SUBJECT: Approval of Contract #22-526-3 with Contra Costa Child Care Council
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)A BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director or his designee (Wendel
Brunner, M.D. ) to execute on behalf of the County, Contract #22-526-3 with
Contra Costa Child Care Council, in the amount of $25, 017, for the period
from March 1, 2000 through February 28, 2001, to provide nutrition education
and outreach services.
FISCAL IMPACT:
100% Federal funds.
CHILDREN'S IMPACT STATEMENT:
This Nutrition Program supports County's "Children Ready for and Succeeding
in School" and "Children and Youth Healthy and Preparing for Productive
Adulthood" community outcomes by providing nutrition and physical activity
training to 400 child care providers, serving 2000 low-income families who
are receiving federal food assistance grants. Expected outcomes include an
increased adherence to dietary guidelines for healthy eating and increased
routine physical activity and exercise.
BACKGROUNDLREASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of County's population in that it
improves the quality of life of low-income families with children by
reducing the risk of, and incidence of chronic diseases related to issues
of diet and physical activity.
In April 1999, the County Administrator approved and the Purchasing Services
Manager executed Contract #22-526-2 with Contra Costa Child Care Council,
for the period from February 15, 1999 through February 29, 2000, for the
provision of nutrition education and outreach services.
Approval of Contract #22-526-3 will allow the Contractor to continue
providing services through February 28, 2001.

Veterans Affairs
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Frank
Puglisi, Jr. ) , to execute on behalf of the County, Contract #26-343-7
(V261S-0424) with the U.S . Department of Veterans Affairs Northern
California Health Care System (VANCHCS) , in the amount of $194 , 705 for the
provision of Ophthalmology services, for the period from January 1, 2000
through September 30, 2002 . In accordance with the term of the agreement,
County agrees to indemnify and hold harmless Contractor from claims, losses,
damages, liabilities, costs, expenses or obligations arising out of, or
resulting from CCRMC' s wrongful or negligent conduct in the performance of
this Contract .
FINANCIAL IMPACT:
The Agreement is funded in the Health Services Department ' s Enterprise I
Budget . Under the term of the agreement, VANCHCS is paid a fee for service
in accordance with the fee schedule which is incorporated in the Contract .
The services provided for the County' s patients under this Contract are
billable to patients and third party payors.
REASONS FOR RECOMMENDATIONSIBACKGROUND:
For many years the County and U.S. Department of Veterans Affairs has
maintained a mutual sharing agreement which has made available to the County
specialized medical services not otherwise available due to lack of
resources, equipment, and personnel . These services included specialized
laboratory testing, radiology services, nuclear medicine studies, CT scans,
MRI ' s, dermatology, gastroenterology, urology, audiology and speech, and
ophthalmology services . The County provided Emergency Room treatment and
inpatient care, including certain ancillary services for VA-referred
patients .
Approval of Contract #26-343-7, will allow Contractor to continue providing
specialized ophthalmology medical services to County' s patients referred for
treatment by Contra Costa Regional Medical Center physicians through
September 30, 2002 .
The Contract documents have always been prepared by the Veterans
Administration.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Contract #26-
907-3 with Laszlo Tamas, M.D. , in the amount of $1, 006, 247, for the
provision of Neurosurgery Services, for the period April 1, 2000
through March 31, 2003 .
FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payors will be billed for services .
REASONS FOR RECOMMENDATIONS/BACKGROUND:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics .
Under Contract #26-907-3 , Laszlo Tamas, M.D. will provide professional
Neurosurgery services, including consultation, training, on-call
coverage, administration services, and Medical and/or surgical
procedures, for Contra Costa Regional Medical Center and Contra Costa
Health Centers through March 31, 2003 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #27-427-1 with Kevin
Beadles, M.D. , for the period from February 1, 2000 through January 31,
2001, for the provision of professional ophthalmology services for the
Contra Costa Health Plan, to be paid as follows :
a. For Medi-Cal and Commercials Members:
County shall pay Contractor those rates set forth in the Medi-Cal Schedule
of Maximum Allowances in effect on August 1, 1998. In the event rate
increases are subsequently approved by the State of California and are
included in the County's Health Plan capitated payment, County will
thereafter increase the rates County pays to Contractor accordingly.
b. For Medicare members:
Services for members who are Medicare recipients will be reimbursed at the
Medicare rate of payment. physician will bill Medicare as primary payor and
County will pay Medicare-required copayments and deductibles for Medicare
approved services.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member
premiums . Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On April 27, 1999, the Board of Supervisors approved Contract #27-427 with
Kevin Beadles, M.D. , for the period from February 1, 1999 through January
31, 2000 .
Approval of Contract #27-427-1 will allow the Contractor to continue to
provide professional ophthalmology services to Contra Costa Health Plan
members, through January 31, 2001 .

Approve and authorize the Health Services Director, or his designee (Milt Camhi) ,
to execute on behalf of the County, Contract #27-261-=3 with Sharon Drager, M.D. ,

professional vascular surgery services for the Contra Costa Health Plan, to be
paid as follows :

RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee (Milt
Camhi) to execute on behalf of the County, Contract #27-455 with Carlos
Zapata., M.D. , for the period from April 1, 2000 through March 31, 2001, for
the provision of professional psychiatric services, to be paid as follows :
' 100.00 per initial 45-50 minute therapy session; and
70 .00 for each additional 45-50 minute therapy session.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums . Costs
depend upon utilization. As appropriate, patients and/or third party
payors will be billed for services.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Contra Costa Health Plan (Health Plan) has an obligation to provide
professional psychiatric services for Health. Plan members with mental
health therapy services as a covered benefit . This population includes
Medi-Cal, Medicare, and Commercial members enrolled in the Health Plan.
Under Contract #27-465, the Contractor will provide professional outpatient
psychotherapy services for Health Plan members, through March 31, 2001 .
