I. RECOMMENDATION
APPROVE Change Order No. 6 to the Contract dated June 15, 1999 for construction of Tenant Improvements at Los
Medanos Health Center, 2311 Loveridge Road, Pittsburg, with Len, Construction, Inc., Burlingame, California, and
AUTHORIZE the Director of General Services to execute the Change Order, which provides for support of existing
electrical conduits and junction boxes in accordance with current code requirements, replacement of existing corroded
mechanical system control valves, and changes to the installation of cable tray and conduit sleeves for telecommunications
wiring. This change order increases the Contract Price by a total of$373,379.00,and increases the contract completion time
by 81 days.
11. FINANCIAL IMPACT
The additional cost of this change order is within the budget recognized by the Board on November 16, 1999.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND
A. After construction began, the contractor discovered that many existing electrical conduits and junction boxes are not
supported in compliance with current code requirements. Change Order 6, Item 6-1 provides for this code-required
support to be installed on conduits and junction boxes affected by the work of this contract.
B. The original scope of work includes replacing the old, non-functional pneumatic mechanical control system with a
modern digital control system. This requires the replacement of the internal valve components at all existing mechanical
terminal units. During construction, however, the contractor discovered that many of the existing mechanical control
valve bodies were corroded and unusable. Change Order 6,Item 6-2 provides for the replacement of the existing control
valves with new at all existing mechanical terminal units. This item addresses some of the unanticipated mechanical
problems acknowledged by the Board on November 16, 1999.
C. The original scope of work includes installing new cable trays and conduit sleeves above the ceilings of all five floors
for new telecommunications wiring. During construction the contractor discovered that a significant portion of the
planned installation of the cable tray was in areas that were not accessible due to interference by existing utilities.
Change Order 6, Item 6-3 provides for changes to that installation so that the cable tray will be accessible, and adds
conduit sleeves to carry the wiring through inaccessible areas.

Item 6-1: Reference: QCQ No. 13, Proposal Request No. 11 dated December 10, 1999
Furnish and install all labor and materials required for the support of existing electrical conduits
and junction boxes in compliance with current code requirements. This work is required by the
County Building Inspection Department due to the lack of proper support of these existing items.
The work shall be done in accordance with the attached Proposal Request No. 11.
ADD: $ 41,602.00
Item 6-2: Reference: QCQ No. 25, Proposal Request No. 12 dated November 4, 1999
Furnish and install all labor and materials required for installation of new automatic control
valves at all mechanical terminal units. The original scope of work required only that the internal
valve components be replaced,but many of these valves were discovered to be badly corroded
and unusable. The work shall be done in accordance with the attached Proposal Request No. 12.
Extend the contract completion time by 81 days.

Item 6-3: Reference: QCQ No. 32, Proposal Request No. 15 dated November 5, 1999
Furnish and install all labor and materials required for altering the layout and installing
additional cable tray and conduit sleeves for the telecommunications cabling throughout the
building. These changes and additions are required due to the congested and inaccessible nature
of some of the ceiling spaces where the cable tray was originally to be installed. This condition
was not apparent until after demolition of the existing ceiling was underway. The scope of work
shall be in accordance with the attached Proposal Request No. 15. p (�/�

1. RECOMMENDATION
APPROVE Change Order No. 7 to the Contract dated June 15, 1999 for construction of Tenant
Improvements at Los Medanos Health Center, 2311 Loveridge Road, Pittsburg, with Lem Construction,
Inc., Burlingame, California, and AUTHORIZE the Director of General Services to execute the Change
Order, which provides for replacement of existing corroded mechanical system reheat coil connection
assemblies. This change order increases the Contract Price by a total of$402,120.00, and increases the
contract completion time by 52 days.
II. FINANCIAL IMPACT
The additional cost of this change order is within the budget recognized by the Board on November 16,
1999.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND
A. After construction began, the contractor discovered that many components of the existing
mechanical reheat coil connection assemblies on floors one through four were corroded and leaking
and therefore required replacement. (The fifth floor improvements were constructed in 1990, are
therefore 14 years newer than the rest of the facility, and the reheat coil connection assemblies on
that floor are in good condition.) Change Order 7 provides for the replacement of the existing reheat
coil connection assemblies with new on floors one through four.
B. This condition is similar to the damage found at the mechanical control valves and the central
mechanical plant as described in the February 5, 1999 report by Mechanical Consulting Company
and acknowledged by the Board on November 16, 1999.

