SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #27-332-1 with
Elizabeth Odumakinde, M.D. , for the period from April 1, 2000 through. March
31, 2001, for the provision of professional oncology/hematology services for
the Contra Costa Health Plan, to be paid as follows :
a. For Medi-Cal and Commercials Members:
County shall pay Contractor those rates set forth in the Medi-Cal Schedule
of Maximum Allowances in effect on August 1, 1998. In the event rate
increases are subsequently approved by the State of California and are
included in the County's Health Plan capitated payment, County will
thereafter increase the rates County pays to Contractor accordingly.
b. For Medicare members:
Services for members who are Medicare recipients will be reimbursed at the
Medicare rate of payment. Physician will bill Medicare as primary payor and
County will pay Medicare--required copayments and deductibles for Medicare
approved services.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health 'Plan (Health Plan) member
premiums . Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION{S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
Under Contract #27-332-1, the Contractor will provide professional
oncology/hematology services to Contra Costa Health Plan members, through
March 31, 2001 .

RECOMMENDATIOS} :
Approve and authorize the Health Services Director or his designee
(Donna Wigand) to execute on behalf of the County, Novation Contract
#24-818-5 with Bonita House, for the period from April 1, 2000
through March 31, 2001, in the amount of $127, 837, for the provision
of dual diagnosis social rehabilitation residential services . This
contract includes a six-month automatic contract extension through
September 30, 2001, in the amount of $69, 919 .
FISCAL IMPACT:
This Contract funded as follows :

BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
it is estimated that as many as 80% of persons with a serious mental
illness also have a substance abuse problem, making this contract
crucial under managed care . Bonita House is the only program of its
type in the Bay Area and has received national recognition.
On April 20, 1999, the Board of Supervisors approved Novation
Contract #24-818-4 with Bonita House, for the period from April 1,
1999 through March 31, 2000, which included a six-month automatic
contract extension through September 30, 2000, for the provision of
dual diagnosis social rehabilitation residential services.
This Novation Contract #24-818-5 replaces the six-month automatic
contract extension under the prior Contract, and allows Contractor
to continue providing services through March 31, 2001 .

RECOMMENDATIONS

for the Department' s Information Systems.
FISCAL IMPACT:

BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On May 18, 1.999, the Board of Supervisors approved Contract
#23-244-2 with DB Specialists, to provide temporary computer
programmers/analysts for the Department' s Information Systems, for
the period from July 1, 1999 through June 30, 2000 .
Approval of Contract #23-244-3 will allow the Contractor to continue
providing services through June 30, 2001 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Contract
#26-961-1 with the Women' s Cancer Center (Specialty: Obstetric/
Gynecology) , in the amount of $40, 000, for the period from May 1, 2000
through April 30, 2001 .
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics .
On July 20, 1999, the Board of Supervisors approved Contract #26-961
with the Women' s Cancer Center, for the period from May 1, 1999
through April 30, 2000, to provide Obstetric and Gynecology surgical
services, including, but not limited to pre-operative and post-
operative care of patients, twenty-four hour on-call coverage
services, and in-service training for Registered Nurses at Contra
Costa Regional Medical Center and Contra Costa Health Centers .
Approval of Contract #26-961.-1 will allow Contractor to continue
providing services through April 30, 2001 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee (Frank
Puglisi, Jr. ) , to execute on behalf of the County, Agreement #26-023-65 with
the U.S. Department of Veterans Affairs, Northern California Health Care
System (VANCHCS) , for the period from January 1, 2000 through December 31,
2000, to be paid in accordance with the attached fee schedule.
FISCAL IMPACT`
This Contract is funded in the Health Services Department' s Enterprise I
Budget. The services provided for the County's patients under this Contract
are billable to patients and third party payors.
VANCHCS will pay County a fee for service in accordance with the attached
fee schedule.
BACKGROUNDIREASON(S) FOR RECOMMENDATION(S) :
For many years the County and U. S. Department of Veterans Affairs have
maintained a mutual sharing agreement which has made available to the County
specialized medical services not otherwise available due to lack of
resources, equipment, and personnel. These services included specialized
laboratory testing, radiology services, nuclear medicine studies, CT scans,
MRI 's, dermatology, gastroenterology, urology, audiology and speech, and
ophthalmology services. The County provided Emergency Room treatment and
inpatient care, including certain ancillary services, for VA-referred
patients.
Approval of Contract #26-023-65 will allow Contra Costa Regional Medical
Center to provide Emergency services, and Inpatient and Outpatient medical
and surgical services to patients referred by VANCHCS, through December 31,
2000.
The Contract documents have always been prepared by the Veterans
Administration and require final approval from the Department of Veterans
Affairs in Washington, D.C.

Contra Costa Regional Medical Center(CCRMC), shall provide Healthcare Services, as defined
herein, to beneficiaries of the Department of Veterans Affairs (VA) in accordance with the
reimbursement schedule identified below:

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Contract
Amendment Agreement #22-601-5 with Roger Way, effective April 1,
2000, to amend Contract #22-601-2 (as amended by. Contract Amendment
Agreements #22-601-3 and #22-601-4) to increase the Contract Payment
Limit by $10, 000, from $35, 000 to a new Contract Payment Limit of
$45, 000, and extend the term through May 31, 2001.
FISCAL IMPACT•
This Contract is 100% County funded.
BACRGROUNDIREASON(S) FOR RECOMMENDATION(S) :
In July, 1998, County Administrator approved and the Purchasing
Services Manager executed Contract #22.601-2 (as amended by Contract
Amendment Agreements #22-601-3 and #22-601-4) with Roger Way, for
the period from June 1, 1998 through May 31, 2000, for the provision
of consultation and technical assistance to the department with
regard to fundraising for the Center for Health in North Richmond.
Approval of Contract Amendment Agreement #22-601-5 will allow the
Contractor to provide additional hours of service through May 31,
2001.
