SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
1. RECOMMENDATION:
APPROVE and AUTHORIZE the acceptance of a "Community Approach Award", including $35,000 ($17,500 for
the Children and Families Policy Forum and $17,500 for the Service Integration Program) from the Foundation
Consortium. DIRECT the $35,000 award to be expended as follows: $17,500 for the Children and Families
Policy Foram, to be earmarked for Policy Forum activities as directed by the Policy Forum Executive Committee;
and $17,500 for the Service Integration Program, to be earmarked for Service Integration activities as directed
by the Service Integration Oversight Committee.
II. FISCAL IMPACT:
There is no County cost or matching requirement for these funds. Staff from the Policy Forum and Service
Integration Program are expected to make a presentation about these County innovations at the "Pilots to
Policy" Conference on March 23-24, 2000 and at up to two other one-day events in the year 2000.
Ill. BACKGROUND/REASON(S) FOR RECOMMENDATION:
The Foundation Consortium conducted a statewide solicitation for nominations for the "Community Approach„
awards, honoring innovations in policy and organizational practice that sustain community approaches to
improve child and family well-being, including a $35,000 cash award for up to three selected programs or
innovations. The Children and Families Policy Forum and the Service Integration Program were selected by a
national panel of judges to share one of the $35,000 awards.
Both the Children and Families Policy Forum and the Service Integration Program were chosen on the basis of
criteria that included:
➢ An inclusive process for determining community-wide results and indicators that involve
consumers of services, community residents, front line staff and program managers as well as policymakers;
➢ Public communication of results and activities to the community;
➢ A system for tracking results using baseline information and forecasts;
➢ Identifying practices that contribute to the desired results;
➢ Pool or blend funding to more effectively use resources;
➢ Track performance of agency and contractors against established performance measures
related to community-wide outcomes.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director to execute on
behalf of the County, Standard Agreement #28-303 with the State
Department of Health Services and any subsequent amendments
thereto which do not result in increases or decreases in revenue.
FISCAL IMPACT:
Approval of this agreement will result in $23, 675 of State funding
for the Public Health Preparedness and Response to Bioterrorism
Project, for the period from October 1, 1999 through September 30,
2000 . No County funds are required.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
California Department of Health Services is one of the recipients
of the Health and Human Services CDC grant awards for Bioterrori.sm
Preparedness. Contra Costa Health Services is in the process of
implementing a new Emergency Plan that coordinates the . emergency
activities of the various Divisions of the Department . These
funds will equip the Health Services Operations Center with
computers, software, Emergency Information Systems licenses, an
electronic print board and identification vests. The equipment
will improve the Department ' s ability to respond to bioterrorism
incidences by supporting the coordination of emergency responders,
communications, and the gathering and sharing of preparedness and
response information.
Three sealed/certified copies of this Board Order should be
returned to the Contracts and Grants Unit for submission to the
State .

BACKGROUNDIRREASON(8) FOR RECOMMENDA`ION(S) :
California Department of Health Services is a recipient of the
Health and Human Services CDC grant award for Bioterrorism
Preparedness. These funds allow communities to develop or enhance
public health capacities at the local and state levels . The
monies will help build capacity to detect outbreaks of illness
that might have been caused by terrorists by improving electronic
communications networks for local health officers to share
information among public health officials regarding unusual
disease outbreaks.

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION:
APPROVE and AUTHORIZE the Sheriff-Coroner, or designee, to sign an agreement on behalf
of the County to accept an award from the Governor's Office of Emergency Services for
$114,647 for Emergency Management for the period October 1, 1999 through September 30,
2000.
FISCAL. IMPACT:
NONE; this award was anticipated in the FY 1999/00 budget. The grant requires a 49% match,
which is also budgeted.
BACKGROUND:
The federal government provides States with funding under its State and Local Assistance
Program for Emergency Management Performance Grants. The State annually distributes the
amounts to Counties in California based on a formula. The allocation to Contra Costa County
includes approximately $24,000 for distribution to the City of Richmond.
CONSEQUENCE OF NEGATIVE ACTION:
If this award is not accepted the County will need to provide the services without financial
assistance.

BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of
their Individual and Group Health Plan membership contracts with the
County.
On March 23 , 1999, the Board of Supervisors approved Contract #27-428
with Mahmood Makooi, D. C. , for the period from March 1, 1999 through
February 29, 2000 .
Approval of Contract #27-428-1 will allow the Contractor to continue to
provide chiropractic services to Health Plan members, through February
28, 2001 .

through December 31, 2000, for the provision of professional psychiatric
services, to be paid as follows :
1 . $100 .00 per initial 45-50 minute therapy session; and
2 . 70 .00 for each additional 45-50 minute therapy session.

health therapy services as a covered benefit . This population includes
Medi-Cal, Medicare, and Commercial members enrolled in the Health Plan.
On February 23 , 1999, the Board of Supervisors approved Contract #27-218-2
with California Psychiatric Medical Group, for the period from January 1,
1999 through December 31, 1999 .
Approval of Contract #27-218-3 will allow the Contractor to continue to
provide professional psychiatric services, through December 31, 2000 .

Inc . , for the period from January 1, 2000 through December 31, 2001, for provision
of primary care services for Contra Costa Health Plan members, to be paid as
follows :
1. For Medi-Cal Beneficiaries:
a. County will pay Physicians for covered services,those rates set forth in the Medi-Cal Schedule of Maximum Allowances in effect on August 1, 1998 plus
5%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan capitated payment,
County will thereafter increase the rates County pays to Contractor accordingly.
b. County will pay a quarterly case management fee,as follows:

*Panel Size is the number of Medi-Cal beneficiaries receiving treatment by Contractor during each quarter as specified in the "CCHP's Community

2. For Healthy Family Program Members. County will pay Physicians for covered services,those rates set forth in the Medi-Cal Schedule of Maximum Allowances
in effect on August 1, 1998 plus 10%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan
capitated payment,County will thereafter increase the rates County pays to Contractor accordingly.
3. For Plan B Commercial Members. County will pay Physicians for covered services,those rates set forth in the Medi-Cal Schedule of Maximum Allowances in
effect on August 1. 1998 plus 20%. In the event rate increases are subsequently approved by the State of California and are included in the County's Health Plan
capitated payment,County will thereafter increase the rates County pays to Contractor accordingly.

This Contract is funded by Contra Costa Health Plan member premiums . Costs depend
upon utilization.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On February 1, 1997, the Local Initiative for Medi--Cal managed care in Contra
Costa County was implemented. Local Initiatives are required to include
traditional Medi-Cal providers from the community in their provider networks .
On January 28 , 1997, the Board of Supervisors approved Contract #27-159 with Abbas
Mandavi, M.D. , Inc . , for the period from February 1, 1997 through December 31,
1999, for provision of primary care services .
Approval of Contract #27-159-1 will allow the Contractor to provide services to
Health Plan members, through December 31, 2001 .

Touchstone Counseling, for the period from January 1, 2000 through December
31, 2000 , for the provision of professional outpatient psychotherapy
services, to be paid as follows :
1 . $ 40.00 per 45-50 minute individual therapy session;
2 . $ 40 .00 per 60 minute family therapy session;
3 . $ 60 . 00 per 90 minute family therapy session;
4 . $ 30. 00 per hour for case conferences; and
5 . $ 15.00 per individual in a 60-90 minute group session.

Kenney, DPM, for the period from February 1, 2000 through January 31, 2001,
for the provision of professional podiatry services for the Contra Costa
Health Plan, to be paid as follows :
a. For Nadi-Cal and Commercials Members:
County shall pay Contractor those rates set forth in the Medi-Cal Schedule
of Maximum Allowances in effect on August 1, 1996. In the event rate
increases are subsequently approved by the State of California and are
included in the County's Health Plan capitated payment, County will
thereafter increase the rates County pays to Contractor accordingly.
b. For Medicare members:
Services for members who are Medicare recipients will be reimbursed at the
Medicare rate of payment. Physician will bill Medicare as primary payor and
County will pay Medicare-required copayments and deductibles for Medicare
approved services.
