SUBJECT: Approval of Standard Agreement (Amendment) 429-775-3 with the State
of California, Managed Risk Medical Insurance Board
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee (Milt Camhi) ,
to execute on behalf of the County, Standard Agreement (Amendment) #29-775-3
(State #97MHF054, A3) , with the State of California, Managed Risk Medical
Insurance Board, effective May 1, 1999, for participation in the Healthy Families
Program.
FINANCIAL IMPACT:
This amendment will make technical adjustments to the fiscal provisions to
establish the process for reimbursing the Contra Costa Health Plan (Health Plan)
for the actual cost of services provided to Healthy Family members prior to the
effective date of coverage.
In accordance with Government Code Section 6254 the fiscal provisions attachment
shall remain confidential for a period of four years .
BACKGROUND/REASON_(S) FOR RECOMMENDATION(S)_:
On June 16, 1998, the Board of Supervisors approved Standard Agreement #29-775
(as amended by Standard Agreement (Amendment) #29-775-1 and #29-775-2) with the
State of California, Managed Risk Medical Insurance Board, for the period from
May 1, 1998 through June 30, 2000, for the County' s participation in the Healthy
Families Program. The Healthy Families Program provides low income children and
young adults through the age of nineteen with low cost health insurance. For a
minimal monthly premium, eligible children can enroll in the Contra Costa Health
Plan and receive full health care coverage .
Approval of Standard Agreement (Amendment) #29-775-3 will allow the Health Plan
to be reimbursed for casts of services prior for Health Family Program members
effective date of coverage and allow the Health Plan to continue to provide
health care coverage to Healthy Families Program members, through June 30, 2000 .
Three certified/sealed copies of this Board Order should be returned to the
Contracts and Grants Unit for submission to the State.

FINANCIAL IMPACT:
Per State Department of Education Agreement F2AP-9210 (Stage 2) County to receive funding in the amount of
$207,952.
BACKGROUND:
The Employment and Human Services Department has been contracting with the State Department of
Education (SDE) for over 25 years to provide child care services under the State General Fund Child Care
and Development Program. These funds serve families who receive Child Protective Services, teen parents,
children at risk of abuse and neglect, children with special needs and low income families.

SUBJECT: Approve Standard Agreement #28543 with the State Department of
Health Services for the Sexually Transmitted Disease Program
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his Designee (Wendel Brunner) ,
to execute on behalf of the County, Standard Agreement #28-543 (State #99-85524)
with the State Department of Health Services (Sexually Transmitted Disease Program) ,
in the amount of $25,074, for the period from January 1, 2000 through June 30, 2000,
for the Chlamydia Awareness and Prevention Project (CAPP) .
FINANCIAL IMPACT:
Approval of this Standard Agreement will result in $25,074 of State funding, for the
Chlamydia Awareness and Prevention Project (CAPP) through June 30, 2000. No County
funds are required.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
More than 58,700 cases of Chlamydia (CT) were reported in California in 1997.
However, due to under reporting and undiagnosed infections, experts estimate that
closer to 300,000 women and their partners are infected annually. The purpose of
this project is to facilitate, develop, and enhance local capacity for Chlamydia
(CT) awareness and prevention through a process of community-health department
collaboration and partnership.
Approval of Standard Agreement #28-543 will allow funding for this Chlamydia
Awareness and Prevention Project (CAPP) to focus on primary intervention of
Chlamydia infections and facilitate at-risk-individuals to seek testing through June
30, 2000.
Three certified/sealed copies of this Board Order should be returned to the
Contracts and Grants Unit.

State of California Department of Health Services

RECOMMENDATION(SI :
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) to execute, on behalf of. the County, Standard
Agreement (Amendment) #28-573-3 (State #96-26667 02) with the State
of California Department of Health Services, effective February 29,
2000, to increase the FY 1999/2000 Payment Limit by $120,203, from
$240,417 to a new fiscal year total of $360, 620, and to extend the
term through February 28, 2002 , for the Breast Cancer Partnership
Project.
FISCAL IMPACT:
Approval of this Standard Amendment (Amendment) will result in
$120,203 increased State funding for FY 1999-2000 and increase the
maximum amount of State funding through June 30, 2000, to a new
total of $1, 094, 570. No County funding is required.
BACKGRODNDIREASON(BS FOR RECOMMENDATION(S) :
On February 4, 1997, the Board of Supervisors approved Standard
Agreement #28-573-1 (State #96-26667) as amended by Standard
Agreement (Amendment) #28-573-2 , with the State of California
Department of Health Services, for the period from March 1, 1997
through February 29, 2000 for County's Breast Cancer Partnership
Project in an effort to reduce breast cancer mortality.
Approval of Amendment #28-573-3 will provide additional funding for
County's Breast Cancer Partnership Project for FY 1999-2000 and
extend the term of the agreement through February 28, 2002 .
Two certified and sealed copies of this Board Order should be
returned to the Contracts and Grants Unit.
