Services will be provided to 120 households. HSG 99-02 Fair Housing Services/ Contra Costa Legal $35,000 Legal Assistance Services Foundation The purpose of this program is to alleviate problems of housing discrimination through the provision of legal assistance to Urban County residents experiencing discrimination. Services will be provided to 100 households. HSG 99-03 Fair Housing Counseling Pacific Communities $16,000 Services Inc. The purpose of this program is to provide fair housing counseling to Urban County residents. Services will be provided to 50 households. HSG 99-04 Rental Rehabilitation Program Housing Authority of the $250,000 County of Contra Costa The purpose of this program is to increase and maintain the supply of rental housing affordable to extremely-low, very-low, and low-income urban County households through the provision of subsidized Dousing rehabilitation loans. Loans will be provided to rehabilitate up to 30 housing units. HSG 99-05 Housing Rehabilitation City of San Pablo $160,000 Program Community Development Department The purpose of this program is to maintain the supply of affordable housing owned and occupied by very-low and low-income households in the City of San Pablo through the provision of subsidized housing rehabilitation loans to 12 qualified households. The program includes 5 emergency,grants and 10 paint rebates. HSG 99-08 Transitional dousing for Housing Authority of the $ 123,308 Homeless Families Contra Costa County The purpose of this program is to provide transitional housing for homeless families. One site in East County and one site in West County will each include 20 to 30 apartments. ED 99-10 .Family Day Care Contra Costa Child $75,000 Development Project Care Council The purpose of this project is to assist low- and moderate-income persons become licensed in-home child care providers, by providing technical assistance, and training to meet state licensing requirements. ED 99-11 Small Business Development Private Industry Council $60,000 Assistance The purpose of this project is to increase the number of micro-enterprises, owned and operated by low-and moderate-income persons, to succeed in starting and operating a micro-enterprise. Services will be provided to 20 low-moderate income Urban County residents. ED 99-13 'forth Richmond Employment Rubicon Programs Inc. $100,000 Partnership The purpose of this program is to provide job training, work experience and placement assistance to 120 low-and moderate-income North Richmond residents. ED 99-14 Building& Grounds Rubicon Programs Inc. $42,000 Economic Partnership The purpose of this program is to create stable jobs and livable wages for 10 low-income persons through business expansion and job creation. OP 99-17 1998-99 Access.Ramps City of El Cerrito $40,000 Projects The purpose of this project is to improve access for mobility impaired persons through the installation of 71 curb ramps. OP 99-18 Ramp Installation Project City of Martinez $25,500 The purpose of this project is to improve access for mobility impaired persons through the installation of 16 curb ramps. OP 99-20 Pedestrian Ramps -Old City of San Pablo $22,400 Town Section Phase V The purpose of this project is to remove architectural barriers to -nobility unpaired individuals that live in the city of San Pablo. The sponsor proposes to install 30 curb ramps in the Old Town Section of San Pablo. OP 99-22 Village Drive Area Street City of Brentwood $30,000 Lighting The purpose of this project is to improve safety and security in the Village Drive neighborhood through the replacement of 23 street lights and the installation of 6 new lights. OP 99-23 Orchard Drive Area City of Brentwood $25,000 Infrastructure Improvements The purpose of this project is to increase fire safety and security at Los Nogales Public Housing Authority through the installation of new water lines, and an access road. OP 99-24 Capital Improvements of Rehabilitation Services of $ 8,000 490 Golf Club Road :Northern California The purpose of this project is to improve interior space through the installation of additional HVAC; system and new carpets. PS 99-25 After School Outreach East Bay Center for $22,000 Performing Arts Program the Performing Arts The purpose of this program is to provide an after school multi cultural performing arts program in music and dance for low-income elementary school students residing primarily in Urban gest County. The program will serve approximately 500 children. PS 99-26 Boys & Girls Club Boys & Girls Club of Martinez $1.0,000 Program Director The purpose of this program is to increase self esteem and develop potential of school aged youth through education and career development, sports and recreation, life shills and leadership development. Approximately 220 youth will be served by this program. PS 99®27 Child Assault Prevention Family Stress Center $12,000 Program The purpose of this program is to prevent child assault by providing assault self-protection shills and techniques to children ages 3 to 18. The program, through classroom presentations, teaches children how to recognize abuse, and strategies to deal effectively with potentially dangerous situations. Presentations will be made to 1.,500 pre-school and elementary school children.in East and Central County. PS 99-28 Child Assault Prevention Rape Crisis Center $14,000 Program The purpose of this program is to decrease sexual violence among teen by providing education factors leading to sexual abuse. Sponsor will provide classroom instruction to 840 middle and high school youth. PS 99-29 Children. At Risk Court Appoint Special $25,000 Advocates The purpose of this program is to prevent prolonged foster home placement of abused, dependent children of the juvenile court and reunite families by providing special court advocates and case management services to low- income Urban