SPECIFIC REQUEST(S)OR RECOMMENDATION(S)3 BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) , to execute on behalf of the County, Contract
#24-959-3 with Family Stress Center, in the amount of $82 , 614, for
the period from July 1, 2000 through June 30, 2001, for the
provision of mental health services, including individual, group,
and family collateral counseling, case management, and medication
management for CalWORKs participants .
FISCAL IMPACT:
This Contract is 100% funded by the State CalWORKs through the
Employment and Human Services Department .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S)_ :
In August, 1997, the State of California Legislature passed Assembly
Bill 1542 which brought major changes to the welfare programs
previously operated by the State. Among the changes was a provision
which required treatment of substance abuse and mental illnesses of
Welfare-to-Work participants, when these conditions interfere with
participation in Welfare-to-Work activities . Subsequently, the
County' s Employment and Human Services Department and Health
Services Department signed an Interdepartmental Services Agreement
(#21-427) which allowed the Health Services Department to provide
substance abuse and mental health services to Welfare-to-Work
participants referred by the Employment and Human Services
Department .
On February 15, 2000, the Board of Supervisors approved Contract
#24-959-2 with Family Stress Center, for provision of mental health
services, including individual, group and family collateral
counseling, case management, and medication management services for
CalWORKs participants to reduce barriers to employment, through June
30, 2000 .
Approval of Contract #24-959-3 will allow the Contractor to continue
providing services, through June 30, 2001 .

RECOMMENDATIONO) :
Approve and authorize the Health Services Director, or his designee
(Milt Camhi) to execute on behalf of the County, Contract #27-442-1
with Doris James, for the period from July 1, 2000 through June 30,
2001, in the amount of $37,883 , for consultation and technical
assistance to Contra Costa Health Plan (Health Plan) with regard to
the Verifications Unit.
FISCAL IMPACT:
This Contract is funded by Health Plan member premiums.
BACKgROUNDfREASON($) FOR RECOMMENDATION(S) :
The Health Plan's Verifications Unit has a key role in the correct
appropriation of funds and services to Health Plan members and
protecting the Health Plan against fraudulent claims. Contractor's
services include developing Procedure and Training Manuals, training
County-selected staff in the policies and procedures, overseeing
daily activities, and maintaining the Resource Information
Management System (RIMS) .
On July 20, 1999, the Board of Supervisors approved Contract #27-442
with Doris James to provide consultation and technical assistance
to the Health Plan with regard to the Verifications Unit for the
period from July 1, 1999 through June 30, 2000.
Approval of Contract #27-442-1 will allow the Contractor to continue
providing services through June 30, 2001.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #27-386-2 with Diane
Chow, D.P.M. , for the period from June 1, 2000 through May 31, 2001, for the
provision of professional podiatry services for the Contra Costa Health
Plan, to be paid as follows :
a. For Medi-Cal and Commercials Members:
County shall pay Contractor those rates set forth in the Medi--Cal Schedule
of Maximum Allowances in effect on August 1, 1998. In the event rate
increases are subsequently approved by the State of California and are
included in the County's Health Plan capitated payment, County will
thereafter increase the rates County pays to Contractor accordingly.
b. For Medicare members:
Services for members who are Medicare recipients will be reimbursed at the
Medicare rate of payment. Physician will bill Medicare as primary payor and
County will pay Medicare-required copayments and deductibles for Medicare
approved services.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member
premiums . Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
BACKGROUNDJREASON(S) FOR RECOMMENDATION(S)-:
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On June 22 , 1999, the Board of Supervisors approved Contract #27-386-1 with
Diane Chow, D. P.M. , for the period from June 1, 1999 through May 31, 2000 .
Approval of Contract #27-386-2 will allow the Contractor to continue to
provide professional podiatry services to Contra Costa Health Plan members,
through May 31, 2001 .

