SUBJECT: APPROVE and AUTHORIZE the Employment & Human Services Director, or designee, to
EXECUTE a contract extension (#19-6001-2) with the Alameda County Private Industry
Council (part of the Regional Workforce Preparation & Economic Development Act Grant

September 30, 2000 with no change in payment limit.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment & Human Services Director, or designee, to EXECUTE a
contract extension (#19-6001-2) with the Alameda County Private Industry Council (part of the Regional
Workforce Preparation & Economic Development Act Grant collaborative) for Welfare-to-Work services to
eligible clients from January 1, 2000 to September 30, 2000 with no change in payment limit.
FINANCIAL IMPACT:
None. Activities under this agreement are funded by the State Regional Workforce Preparation and
Economic Development Act grant funds.
CHILDREN'S IMPACT STATEMENT:
Activities under this grant support the third community outcome - "Families that are Economically Self-
sufficient.”
BACKGROUND:
By its action on February 23, 1999, the Board of Supervisors approved execution of a contract with Alameda
County Private Industry Council (PIC) for the period July 1, 1998 to June 30, 1999 with a payment limit of
$139,360 for the latter agency to provide services to Welfare-to-Work eligible Ca1WORKs clients through
EASTBAY Works One-Stop Career Centers in their jurisdiction as part of a regional collaboration to provide an
integrated workforce development system.
On September 21, 1999, the Board authorized the extension of the contract with Alameda County PIC to
December 31, 1999. The State of California Employment Development Department further extended the first
year allocation to September 30, 2000. Extension of this agreement will allow Alameda County PIC to
continue providing services to Welfare-to-Work eligible clients as part of the regional collaboration.

RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment and Human Services Department Director, or designee, to
contract with Worldwide Educational Services, Inc. (#18-009-0) for direct services to Welfare-to-Work
clients in the amount of$68,640 for the period from October 1, 1999 through September 30, 2000.
FINANCIAL IMPACT:
No County Costs, The Governor's 15% Welfare-to-Work grant funds for special projects cover 100 percent of
the costs of this contract.
CHILDREN'S IMPACT STATEMENT:
This contract will directly support the third community outcome on the Children's Report Card, "Families
that are Economically Self-Sufficient". This contract will provide job retention and post-employment services
to Welfare-to-Work eligible clients residing in Concord's Monument Corridor.
BACKGROUND:
The Board authorized a contract with Worldwide Educational Services, Inc. for Welfare-to-Work direct
services on July 20, 1999 (Item C. 123). Funds identified in this action will augment those Welfare-to-Work
activities authorized by the Board last July. However, services paid for with these funds will specifically
target clients in the Monument Corridor of Concord.

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee (Donna
Wigand) , to execute on behalf of the County, Contract #24-681-19 (2) with
Jean Michaelides (dba Concord Boulevard Home) , in the amount of $36, 000, for
the period from May 1, 2000 through April 30, 2001, for the provision of
augmented board and care services for County-referred mentally disordered
clients.
FISCAL IMPACT:
Funding for this Contract is included in the Department' s Fiscal Year 2000-
2001 Budget and is funded 100% by Mental Health Realignment.
BACKGROUND1REASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of the County's population in that it
provides augmentation of room and board, and twenty-four hour emergency
residential care and supervision to eligible mentally disordered clients,
who are specifically referred by the Mental Health Program Staff and who are
served by County Mental Health Services.
On December 7, 1999, the Board of Supervisors approved Contract #24-681--
19 (1) with Jean Michaelides (dba Concord Boulevard Home) , for the period
from October 1, 1999 through April 30, 2000, to provide augmented
Residential Board and Care Services for County-referred mentally disordered
clients.
Approval of Contract #24-681-19 (2) will allow Contractor to continue
providing services, through April 30, 2001.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt Camhi) ,
to execute on behalf of the County, Contract #27-285-2 with William Jenkins, M.D. ,
for the period from April 1, 2000 through March 31, 2001, for provision
professional primary care services for Contra Costa Health Plan members, to be
paid as follows :
1. For Medt-Cal Beneficiaries:
a. County will pay Physicians for covered services,those rates set forth in the Medi-Cal Schedule of Maximum Allowances in effect on August 1, 1998 plus
5%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan capitated payment,
County will thereafter increase the rates County pays to Contractor accordingly.
b. County will pay a quarterly case management fee,as follows:
Panel Size* Quarterly Fee
1 to 499 $3.00 per beneficiary per quarter
500 to 999 $3.25 per beneficiary per quarter
1000 or more $3.50 per beneficiary per quarter
*Panel Size is the number of Medi-Cal beneficiaries receiving treatment by Contractor during each quarter as specified in the "CCHP's Community
Provider Network Primary Care Providers Compensation Plan".
2. For Healthy Family P'romram Members. County will pay Physicians for covered services,those rates set forth in the Medi-Cat Schedule of Maximum Allowances
in effect on August 1, 1998 plus 10%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan
capitated payment,County will thereafter increase the rates County pays to Contractor accordingly.
3. For Flan B Commercial Members. County will pay Physicians for covered services,those rates set forth in the Medi-Cal Schedule of Maximum Allowances in
effect can August 1, 1998 plus 20%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan
capitated payment,County will thereafter increase the rates County pays to Contractor accordingly.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums. Costs depend
upon utilization. As appropriate, patients and/or third party payors will be
billed for services .
BACKGROUND/REASON(S) FOR RECOMMMMATION(S) :
On February 1, 1997 the Local Initiative for Medi-Cal managed care in Contra Costa
County was implemented. Local Initiatives are required to include traditional
Medi-Cal providers from the community in their provider networks . This Contract
is necessary to meet State mandates to expand the number of community providers
for the Local Initiative, along with a Department of Corporations audit finding
that requires formal contracts with low volume providers .
Under Contract #27-285-2, the Contractor will provide professional primary care
services to Health Plan members through March 31, 2001 .

the provision of professional chiropractic services for Contra Costa Health
Plan members, to be paid as follows :

BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The .Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On July 20, 1999, the Board of Supervisors approved Contract #27-382-1 with
Victor Kerenyi, D.C. , for the period from June 1, 1999 through May 31,
2000 .
Approval of Contract #27-382-2 will allow the Contractor to continue to
provide professional chiropractic services, through May 31, 2001 .

to execute on behalf of the County, Contract #27-466 with Michael Lenoir, M.D. ,
for the period from May 1, 2000 through April 30, 2002, for provision of
professional primary care services for Contra Costa Health Plan members, to be
paid as follows :
1. For Medi-Cal Beneficiaries:
a. County will pay Physicians for covered services,those rates set forth in the Medi-Cal Schedule of Maximum Allowances in effect on August 1, 1998 plus
