RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #27-211-3 with Allan
Barza, M.D. , Inc . , for the period from January 1, 2000 through December 31,
2000, for the provision of professional psychiatric services for the Contra
Costa Health Plan, to be paid as follows :
1. $100.00 per inital 50 minute therapy session; and
2. 70.00 for each additional 50 minute therapy session.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member
premiums . Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On January 19, 1999, the Board of Supervisors approved Contract #27-211-2
with Allan Barza, M.D. , Inc . , for the period from January 1, 1999 through
December 31, 1999, to provide psychiatric services to Health Plan members .
Approval of Contract #27-211-3 , will allow the Contractor to provide
professional psychiatric services to Contra Costa Health Plan members,
through December 31, 2000 .

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee (Milt
Camhi) , to execute on behalf of the County, Contract #27-265-3 with Mark
Drucker, M.D. , for the period from February 1, 2000 through January 31,
2001, for the provision of professional ophthalmology services for the
Contra Costa Health Plan, to be paid as follows :
a. For Medi-Cal and Commercials Members:
County shall pay Contractor those rates Bet forth in the Medi-Cal Schedule
of Maximum .Allowances in effect on August 1, 1998. In the event rate
increases are subsequently approved by the State of California and are
included in the County's Health Plan capitated payment, County will
thereafter increase the rates County pays to Contractor accordingly.
b. For Medicare members:
Services for members who are Medicare recipients will be reimbursed at the
Medicare rate of payment. Physician will bill Medicare as primary payor and
County will pay Medicare-required copayments and deductibles for Medicare
approved services.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan) member
premiums . Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of their
Individual and Group Health Plan membership contracts with the County.
On April 20, 1999, the Board of Supervisors approved Contract #27-265-2 with
Mark Drucker, M.D. , for the period from February 1, 1999 through January 31,
2000 .
Approval of Contract #27-265-3 will allow the Contractor to continue to
provide professional ophthalmology services to Contra Costa Health Plan
members, through January 31, 2001 .

residential, community living, socialization, vocational and Medi-Cal

AACKGROUND/R ABO (8) FQR RECOMMENDATION t S :
This Contract meets the social needs of County's population in that it
provides a continuum of services to residents transitioning from crisis
residential care to transitional residential care and keeps these clients
out of higher levels of care, including State Hospitals.
On January 19, 1999, the Board of Supervisors approved Novation Contract
#24-751-63 (as amended by Administrative Amendment Agreement #24-751-64)
with Phoenix Programs, Inc. , for the period from July 1, 1998 through June
30, 1999 (which included a six--month automatic extension through December
31, 1999) for the provision of mental health program services for mentally
disturbed adults.
Approval of Novation Contract #24-751-65 replaces the six-month automatic
extension under the prior Contract and allows the Contractor to continue
providing services through June 30, 2000.

On February 23, 1999, the Board of Supervisors approved Contract
#27-221-2, with Tuong-Vi Ta, M.D. , Inc. , for the period from January
1, 1999 through December 31, 1999, for the provision of psychiatric
services.
Approval of Contract #27-221-3 will allow the Contractor to continue
providing mental health services to Health Plan members through
December 31, 2000.

(2) $ 95.00 for each of the first two (2) follow-up therapy sessions
for adolescents;

for children twelve (12) years of age and younger;

and

the request of the Interim Workforce Investment Board, to AMEND contracts with various Job

RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment & Human Services Director, or designee, at the request of
the Interim Workforce Investment Board, to AMEND contracts with various Job Training Partnership Act
service providers, in the amounts listed on Attachment A for the period from July 1, 1999 through June 30,
2000.
FINANCIAL IMPACT:
No County Costs. Funds affected in this action are the carry-over Federal Job Training Partnership Act grant
funds from fiscal year 1998-1999. These funds will augment fiscal year 1999-2000 JTPA funds to provide
services to JTPA clients in Contra Costa County.
CHILDREN'S IMPACT STATEMENT:
Services under this contract directly support the third community outcome identified in the Contra Costa
Children's Report Card: "Families that are Economically Self-Sufficient".
BACKGROUND:
On July 27, 1999 (Items C. 92 and C. 93), on August 17, 1999 (Item C. 154), and on September 21, 1999
(Items C. 150 and C. 151) the Board authorized execution of contracts with various service providers to provide
services to Job Training Partnership Act clients in Contra Costa County through the One-Stop Career Centers.
The contractors listed on the attached JTPA Carryover List provide services to JTPA clients in Contra Costa
County. The funds used to amend these contracts come from the unspent fur�ds,from last program year.

SPECIALITY HVAC INSTITUTE (#3019)
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGORUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Authorize the Director, Employment & Human Services Department, or his designee to:
(1) Execute on behalf of the County a standard form Vocational Training Agreement
(#3019) with Specialty HVAC Institute, 5250 East Second Street, Benicia, CA 94510 for the
term beginning February 15, 2000 and ending June 30, 2000 to provide vocational training to
individually referred Job Training Partnership Act (DTPA) participants for fees as specified in
said agreement; and,
(2) Execute additional standard form Amendment Agreements with the above referenced
institution as may be needed for periodic revisions, such as adding or deleting a training
course, changing the tuition fees or to comply with changes in State or Federal regulations.
IL FINANCIAL IMPACT:
None, one hundred percent federal funded.
111, CHILDREN IMPACT STATEMENT:
This contract is to provide vocational training to appropriate DTPA eligible participants. This
will support the third outcome— Families that are economically self-sufficient.
IV, REASONS FOR RECOMMENDATION:
Allocation of DTPA funds for County's overall DTPA Vocational Training Programs for
1999/2000 was approved by the Board on Ju e 2 , 1999.