The additional cost of this change order is within the budget recognized by the Board on November 16,

III. REASONS FOR RECOMMENDADONS/BACKGROILM
A. After construction began, the contractor discovered that many of the existing domestic water valves
are corroded, leaking and cannot be fully closed, and therefore require replacement.
B. The bulk of these valves are on floors one and three. Some of the valves in the first floor ceiling
space are currently feeding the second floor. Due to the small amount of space being utilized by
Health Services on the second floor at the conclusion of this project, the second floor will be
supplied by a new system. This means that the failed existing domestic water valves in the ceiling
space of the first floor that feed the second floor can be removed and the lines capped, rather than
replace these valves.
C. Change Order 8 provides for the replacement of existing domestic water valves with new on floors
one and three.

A. After construction began, the contractor discovered that several of the existing ceiling-mounted fire
dampers had damper blades that were coated with an asbestos-containing material. Clayton
Environmental, an industrial hygiene firm, analyzed the material and determined that it was
classified as hazardous waste under State and Federal regulations. Change Order 9, Item 9-1
provides for the abatement of this hazardous waste.
B. Demolition of the existing wall and ceiling finishes is required before replacement of the existing
failed cast iron soil and vent piping system can be undertaken. Change Order 9, Item 9-2 provides
for the demolition of these finish materials in accordance with the direction given by the Board on
November 16, 1999.

I. RECOMMENDATION
A. APPROVE a Second Amendment authorizing extra work under the Consulting Services
Agreement dated December 1, 1998, with RGM & Associates, for Tenant Improvements, Los
Medanos Health Center, 2311 Loveridge Road, Pittsburg, for Health Services Department. The
Second Amendment increases the scope of services and increases the Payment Limit, from
$412,423.12 to $578,877.99 an increase of$166,454.87.
B. AUTHORIZE The Director of General Services to execute the Second Amendment to the
consulting services agreement, and to issue written authorizations for extra work, in addition to
the authorization for the Second Amendment, provided that the cumulative total extra cost for
such additional authorizations shall not exceed$25,000.00.

A. The Director of General Services executed a consulting service agreement with RGM &
Associates effective December 1, 1998, for project management and technical support services
during the Tenant Improvements at Los Medanos Health Center, 2311 Loveridge Road,
Pittsburg, for Health Services Department.
B. The First Amendment increased the scope to authorize investigation of the heaving slab and cast
iron pipe failure, provide solution for repairs and provide construction administration during the
alterations and repairs.
C. The Second Amendment increases project scope to include:
1. Coordination of architectural design, contractor negotiations and construction administration
services for Second Floor Dental Suite.
2. Construction administration services required to coordinate with Health Services, Architect
and Engineers to evaluate structural and electrical requirements that will accommodate new
radiological equipment.
3. Construction administration services required to change mechanical system from the existing
"Constant Volume System"to a new"Variable Air Volume System".
4. Design, proposal evaluation and construction administration services for Central Plant
Improvements as a Change Order to original Construction Contract and as a separate bid
package. Separate bid package includes document review, site visits, preparation of bid
package, addendum, bid evaluation, contract award and construction management services.
5. Provide coordination efforts with equipment and furniture planning and inventory services
consultant.
6. Prepare separate bid packages and addenda, assist with bid evaluations and contract award,
and provide additional construction administration services, as required by the increase in
scope from that anticipated under the First Amendment for the cast iron soil pipe replacement
work.

I. Effective ate ant} arf#es: Effective March 21,2000,RGM&Associates(herein called"Consultant"),a corporation, and the County of Contra
Costa(herein called"County"),a political subdivision of the State of California,mutually agree as follows:
2. kur :On December 1, 1998,the parties entered into a contract entitled"Consulting Services Agreement,"referred to as the"Agreement,"which
covers project management,and technical support services for design and construction of building renovations at Los Medanos Health Center,2311
Loveridge Road,Pittsburg for Health Services Department. The Agreement has previously been amended as follows: First Amendment dated July
20,1999. The parties desire to further amend the Agreement to expand the scope of service and to increase the payment limit accordingly.

A. In the Agreement Section I (e),change the Payment Limit from$412,432.12 to$578,877.99,an increase of$166,454.87.This change shall
apply to charges from and after October 15,1999.
B. In accordance with Section 14 of the Agreement,provide the following extra services:
