RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr, ) to execute on behalf of the County, Contract
#26-965-1 with Dennis Zai, M.D. (specialty: Endoscopy - Internal
Medicine) , for the period from June 1, 2000 through May 31, 2003, to
be paid in accordance with the attached fee schedule.
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics .
On August 3 , 1999, the Board of Supervisors approved Contract #26-965
with Dennis Zai, M.D. , Inc. , for the period from June 1, 1999 through
May 31, 2000, to provide Endoscopy services, including on-call and
clinical coverage services for Contra Costa Regional Medical Center
and Contra Costa Health Centers .
Approval of Contract #26-965-1 will allow the Contractor to continue
providing services through May 31, 2003 .

Psychiatry) , in the amount of $81, 272, for the period from March 1, 2000
through February 28, 2001, for the provision of professional psychiatric
services .
FISCAL IMPACT:
This Contract is 100% funded by Mental Health Realignment funding, off-set
by third party billing.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On March 16, 1999, the Board of Supervisors approved Contract #24-934-1 (as
amended by Contract Amendment Agreement #24-934-2) , with Medical and
Behavioral Consultants, Inc . , for the period from March 1, 1999 through
February 29, 2000, for the provision of professional psychiatric services,
including on-call coverage services at County' s Juvenile Hall Facility.
Approval of Contract #24-934-3 will allow the Contractor to continue
providing services, through February 28, 2001 .

RECOMMENDATION(S) :
Approveand authorize the expenditure of an amount not to exceed $17, 000 for the
period from July 1, 2000 through June 30, 2001, to fund the Department ' s Mental
Health Division' s continued participation in U.C. Berkeley' s College Work-Study
Program under Agreement #24-252-1 with the University.
FINANCIAL IMPACT:
The Paid Work-Study Program is funded 50%5 by the participating agency (County)
and 50%- by the University of California, Berkeley.
Funding for the County' s portion of this Agreement is included in the
Department ' s Fiscal Year 2000-2001 Budget and is funded by State and Federal
Funds . The County Administrator approved and the Purchasing Agent executed a
payment limit of $17, 000 for Fiscal Year 1599-2000, and approval of the Fiscal
Year 2000-2001 payment limit of $17, 000 will result in a total two-year payment
of $34, 000 for this second-year Contract which expires on June 30, 2001 .
Expenditure of the total payment limit under this Agreement will depend upon the
Department ' s utilization of students participating in the program during the
term of the Agreement .
BACKGROUND/REASON(S) FOR RECOMMENDATIONS} :
The Health Services Department has participated in the College Work-Study
Program with the University of California, Berkeley since 1982 . This program,
which is jointly funded by the County and the University, provides the County
with the services of University students who perform part-time work at very
reasonable hourly rates, and provides the students with funds needed to pursue
higher education.
1n June, 1999, the County Administrator approved and the Purchasing Agent
executed funding for the College Work-Study Program Agreement #24-252-1 with the
University of California, Berkeley for the period, from July 1, 1999 through
June 30 2000 .
The purpose of this Board Order is to provide continued funding for the Program
through' FY 2000-2001, so that University students may continue to provide
services for the Health Services Department through June 30, 2001 .

a. Approve and authorize the Health Services Director or his designee (Donna M. Wigand)
to execute on behalf of the County Emergency Residential Care Placement .Agreement
#24-066-112 (4) with Bonita House, Inc. , for the period from July 1, 2000 through
June 30, 2001, to provide emergency residential care to mentally disturbed adults
who are potentially eligible for SSI/SSP, in an amount not to exceed $847 per client
per month through December 31, 2000, and in an amount not to exceed $890 per client
per month beginning January 1, 2001; and
b. Authorize the Auditor-Controller to pay an amount not to exceed $174 per month
through December 31, 2000, and an amount not to exceed $183 per month beginning
January 1, 2001, to clients who are placed in the board and care facilities under
said Emergency Residential Care Placement Agreements for incidental expenses and
personal needs.
Fxsc" IMPAC :
Under this program, the County pays the State-established rates for the residential care
of mental health clients who are not vet receiving SSI/SSP (but who are potentially
eligible and have made formal application to the Social Security Administration for
SSI/SSP) , contingent upon the client's signing an agreement with the County to reimburse
the County from the SSI/SSP money, once it is finally granted by the federal government
(usually 3 to 6 months retroactively back to the month of application) .
For those mental health clients who are not granted SSI/SSP (i.e. , are found ineligible) ,
the County covers the residential care payments to the facility operators through the
County's established Mental Health Program Funding. Initially, ten to twenty percent of
the clients are found ineligible by the Federal Social Security Administration, depending
on changes in Federal guidelines, and then some of these are ultimately found eligible
through appeal processes.