County residents. The program trains and provides volunteers who provide reports to the court and attend juvenile court proceedings, and follow-up. The program expects to provide 40 children with advocacy services. PS 99-30 Invest in Fids -Mentoring Healing Our Nation From $15,000 & Scholarship Program Violence The purpose of this program is to help youth stay in school, maintain or improve school performance, postpone parenthood, and avoid juvenile infractions. fine-on-one mentors will serve 6 youth in San Pablo, North Richmond, and Bay Point schools. PS 99-31 'Youth Cultural Arts Los Centzontles $13,000 Program The purpose of this program is to increase self-confidence, teamwork and leadership skills in 60 youth through integrated cultural arts training. PS 99-32 "Youth Delinquency New Connections $10,500 Diversion Program The purpose of this program is to prevent the incarceration or placement ofjuvenile offenders with substance abuse and gang problems by providing case management services and advocacy to 26 low-income Urban County youth and their families. The program will provide assessment and short-term youth and family counseling, long- term special needs counseling, chemical dependent referral for treatment and parental education. PS 99-33 Youth Violence Relationship Battered'Women's Alternatives $1.5,00+0 and Alliance Building The purpose of this program is to prevent teen violence by providing violence awareness instruction to low- and moderate-income Urban County teens. Classroom presentations will be made to 800 students, individuals and group counselors. PS 99-34 Brookside Community Health Brookside Community Health $10,000 Center Rental Clinic Center, Inc. The purpose of this program is to provide and ongoing source of primary dental care for un-and under-insured residents. The San Pablo Mental Clinic will serve 1,500 residents in Urban West County. PS 99-35 Child Parent Enrichment Family Stress Center $13,000 Program The purpose of this program is to prevent child abuse and neglect through intervention with at-risk families. The program will provide in-home counseling and support to 15 families throughout the Urban County. PS 99-36 YWCA Family 'YWCA of Contra $.20,000 Empowerment Program Costa County The purpose of this program is to increase family self-sufficiency by providing mental and physical health, and job training services to 150 families who have children in the YWCA's day care program. PS 99-37 Multicultural/Senior Neighborhood House $55,000 Family Center of North Richmond The purpose of this program is to maintain a community center for residents of North Richmond. The program provides, in collaboration with other community agencies, nutrition program, information and referral, education programs, arts and crafts, and social programs reflective of the area s cultural diversity. PS 99-3 24-Hour homeless Hotline Contra Costa $16,000 and Emergency Shelter Program Crisis Center The purpose of this program is to prevent homelessness and provide 24-hour crisis intervention and emergency shelter to low-income Urban County residents. The program will provide a 24-hour hotline for the homeless to assist them in securing shelter and other essential services. The program also provides counseling to assist clients in locating long term shelter, and motel vouchers to homeless families with children, and persons whose health and safety is at risk for one or two nights. Approximately 1,544 persons will be served by this program. PS 99-40 Assistance for Homeless Cambridge Community $22,000 & Families in Crisis Center The purpose of this program is to provide homeless persons and families in crisis with basic survival needs through the provision of food, clothing and basic personal care items. Approximately 796 families (1,688 individuals)will be served by this program. PS 99-41 Comprehensive Housing Pacific Community Services $10,000 Counseling Incorporated The purpose of this program is to prevent homelessness and maintain affordable housing by providing Dousing counseling to 4014 low-income persons. The program provides counseling in areas such as landlord/tenant, mortgage default and delinquency counseling, prepurchase/rental, educational, and information and referral. PS 99-42 Emergency Family Shelters St, Vincent de Paul $10,000 The purpose of this program is to prevent homelessness by placing homeless families in permanent/transitional housing. The program will provide stabilization and case management via the emergency rotating shelter for 15 families in East and Central County. PS 99-43 Emergency Housing and Shelter Inc. $27,000 Homeless Services The purpose of this program is to maintain housing and prevent homelessness through landlord/tenant counseling or direct housing assistance. The program provides landlord/tenant counseling, rental/mortgage assistance, and home owner assistance to 475 residents throughout the Urban County. PS 99-45 Housing and Homeless Contra Costa Legal $15,000 Assistance Program Services Foundation The purpose of this program is to prevent homelessness and maintain affordable housing by providing legal assistance to low-income Urban County residents. Services include advice and counseling, brief advocacy services, and direct representation before judicial and administrative tribunals. Legal services will be provided to a minimum of 240 residents. PS 99-46 Martinez Dining Room Loaves and Fishes of $ 10,000 Contra Costa The purpose of this program is to alleviate hunger by providing nutritious meals to people seeking emergency food assistance living in the City of'Martinez. Sponsor will provide meals to 175 persons. The project works in cooperation with PS 99-47, Prepared&Perishable Foods Program. PS 99-47 Prepared and Perishable Contra Costa Food Bank $12,000 Program The purpose of this program is to provide nutritious meals to people seeking