FISCAL IMPACT This Contract is funded as follows: $123 , 250 Federal Medi-Cal funds $ 65, 040 Federal Adolescent Treatment Program funds $117 ,239 State EPSDT funds $305,529 Contract Payment Limit CHILDREN'S IMPACT STATEMENT: This Dual-Diagnosis Program will support the following Board of Supervisors ' community outcomes: "Children Ready For and Succeeding in School"; "Families that are Safe, Stable, and Nurturing" ; and "Communities that are Safe and Provide a High Quality of Life for Children and Families" . Expected program outcomes include an increase in positive social and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CAFAS) and Youth Self Report (YSR) and a decrease in juvenile offender recidivism as measured by probation database information. BACKGROUNDLREASON(S) FOR RECOMMENDATION(S) : This Contract meets the social needs of County's population in that it provides day treatment and mental health services to dually--diagnosed adolescents and their families in order to reduce unsafe behavior, improve school attendance and performance, avoid out-of-home placement, and deal with emotional problems. Under Contract #74-063 , New Connections will provide dual-diagnosis treatment services to SED adolescents admitted into the West County Youth Drug Court through June 30, 2001. ECHO Consulting Services of California, Inc. , is the sale provider of support services for this software. On January 18, 2000, the Board of Supervisors approved Contract #23•-274 with ECHO Consulting Services of California, Inc. , for a hardware and software maintenance agreement supporting the Department's Mental Health and Community Substance Abuse Services Divisions, for the period from October 1, 1999 through June 30, 2000. Approval of Contract #23-274-1 will allow the Contractor to continue providing services through June 30, 2001. SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMNNDATION(S} : Approve and authorize the Health Services Director, or his designee {Donna Wigand} to execute on behalf of the County, Novation Contract #24-935-3 with Crestwood Behavioral Health, Inc. , in the amount of $109,500, for the period from July 1, 2000 through June 30, 2001, for the provision of mental health treatment services for Contra Costa County geriatric clients at Crestwood Geriatric Center in Fremont. This Contract includes a six-month automatic extension through December 31, 2001, in the amount of $54,750. FISCAL IMPACT: This Contract is funded by Mental Health Realignment. BACXGROU- D f REASON t S) FOR RECO1Q4-ENDATION(S) : This Contract is vital to the Department's Geriatrics Mental Health Program because it provides an alternative to both Morton Bakar Center and Napa State Hospital for geriatric clients in need of longer term psychiatric treatment in a skilled nursing setting. Adults, age 65 and over, who require such treatment for more than two years will be transferred from Morton Bakar and Napa to Crestwood Geriatric Center in Fremont, On August 3, 1999, the Board of Supervisors approved Novation Contract #24-935®2 with Crestwood Behavioral Health, Inc. , for the provision of mental health treatment services, for the period from July 1, 1999 through June 30, 2000 which included a six-month automatic extension through December 31, 2000. Approval of Novation Contract #24-935-3 replaces the automatic extension of the prior Contract and allows the Contractor to continue providing services through June 30, 2001. FISCAL IMPACT: Source of funding for this Contract is CSA BM-1 (Measure H) . BACKGRO11NL"l(REASON t S) FOR RECOMMENDATION(S) : In October 1996, the Board of Supervisors authorized the Bethel Island Fire Protection District, in conjunction with the Department' s Emergency Medical Services, to undertake a one-year pilot paramedic program utilizing part-time paramedic/firefighters as non-transporting first responder personnel . The Board of Supervisors subsequently approved agreements which allowed the continuation of this program through June 30, 2000 . This program has been well received by the community within the Bethel Island Fire Protection District and funding is available for the 2000-2001 Fiscal Year. Approval of Agreement #23-208-4 will allow these services to continue through June 30, 2001 . SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION(S) Ratify purchase of services from Medical and Behavioral Consultants, Inc. , and authorize the County Auditor-Controller to pay the $5, 678 . 04 outstanding balance for provision of psychiatric services for the Mental Health Division. FISCAL IMPACT: Funding for this Contract is included in the Health Services Department budget . BACKGROUNDIREASON(S) FOR RECOMMENDATION(S) : On May 16, 1999, the Board of Supervisors approved Contract #24-934-1 (as amended by Contract Amendment Agreement #24-934-2) with Medical and Behavior Consultants, Inc. , for the period from March 1, 1999 through February 29, 2000, to provide psychiatric services for the Mental Health Division. The Division of Mental Health has had difficulty in maintaining psychiatric services for Juvenile Hall clients . Medical and Behavioral Consultants, Inc. has had to provide full-time pager coverage in order to provide psychiatric coverage for these clients . This pager coverage is in addition to the clinic responsibilities provided by the Contractor. Unfortunately, due to the higher anticipated volume of pager coverage of which services were requested by County staff and provided by the Contractor in good faith, use of the Contractor' s services exceeded the authorized limits. Services were requested and provided beyond the payment limit and by the end of February, 2000, charges of $156, 678 had been incurred, of which $150, 040 has been paid and $6, 678 remains outstanding. The Department is requesting that the amount due to the Contractor be paid. This can be accomplished by the Board of Supervisors ratifying the actions of the County employees in obtaining provision of services of a value in excess of the contract payment limit . This will create a valid obligation on the part of the County, retroactively authorizing all payments made by the Auditor-Controller up to now, and authorizing payment of the balance. However,the Division of Mental Health had to continue to provide psychiatric coverage for Juvenile Hall clients, The Division of Mental Health is,therefore,requesting approval of this ane-time only retroactive payment in the amount of$6,678 to cover the cost of providing psychiatric coverage for Juvenile Hall clients.