Approve and authorize the Health Services Director, or his designee (Donna
Wigand) to execute on behalf of the County, Contract Amendment Agreement
#24-974-2, with Jaginder Singh, M.D. , effective September 1, 1999, to amend
Contract #24-974-1, to remedy the mutual mistake of the County and
Contractor and to accurately reflect the intent of the parties .
FISCAL IMPACT:
This Amendment will increase the hourly salary from $66 .50 to a new hourly
salary of $70.00, and increase the Total Contract Payment Limit by $5, 460
from $103 , 740 to a new payment limit of $109,200 .
This Contract is funded by Mental Health Realignment, and offset by third
party billing.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On September 28, 1999, the Board of Supervisors approved Contract #24-971-1
with Jaginder Singh, M.D. , for the period from September 1, 1999 through
August 31, 2000, for the provision of professional psychiatric services .
Due to the mutual mistake of the parties, the Contract did not accurately
reflect the oral agreement between parties, which was that the Contractor
would provide psychiatric services at a negotiated increase to his hourly
salary, effective September 1, 1999 .
Approval of this Contract Amendment Agreement #24-974-2 will reform the
Contract, to make the formal Contract consistent with the oral agreement,
which was negotiated with the Contractor and allow the Contractor to
continue providing services through August 31, 2000 .

Fred Williams (dba Williams .Board and Care Home)

RNCOMMEMATION(S) :
a. Approve and authorize the Health Services Director or his designee (Donna M. Wigand)
to execute on behalf of the County Emergency Residential Care Placement Agreement
#24-086-118 (3) with Fred Williams (dba Williams Board and Care Dome) , for the period
from July 1, 2000 through June 30, 2001, to provide emergency residential care to
mentally disturbed adults who are potentially eligible for SSI/SSP, in an amount not
to exceed $847 per client per month through December 31, 2000, and in an amount not
to exceed $890 per client per month beginning January 1, 2001; and
b. Authorize the Auditor--Controller to pay an amount not to exceed $174 per month
through December 31, 2000, and an amount not to exceed $183 per month beginning
January 1, 2001, to clients who are placed in the board and care facilities under
said Emergency Residential Care Placement Agreements for incidental expenses and
personal needs.
FISCAL IMPACT:
Under this program, the County pays the State-established rates for the residential care
of mental health clients who are not vet receiving SSI/SSP (but who are potentially
eligible and have made formal application to the Social Security Administration for
SSIJSSP) , contingent upon the client's signing an agreement with the County to reimburse
the County from the SSI/SSP money, once it is finally granted by the federal government
(usually 3 to 6 months retroactively back to the month of application) .
For those mental health clients who are not granted SSI/SSP (i.e. , are found ineligible) ,
the County covers the residential care payments to the facility operators through the
County's established Mental Health Program Funding. Initially, ten to twenty percent of
the clients are found .ineligible by the Federal Social Security Administration, depending
on changes in Federal guidelines, and then some of these are ultimately found eligible
through appeal processes.
BACKGROUND/RBASON(S) FOR RBCOMM»NDATIQN_(8) :
On March 7, 2000, the Board of Supervisors approved Emergency Residential Care Placement
Agreement #24-085-118(2) with Fred Williams (dba Williams Board and Care) to provide
emergency residential care to mentally disturbed adults who are potentially eligible for
SSI/SSP, for the period from January 1, 2000 through June 30, 2000.
Approval of Contract #24-085-118 (3) will allow the Contractor to continue providing
services through June 30, 2001.

SUBJECT: Explosive Ordnance Disposal Contract With Alameda County
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
AUTHORIZE the Sheriff or his designee to execute on behalf of the COUNTY a contract with
Alameda County for up to $15,000 each year to secure the services of the Alameda County
Sheriffs Office Explosive Ordance Disposal (EOD) Squad on an as-needed basis between April
6, 2000 and April 5, 2002.
FISCAL IMPACT:
NONE. Funds are available in the Sheriffs Office budget request to support the cost of this
contract.
BACKGROUND;
The,County has historically contracted with other law enforcement agencies to secure Explosive
Oronce Disposal services. The frequency of incidents requiring this disposal within the County
has not justified the establishment of such a Unit within the Sheriffs Office. Services of a Unit of
trained, certified specialists is available through Alameda County.