emergency food assistance from soup kitchens and, shelters. The program will provide 14,000 meals annually. PS 99-48 Alzheimer's Respite Care West County Adult Day Care $10,000 The purpose of this program is to provide sheltered day care services six days a week to 27 seniors with Alzheimer's. The project sponsor will provide therapeutic and educational activities, personal care and counseling as well as respite support for family members and care givers. PS 99-49 Legal Services for Needy Contra Costa Senior $10,000 Urban County Seniors Legal Services The purpose of this program is to maintain housing, income and to prevent elder abuse by providing free legal counsel and representation to Urban County seniors on basic needs such as housing, income, health care rights, to maintain housing and prevent homelessness. The program proposes to assist 280 seniors. PS 99-50 Ombudsman Services of Ombudsman Services of $ 10,000 Contra Costa Contra Costa The purpose of this program is to prevent elder abuse by providing quality of care abuse investigations and advocacy for frail and dependent elders residing in Urban County nursing and residential care facilities. The program trains volunteers to be certified Ombudsman. Ombudsman visit nursing and residential care facilities on a regular basis thereby maintaining a presence in order to safeguard the rights of the long-term care residents. Approximately 330 seniors will receive services. PS 99- 1 Senior Home:Equity Eden Council for Hope 5109000 Conversion Counseling and Opportunity The purpose of this program is to prevent homelessness by providing housing related services to low-income Urban County seniors. Reverse mortgages are designed to assist seniors to remain in their homes by taking advantage of being"house rich and cash poor." The program will provide professional staff to provide information and intensive counseling sessions to 84 low-income Urban County seniors and their families to identify the type of mortgage which potentially best serve their needs. PS 99-52 The Senior Service Network Pleasant Hili Recreation $"9,700 and Park District The purpose of this program is to prevent premature institutionalization and maintain housing independence by providing on-site crisis intervention assistance to low- income Urban County seniors. The program provides on- site crisis intervention and care management at the Pleasant Hill Senior Center. The program expects to serve 250 Central County seniors. PS 99-53 Villa San Ramon City of San Ramon $25,000 "Conserv" Program The purpose of this program is to ensure the provision of congregate care services through direct subsidies for meals, transportation, housekeeping social activities, and security to 24 low-income Urban County senior residents of Villa San Ramon. PS 99-54 HIV/AIDS Resources New Connections $1.7,000 The purpose of this program is to prevent homelessness and maintain independence of people living with HIVADS. The program will provide case management and support services to 75 Urban County residents. PS 99-55 HIV Nutrition Education Diablo Valley AIDS Center $16,000 and. Food Program The purpose of this program is to maintain independence by providing home delivered meals and nutrition education to low-income Urban County residents with HIV/AIDS. The program proposes to serve 100 persons and their families. PS 99-56 Independent Living Shills for Lions Blind Center 510,000 the Visually Impaired of Diablo Valley The purpose of this program is to prevent the institutionalization and maintain independence of 30 newly blind and visually impaired seniors by providing basic self-help living skills instruction to low- income Urban County residents. The program provides three professionals, skilled in teaching the newly blind independent living shills in the clients home. PS 99-57 Integrated Homecare Services Senior Outreach Services $14,000 of Contra Costa The purpose of this program is to prevent premature homelessness and institutionalization or hospitalization by providing integrated home care services to 160 seniors with disabilities and homebound people. PS 99-58 Jobs Plus: Employment Program Independent Living $10,000 for Persons with Disabilities Resources of Contra Costa County The purpose of this program is to prevent unemployment of disabled adults by providing prevocational training to low-income Urban County residents. The program will provide job counseling and training in pre- employment skills, and job placement assistance to 30 persons. The program will also provide outreach to employers on accommodations for disabled, employer incentive programs, and placement relationships. PS 99-59 Living Safely at Horne Independent Living $10,004 Resources of Contra Costa County The purpose of this program is to prevent institutionalization of and maintain the independence of people with disabilities. In-home independent living skills training will be provided to 100 adults throughout the Urban County. PS 9960 Conflict Resolution Panels Center for Human Development $12,000 The purpose of this program is to resolve conflicts peacefully through mediation services. The program provides recruitment and training of volunteers in conflict resolution skills and techniques in conflict management. Volunteers serve the low to moderate income community members by assisting disputing parties to resolve their differences. The program will serve 55 Forth Richmond and San Pablo residents. PS 9961 Sexual Assault Victim Rape Crisis Center $20,000 Empowerment Program The purpose of this program is to provide 24-hour crisis intervention services to sexual assault victims and their families. Services include 24 hour crisis intervention(telephone), 24 hour in-person accompaniment medical exams and legal reporting, court hearings, follow-up counseling and prevention services. The program will provide services to 240 victims.