APPROVE the Agreement between the Contra Costa County Employee Retirement Board (CCCERA) and
Contra Costa County regarding prepayment of annual contributions and DIRECT the County Administrator or
his designee to execute the agreement.
BACKGROUND:
Pursuant to Government Code Section 31582,the County has the ability to prepay its annual contributions to
the County's Retirement Board;'in exchange for making prepayment,the Retirement Board offers the County
a discount on its annual conWbution. For FY 2000.01, this discourt4actor will equal .924 of the actual
contribution requirement. This arrangement has proven financially advantageous to both parties over the past
Several years.
A previous agreement between the County and CCCERA required the County to submit payroll reports to
CCCERA"as soon as practical'at the end of the fiscal year. The new agreement specifies that the County
has up to 45 days'to provide reconciled payroll reports to CCCERA to allow fora truing up"of costs for the
year.
KCAL IMPACT:
There should be no fiscal impact associated with this new agreement so long as the Auditor-Controller's office
provides reconciled payroll reports to CCCERA within 45 days of the end of each fiscal year.

REGARDING PREPAYMENT OF ANNUAL CONTR.IBBUTI -----------

--Contra Costa County would Iike to prepay its annual contribution for fiscal year 2000-01 in one
lump sum rather than on a monthly basis thoughout the year. Any such prepayment will be
pursuant to Govt.code Section 31582 of the County Employees Retirement Law of 1937 and its'
amendments.
The prepayment may or may not include the County's subvention of employee required
contributions.
IT IS AGREED THAT:
1. With regard to any prepayment of Costs under Govt.Code Section 31582:
a) Prepayment will be made on or before the last working day in July to be applied against
that fiscal year's Costs.
b) The prepayment amount will be computed using the most recently adopted contribution
rates and assumed County payroll, and will be discounted by a fraction the numerator of
which is one(1)and the denominator of which is the sum of one(1)plus the assumed
interest rate. For the Fiscal year 2000-2001,this discount factor will equal .924(1
divided by 1.0825).
c) This prepayment is intended to be the County's estimated full contribution for the fiscal
year.
2. As soon as practical,but no later than 45 calendar days after the end of the fiscal year,a
"true-up"will be done by the Retirement System(CCCERA). If the"true-up"cannot be
completed within the 45 day period because the County has not provided a reconciled
contribution report through the last payroll of the fiscal period by July 31 pt,an additional
interest charge on underpayments shall be paid to CCCERA at the actuarial assumed interest
rate. Interest will be computed from the first day of the new fiscal year to the date of receipt
of the final reconciled contribution report plus one-day. If all the reconciled contribution
reports are provided to CCCERA by the required due dates and CCCERA does not provide a
"true-up"within 45 days,CCCERA will pay interest on overpayments at the actuarial
assumed interest rate up to the date of the"true-up".
,Any difference between what the County prepaid,before applying the discount factor,and
what the County should have paid once actual payroll amounts are known,will be handled as
follows.
a) If the prepayment, before applying the discount factor,is less than the actual amount that
should have been paid,the County will be required to pay the difference within 30 days
of notification.
b) If the prepayment, before applying the discount factor,is greater than the actual amount
that should have been paid,the difference will be applied as a credit against the next
year's prepayment.

program alternatives. The preliminary program study for the preschool facility and program

I. Effective Date and Parties: Effective May 16,2000, George Miers&Associates(herein called"Consultant"),a California Corporation,and the
County of Contra Costa(herein called"County"),a political subdivision of the State of California,mutually agree as follows:
2. Purpos : On May 26, 1999,the parties entered into a contract entitled"Consulting Services Agreement,"referred to as the "Agreement," which
covers Feasibility Studies and ProgramPlanning for Application and Permit Center,Glacier Drive,Martinez. The Agreement has previously been
amended as follows: NONE. Tlarties desire to amend the Agreement to expand the scope of service and to increase the payment limit
accordingly.
3. Amendments to Agreement:
A. In the Agreement, Section 1(c), change the Payment Limit from$20,500.00 to $30,545.00, an increase of$10,045.000. This change shall
apply to charges from and after December 1, 1999.
B. In accordance with Section 14 of the Agreement,provide the following extra services:
1. Attend additional planning meetings with Public Agency.
2. Provide additional program reviews, site plan studies, civil engineering services and additional program alternatives. The preliminary
program study for the preschool facility and program development for the Public Works Construction Group are included in these
additional services.
C. In Paragraph 1(F),change the Completion Date from December 31, 1999 to May 31,2000.

SPECIFIC REQUESTS OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
I. RECOMMENDATION
A. APPROVE a Third Amendment authorizing extra work under the Consulting Services Agreement
dated January 13, 1999, with Daniel Rainey, Architect for architectural services for Court Security
Improvements at Various County Sites. The Third Amendment increases the scope of services and
increases the payment limit from$74,540.58 to $101,295.55, an increase of$26,755.00.
B. AUTHORIZE the Director of General Services to execute the Third Amendment to the consulting
services agreement and to issue written authorization for extra work, in addition to the authorizations
for the First and Second Amendments, provided that the cumulative total extra cost for such additional
authorizations shall not exceed$10,000.00.
II. FINANCIAL IMPACT
Sufficient funds are available in the plant acquisition account to cover this increase in the Consultant's
payment limit.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND
A. On February 2, 1999 the Board of Supervisors approved and authorized the Director of General
Services to execute the original consulting services agreement with the Consultant. The First
Amendment increased the scope of services to provide additional architectural services and ADA
modifications for Sally Port at 45 Civic Avenue, Pittsburg and the review and coordination of
mechanical and electrical drawings for the Court Security Improvements at Various County Sites.

perform construction administration at 640 Ygnacio Valley Road, Walnut Creek.
2. Additional architectural services, structural engineering services and electrical services at 1020
Ward Street,Martinez due to unforeseen site conditions.

