resolution as stated above ; however , Supervisor Gerber , District III , stated for the record that she had a potential conflict of interest as to this item arising from her spouse ' s employment . She did not participate or vote on this item. This program will be implemented April 1, 2001, subject to approval by the Board of Supervisors. The Department wishes to expand its current recruitment and retention efforts by offering a financial incentive to reimburse Social Casework Specialists if they have outstanding student loans used to obtain a Masters' Degree in Social Work or its Masters'- level equivalent that qualifies them for the classifications of Social Casework Specialist I or Il. The Employment and Human Services Department pilot program will reimburse up to 65% of a Social Casework Specialist's (I or 11) MSW outstanding loan amount, with a total maximum reimbursement of amount of$34,000 per employee. This is a pilot program and will be available for new and current Social Casework Specialists. Upon adoption by the Board of Supervisors, this program begins for all current employees with outstanding eligible loans. To qualify for reimbursement, employees must complete one full year of employment, defined as 2,080 hours of service. Persons hired as full time Social Casework Specialist I or 11, who later change hours to part time will also be subject to the same one year full time employment hourly equivalent prior to being able to participate in this program. Maximum Amount of employee reimbursement under this program: $34,000. Reimbursement will be based upon the following: (Example) Outstanding Loan Balance Amount % Reimbursement -Completion of 1'year employment or hourly equivalent $25,000 15% $ 3,750 -Completion of 2" year employment or hourly equivalent $21,250 20% $4,250 -Completion of 3rd year employment or hourly equivalent $17,000 30% $5,100 TOTAL 65% $13,100 Employees who resign or are terminated, or accept a position in the EHSD or the County other than as a Social Casework Specialist VII or Social Work Supervisor 11, before completion of five years of full time equivalent employment at 2,080 hours/year, will no longer be eligible for the School Loan Forgiveness Program. Any further payments towards their loan will be discontinued, and they will be obligated to repay the Department as follows: After com letion of the 1' year but before completion of 2"d year 80% of 15% disbursal After completion of the 2 year but before completion of 3` ear 60% of 35% disbursal After completion of the 3rd year but before completion of :Py'year 40% of 65% disbursal After completion of the 4hyear but before completion of 5 year 20% of 65% disbursal After com letion of the 5 year 0% All applicable taxes shall be paid by the employee during their participation in this program. PROGRAM PROCEDURES The Social Casework Specialist VII shall 1. Complete the "Request for Reimbursement School Loan Forgiveness Program" after completing one year of full time equivalent employment (as described above) 2. Attach a copy of his/her outstanding student loan statements yearly. This information will be verified. Notwithstanding the foregoing the Department/County reserves sole discretion to determine whether an employee has submitted appropriate documentation. 3. Be responsible for ensuring the request and all relevant documents are received by the EHSD Personnel Office prior to payment being authorized. I am requesting reimbursement for 20% of my outstanding loan balance up to $28,900 because I have completed two years of full-time equivalent employment (4160 hours), with the Employment and Human Services Department in one of the above listed classifications. I am requesting reimbursement for 30%of my outstanding loan balance up to $23,120 because I have completed three years of full time equivalent employment (6240 hours), with the Employment and Human Services Department in one of the above listed classifications. I understand that should I leave employment as a Social Casework Specialist I/II, prior to the completion of 5 years of service, (10,400 hours) I will be responsible for reimbursing the Contra Costa County Employment and Human Services Department for monies received under this program. In requesting reimbursement, I understand the County has the right to initiate action to recover the full reimbursement in accordance with the chart amount shown above in the event I resign, I am terminated, or I accept a position in the EHSD or the County other than Social Casework Specialist 1111 before completion of five years of full time equivalent employment at 2,080 hours/year. I understand this reimbursement may be discontinued at any time due to the pilot nature of this program, at the sole discretion of Contra Costa County. Signature Date Verification by the Employment and Human Services Department Full time Equivalent Employment verified by Signature Date LOAN AMOUNT VERIFIED BY Signature Date SIDE LETTER AGREEMENT Social Casework Specialist I and II Recruitment Referral Program The purpose of this program is to enhance recruitment efforts of new hires by providing a financial incentive when successful hiring results from county employee referrals of qualified candidates for Social Casework Specialist I and II positions. Eligible county employees will receive $500 when a referred applicant is hired, AND will receive an additional $500 when the worker successfully completes nine months of EHSD service. Eligible county employees are those employees who are permitted to participate in this program and who are not listed among the ineligible employees (below). An eligible employee must refer an applicant who is hired into a SCS position and who successfully completes nine months of EHSD service. All applicable taxes shall be plaid by the employee who receives any monetary referral incentives. Ineligible County employees are excluded from participation in this program because they are (l) employees whose job duties or job description includes recruitment efforts; (2) employees whose job function includes review of employment applications as a point of a recruitment efforts; (3) employees in the Human Resources Department or in the EHSD Personnel Unit; (4) all County executive managers; (5) employees who are assigned the duties in the classification of Social Service Division Manager or its equivalent in the Employment and Human Services Department; (6) employees who refer a former employee who is eligible for reinstatement. PROCEDURES: The referring employee shall be responsible for following the established referral procedures. Failure to follow established procedures shall render the employee ineligible to receive incentive monies. 1. The referring employee shall take steps to reasonably establish that the applicant being referred meets the minimum qualifications(MQ's) of the SCS position. 2. The referring employee shall take steps necessary to ensure that the applicant being referred is not a former employee who is eligible for reinstatement. 3. The referring employee shall complete the applicable portion of the EHSDE Employee Referral Incentive Program Payment form. (EHSDERIPPF) 4. The referring employee shall ensure that the applicant is aware of the referral by obtaining the candidate's signature on the EHSDERIPPF and by ensuring that the applicant attaches the signed form to the candidates' application prior to the application being submitted for employment consideration. 5. The referring employee shall ensure that the application and completed, signed EHSDERIPPF are forwarded to the EHSD Department Personnel Unit by US mail (40 Douglas Drive, Martinez, CA 94553), or by internal route slip. 6. This is a pilot program. The County of Contra Costa has the exclusive right to discontinue this program at any time. 7. In the event the County discontinues the program, eligible employees shall receive the additional $500.00 payment when the worker successfully completes nine months of EHSD service.
