SUBJECT: Approval of Contract #26-394 with Healthcare Financial Staffing
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Frank Puglisi, Jr. ) , to execute on behalf of the County, Contract
#26--394 with Healthcare Financial Staffing, for the period from July
1, 2000 through June 30, 2001, to be paid in accordance with the
attached fee schedule, for temporary certified and experienced
billing professionals to assist the Department with implementation
of Medicare ' s new Ambulatory Payment Classification system.
FISCAL IMPACT:
Funding for this Contract is included in the Department ' s Enterprise
I budget .
BACKGROUND/REASON(g) FQR RECOMMENDATION(S) :
Starting July 1, 2000, Medicare will reimburse the County under the
new Ambulatory Payment Classification (APC) system for all Medicare
clinic and emergency visits and outpatient surgeries. The APC
reimbursement system requires correct, verifiable capture and
Current Procedural Terminology (CPT) coding of all procedures and
International Classification of Diseases (ICD-9) coding of
diagnoses . CPT coding is very complex and, depending upon the type
of billing form required (HCFA 1500 or UB92) , different CPT codes
may need to be applied. In addition, all charges must logically
flow to one of the coded CPT procedures.
Under Contract #26-394, Healthcare Financial Staffing will provide
qualified professionals to assist the Department with implementation
of the APC system for the County' s Medicare population.

Approve and authorize the Health Services Director or his designee (Donna
Wigand, LCSW) to execute on behalf of the County, Novation Contract
#24-932-3 with Rape Crisis Center, for the period from July 1, 2000 through
June 30, 2001, in the amount of $102, 000, for the provision of Early and
Periodic Screening, Diagnosis and Treatment (EPSDT) Program services for
children who have been sexually molested or traumatized. This Contract
includes a six-month automatic contract extension through December 31, 2000,
in the amount of $51, 000.
FISCAL IMPACT:
This Contract is funded as follows:
$ 52,581 Federal FFP Medi-Cal Funds
49,419 State EPSDT Funds
$102, 000 Total Payment Limit
CHILDREN'S IMPACT STATEMENT:
This EPSDT Program supports the following Board of Supervisors' community
outcomes: 1) Children ready for and succeeding in school; 2) Families that
are safe, stable, and nurturing; and 3) Communities that are safe and
provide a high quality of life for children and families. Expected program
outcomes include an increase in positive social and emotional development
as measured by the Child and Adolescent Functional Assessment Scale (CAFAS)
and a decrease in out-of-home placements as measured by mental health and
social services statistics.
MCKGROUND/REASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of County's population in that it
provides in-home and outpatient mental health services for Medi-Cal eligible
children (and their families) who have been sexually molested or traumatized
and are enrolled in the EPSDT Program.
Can July 22, 1999, the Board of Supervisors approved Contract #24-932-2 with
Rape Crisis Center for the period from July 1, 1999 through June 30, 2000
(which included a six-month automatic contract extension through December
31, 2000) to provide EPSDT program services. Novation Contract #24-932-3
replaces the six -month automatic extension of the prior Contract, and allows
the Contractor to continue providing services through June 30, 2001.

Approve and authorize the Health Services Director or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Contract
#22-749-2 with Rubicon Programs, Inc . (dba Rubicon Home Care
Consortium) , in the amount of $100, 000, for the period from July 1,
2000 through June 30, 2001, to provide temporary in-home health care
for AIDS patients .
FISCAL IMPACT:
This Contract is 100% State/Federal funded.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
In September 1999, the County Administrator approved, and the
Purchasing Services Manager executed Contract #22-749 (as amended
by Contract Amendment Agreement #22-749-1) , with Rubicon Programs,
Inc . (dba Rubicon Home Care Consortium) , for the period from August
1, 1999 through June 30, 2000, to provide temporary in-home health
care for AIDS patients .
Rubicon Programs is a nationally recognized social service
organization that services several at-risk populations in unique and
innovative ways . For over 26 years, Rubicon has built housing,
provided employment services, mental health services and created
jobs to assist individuals who have disabilities, are homeless, or
who are otherwise economically disadvantaged.
Approval of Contract #22-749-2 will allow the Contractor to continue
to provide registry services through June 30, 2001 .

RRECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee (Wendel
Brunner, M.D. ) to execute on behalf of the County, Contract #22-456-9 with
Center for Human Development, in the amount of $80, 000, for the period from
July 1, 20070 through June 30, 2001, to provide HIV/AIDS education and
prevention services to youth at increased risk for HIV infection.
FISCAL IMPACT•
This Contract is funded 100 by the State Department of Health Services, Office
of AIDS . No County match is required.
CHILDREN' S IMPACT STATEMENT:
This AIDS Program supports County' s "Children and Youth Healthy and Preparing
for Productive Adulthood" community outcome by providing educational
presentations at schools and youth-serving organizations to help youth delay
sexual involvement and avoid infection with HIV and other Sexually Transmitted
Diseases (STD) . Expected program outcomes are increased knowledge of HIV
transmission and prevention; increased abstinence and/or safer sex; reductions
in the numbers of unplanned pregnancies and STD/HIV infections; and reductions
in health care costs associated with treating STDs including HIV and AIDS.

BACKGROWD/REASON(S) FOR RECOMMENDATION(S):
The Center for Disease Control and the State Office of AIDS have mandated the
County Health Department to create a HIV Prevention Planning Group (HPPG) to
guide local prevention efforts . County' s AIDS program incorporates HPPG
recommendations to provide HIV prevention services through local agencies .
This Contract meets the social needs of the population of the County in that
it provides education and prevention services to reduce the incidence of HIV
and other STDs among County' s at-risk population.
On August 17, 1999, the Board of Supervisors approved Contract #22-456-8 with
Center for Human Development, for the period from July 1, 1999 through June 30,
2000, to provide HIV/AIDS education and prevention services for young men who
are at increased risk for HIV.
Under Contract #22-456-9, Contractor will provide HIV/AIDS education and
prevention services to youth who are at increased risk for HIV through June 30,
2001 .

Approve and authorize the Health Services Director, or his designee (Donna
Wigand) , to execute on behalf of the County, Contract 474-037-1 with Contra
Costa Association for Retarded Citizens, in the amount of $71, 658, for the
period from July 1, 2000 through June 30, 2001, to provide mental health
services for recipients of the Cal-WORKs program, and their children.
FISCAL IMPACT•
This Contract is funded by State funds through the Employment and Human
Services Department .
CHILDREN'S IMPACT STATEMENT:
This Mental Health program supports the following Board of Supervisors'
community outcomes : "Children Ready For and Succeeding in School" and
"Families that are Safe, Stable, and Nurturing" . Expected program outcomes
include an increase in positive social and emotional development, as
measured by the Child and Adolescent Functional Assessment Scale (CAFAS) and
a decreased use of acute mental health care .
BACXQROUND/REASON(S 3 FOR RECOMMENDATION(SI :
In August, 1997, the State of California Legislature passed AB 1542 which
brought major changes to the welfare programs previously operated by the
State. Among the changes was a provision which required treatment of mental
illnesses of Welfare-to-Work participants when the condition interferes with
participation in Welfare-to-Work activities . Subsequently, the County' s
Employment and Human Services Department and Health Services Department
signed an Interdepartmental Services Agreement (#21-427) which allowed the
Health Services Department to provide mental health services to referred
Welfare-to-Work participants . Responsibility for these mental health
services involves contracts with organizational providers to deliver these
services .
Can February 1, 2000, the Board of Supervisors approved Contract #74-037 with
Contra Costa Association for Retarded Citizens, for the period from January
1, 2000 through June 30, 2000 .
Approval of Contract #74-037-1 will allow the Contractor to continue to
provide mental health services to recipients of the Cal-WORKS Program and
their children, through June 30, 2001 .