THIRD AMENDMENT TO CONSULTING SERVICES AGREEMENT
FOR COURT SECURITY IMPROVEMENTS AT VARIOUS COUNTY SITES

1. Effective Date and Parties: Effective May 16,2000, Daniel Rainey,Architect(herein called"Consultant"),a Sole Proprietor,and the County of
Contra Costa(herein called"County"),a political subdivision of the State of California,mutually agree as follows:
2. filrpos : On January 13, 1999,the parties entered into a contract entitled"Consulting Services Agreement,"referred to as the"Agreement,"which
covers architectural services for Court Security Improvements at Various County Sites. The Agreement previously has been amended as follows;
First Amendment dated May 18, 1999,Second Amendment dated January 4,2000. The parties desire to further amend the Agreement to expand the
scope of service and to increase the payment limit accordingly.
3. Amendments to Agreement:
A. In the Agreement, Section 1(e), change the Payment Limit, as modified by the First and Second Amendments from $74,540.58 to
$101,295.58,an increase of$26,755.00.This change shall apply to charges from and after December 1, 1999.
B. In accordance with Section 14 of the Agreement,provide the following extra services:
1. Architectural services and structural engineering services for review of contractor's proposals and perform construction administration at
640 Ygnacio Valley Road,Walnut Creek.
2. Additional architectural services, structural engineering services and electrical services at 1020 Ward Street,Martinez due to unforeseen
site conditions.
4. Effect:
Subject to the revisions made by this and any prior amendments,the Agreement shall remain in full force and effect.

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
APPROVE and AUTHORIZE the Sheriff-Coroner, or designee, to sign a contract on behalf of
the County with Sentencing Concepts, Inc. in the amount of$25,000 for the continued provision
of private Electronic Home monitoring services for the period July 1, 2000 to June 30, 2001.
FISCAL IMPACT:
The cost of this contract is paid by Custody Alternative facility program fees; the unit cost is
unchanged from the 1999-2000 contract. There is no impact on the County General Fund.
BACKGROUND:
Certain inmates referred to the Office of the Sheriff for out-of-custody programs cannot be
accommodated due to their residing outside Contra Costa County.
Section 1203.016 of the Penal Code authorizes the Sheriff,with the Board of Supervisors'approval,
to enter into a contract with a private company to provide electronic home monitoring services
outside the County.
The County has been contracting with Sentencing Concepts for several years to provide these
services. The current contract expires June 30, 2000.
CONSEQUENCE OF NEGATIVE ACTION:
The Sheriffs Office does not have the internal capacity to monitor individuals in out-of-custody
outside Contra Costa County. Therefore, these low risk inmates would have to be to retained in
one of the detention facilities at greater cost to the County.

RECOMMENDATION(S):
APPROVE and AUTHORIZE the County Administrator, or designee, to execute a contract with
Resource Development Associates, Inc. (RDA) in the amount of $25,000 for development of the
Children and Family Data Archive for the period from May 1, 2000 through April 30, 2001.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
The Children and Family Data Archive is part of the CCFutures project authorized by the Board of
Supervisors. CCFutures was conceived to support the move toward increased investment in
early intervention and prevention services for children and families.
CCFutures is being developed primarily under the financial support of the East Bay Community
Foundation and the S. H. Cowell Foundation with a $50,000 County share. Currently, the
County's share provides clerical and administrative support to the project. However, there is
mutual agreement that it would be possible to reduce the scope of clerical and administrative
support so that a portion of the County's match could be allocated to further develop the Children
and Family Data Archive.

The Data Archive will be a linked, aggregate, anonymous data base which eventually will include
infonriat on from various public agency systems, such as CaIWORKs, Child Welfare Services, Public
Health, Maternal Child Health, Juvenile Probation, Public Housing, School Districts, Sheriff and
Police data as well as contextual data such as the location of public facilities and demographic data.
Each of member agencies will able to access the data through an "inquiry driven" software program
available to the participating agencies. The software program will perform crosstabs and
geographical and time-based analysis. development of the Data Archive will be under the guidance
of the County Administrator's Office, supported by a data user group composed of representatives of
all data contributors and will report to the Policy Forum Executive Committee.
The Data Archive is currently in a "prototype" stage of development. This contract will allow the Data
Archive to further receive data from the County's system as well as to clarify that RDA is an "agent"
of the County.
Status of the CCFutures project, including the Data Archive, was reviewed by the Family and Human
Services Committee at their May 8, 2000 meeting.
