Oakland, CA 94612 1 21 510-444.5020 22 DATE CLAIM ACCRUED: 23 ' November 25, 1996 E 24 PLACE CLAIM ACCRUED: 25 Merrithew Memorial Hospital, 2500 Alhambra Avenue, Martinez, California : 26 CIRCUMSTANCES GIVING RISE TO CLAIM: 271 On or about May 13, 1993, Claimant presented to the emergency department at 28 i E 1 Merrithew Memorial Hospital complaining of severe abdominal pain. Claimant was 21 examined by Claimees in the emergency room and was diagnosed as suffering from a i 3 possibly ruptured appendix. Claimant underwent an exploratory laparascopy and 4 I appendectomy at Merrithew Memorial Hospital on May 13, 1998 by physicians and 5 employees of Claimees. Following said surgery Claimant complained to physicians and 6 employees of Claimees that she was suffering from symptoms of pain, numbness and I 7 electrical sensation in her hands and wrists, which she had not suffered from prior to the 8 surgery. Claimant continued to complain of said pain and injury to her hands and on 9 November 25, 1998, Claimant was informed and became aware at that time that said I 10 injuries likely resulted from the surgery performed May 13, 1998, at Merrithew Memorial j 11 Hospital. 12 CAUSATION: 13 I Claimees and their staff failed to properly evaluate, care for, diagnose and perform E j41 surgery and other medical care and services on Claimant as above described that as a 1511 direct and legal result of said negligent care and treatment rendered by Claimees and 16 their staff as alleged herein, Claimant DEBRA MARKHAM suffered severe personal 17 injuries. 18 NATURE AND EXTENT OF DAMAGES: 19 Claimant claims damages in an amount presently unknown for physical injuries as i 20 described above, emotional distress, for medical and related expenses and loss of 21 earning capacity.
