RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Lew Pascalli, Jr. ) to execute on behalf of the County, Contract
#22-769 with Cho Nai Cheung, for the period from April 1, 2000
through March 31, 2001, in the amount of $81, 500, for consultation
and technical assistance to the Department ' s Hazardous Materials
Division regarding risk management and prevention programs .
FISCAL IMPACT:
This Contract is funded by the California Accidental Release
Prevention (CAL/ARP) Program revenues included in the Health
Services Department ' s Budget .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
Under Contract #22-769, Cho Nai Cheung will provide consultation and
technical assistance to the Departments ' s Hazardous Materials
Division with regard to stationary sources subject to the California
Accidental Release Prevention (CAL/ARP) Program and chemical
manufacturing businesses which handle acutely hazardous materials
through March 31, 2001 .

#26-920-7 with James A. Rael, M.D. , Inc . , for the period from April 1,
2000 through March 31, 2001, in the amount of $182 , 520, for the
provision of professional services as Medical Director of the County' s
Adult and Juvenile Detention Facilities .
FISCAL IMPACT:
This Contract is 100k County funded, and is included in the Health
Services Department ' s Enterprise Budget for the Detention Facilities .
BACKGROUND/REASON(S) FOR RECOMMENDATIONS)-:
On April 6, 1999, the Board of Supervisors approved Contract #26-=920-6
with James A. Rael, M.D. , Inc . , for the provision of professional
services as Medical Director of the County' s Adult and Juvenile
Detention Facilities, for the period from April 1, 1999 through March
31, 2000 .
Approval of Contract #26-920-7, will allow Dr. Rael to continue
providing services through March 31, 2001 .

RECOMMENDATIONS? :
Approve and authorize the Health Services Director, or his designee
(Patrick Godley) to execute on behalf of the County, Contract
#23-255-2 with Romax Development, LTD. , in the amount of $75,000,
for the period from March 1, 2000 through February 28, 2001 for the
provision of consultation and technical assistance to the Department
with regard to the Computer Output Laser Disc (COLD) Onbase System.
FISCAL IMPACT•
Funding for this Contract is included in the Health Services
Department's Enterprise I Budget.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
In March 1999, the County Administrator approved and the Purchasing
Services Manager executed Contract #23-255 (as amended by Contract
Amendment Agreement #23-255-1) with Romax Development, Ltd. , for the
period from February 15, 1999 through February 29, 2000, to provide
consultation and technical assistance to the Department with regard
to the Computer Output Laser Disc (COLD) Onbase System.
Approval of Standard Contract #23-255-2 will allow the Contractor
to continue providing services to the Department through February
28, 2001.

SUBJECT: Approval of Contract #27-287-3 with Louis Klein, M.D. , Inc.
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #27-287-3 with Louis
Klein, M.D. , Inc. , for the period from February 1, 2000 through January 31,
2001, for the provision of professional OB\GYN services for the Contra Costa
Health Plan, to be paid as follows :
a. For Nadi-Cal and Commercials Members:
County shall pay Contractor those rates set forth in the Medi-Cal Schedule
of Maximum Allowances in effect on August 1, 1998. In the event rate
increases are subsequently approved by the State of California and are
included in the County's Health Plan capitated payment, County will
thereafter increase the rates County pays to Contractor accordingly.
b. For Medicare members:
Services for members who are Medicare recipients will be reimbursed at the
Medicare rate of payment. Physician will bill Medicare as primary payor and
County will pay Medicare-required copayments and deductibles for Medicare
approved services.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member
premiums. Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On August 17, 1999, the Board of Supervisors approved Contract #27-287-2
with Louis Klein, M.D, Inc . , for the period from February 1, 1999 through
January 31, 2000 .
Approval of Contract #27-287-3 will allow the Contractor to continue to
provide professional OB/GYN services to Contra Costa Health Plan members,
through January 31, 2001 .

RECOMMENDATION(a) :
Approve and authorize the Health Services Director, or his designee
(Donna M. Wigand, LCSW) , to execute on behalf of the County, Contract
#24--887-3 with Willie Hardaway (dba Hardaway Family Care Home) , for
the period from March 1, 2000 through February 28, 2001, in the amount
of $28, 800, for the provision of augmented board and care services for
County-referred mentally disordered clients.
FISCAL IMPACT:
This Contract is 100% funded by County Mental Health Realignment .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Department' s residential support program, called Supported Adult
Group Enterprises (S.A.G.E. ) provides Augmented Board and Care
Services designed to meet the needs of the seriously persistently
mentally ill (SPMI) adults, who currently reside in locked facilities
and state hospitals . Hardaway Family Care Home is one of the few
board and care facilities that was able to meet the S .A.G.E. Program
requirements under the Department' s Request For Proposal .
On March 2 , 1999, the Board of Supervisors approved Contract ##24-887-2
with Willie Hardaway (dba Hardaway Family Care Home) , for the period
from March 1, 1999 through February 29, 2000, for the provision of
augmented board and care services for County--referred mentally
disordered clients .
Approval of Contract #24-887-3 , will allow the Contractor to continue
providing six (6) dedicated beds for use by County--referred clients,
through February 28, 2001 .
