Approval of Contract #27-262-2 with Arie Van Gemeren, M.D.
SPECIFIC 1"EQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services D®rector, or his designee
(Milt Camhi; , to execute on behalf of the County, Contract #27-262-2
with Arie Van Gemeren, M.D. , for the period from February I, 1999
through January 31, 20,00, for the provision of professional 'wealth care
services for the Contra Costa Health Plan, to be paid as follows :
a. For Medi-Cal, and Commercials Members:
County shall pay Contractor those rates set forth in the Medi-Cal
Schedule of Maximum Allowances in effect on August 1, 1-998. In the event
rate increases are subsequently approved by the State of California and
are included in the County's Health Plan capitated payment, County will
thereafter increase the rates County pays to Contractor accordingly.
b. For Medicare members
Services for members who are Medicare recipients will be reimbursed at
the Medicare rate of payment. Physician will bill Medicare as primary
payor and County will pay Medicare-required copayments and deductibles
for Medicare approved services.
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan (Health Plan)
member premiums . Costs depend upon utilization. As appropriate,
patients and/or third party payors will be billed for services .
BACKGROUND/REASON(S) FOR RECQMMENDATION' S} :
On January 27, 3996 , the Hoard of Supervisors approved Contract #27-
262-1 with Arie Van (3remere n, M.D. , for the period from February 11 , 1998
through January 31, 1999, for provision of professional health care
services to the Health Plan.
The Heath Plan has an obligation to provide certain specialized
-professional health care services for its members under the terms of
their ''ndividual and Group Health Plan membership contracts with the
Coun t y.
Approval of this Contract #27-252-2 will allow the Contractor to
continue to provide professional health care services through January
31, 2 0 0 v .

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his Designee
(Frank. Puglisi, Jr. ) , to execute on behalf of the County, Contract
Amendment Agreement #26-167-12, to amend Contract #26-167-11 with
Integrated Environmental Services, effective April 1, 1999, to
increase the Contract Payment Limit by $2, 250, from $215, 640 to a new
Contract Total Payment Limit of $217, 890 .
FISCAL IMPACT:
This contract is funded in the Health Services Department Enterprise
I Budget . County will continue to pay Contractor a flat rate of
S7 . 50 per 44 gallon container, including the cost of all barrels,
liners, and existing storage bins . This Contract guarantees an annual
maximum payment limit of $72, 630 .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
On December 15, 1999, the Board of Supervisors approved Contract
#26-1167-11, with Integrated Environmental Services, for the provision
of bio-hazardous waste removal services, for the period from December
1, 1998 through November 30, 2001 .
Approval of this Contract Amendment Agreement #26-167-12 will allow
the County to continue to reimburse Contractor for bio-hazardous waste
removal services, at an additional site located at the Forth Richmond
Center for Health through November 30, 2001 .

RECOMMENDATIONS:
Approve and authorize the Health Services Director to execute on
behalf of the County, Contract #23-241-2 with Albert William Borden,
in the amount of $99,000, for the period from May 1, 1999 through
April 30, 2000 for the prevision of consultation and technical
assistance with regard to programming and analysis services for the
Department's Information Systems Unit.
FISCAL IMPACT:
Funding for this Contract is included in the Department's Enterprise
I budget.
BACKGROUND I REASC (S) LOR RECOMMENDATION(S) :
In April. 1998, the County Administrator approved and the Purchasing
Services Manager executed Contract #23-241 (as amended by Contract
Amendment Agreement 023--241.-1) with Albert William Borden, for the
period from May 4, 1998 through April 30, 1999, to provide
consultation and technical assistance with regard to programming and
analysis services for the Department's Information Systems Unit.
Approval of Contract #23-241--2 will allow the Contractor to continue
providing services through April 30, 2000.

Court ordered random substance abuse testing.

RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Social Service Director, or designee, to execute contract #21-501 with.
PharmChem Laboratories in the amount of$100,000 for Court ordered random substance abuse testing for
the period from July 1, 1999 through June 30, 2000, as a result of RFP #1051.
FINANCIAL IMPACT:
These costs are of Child Welfare System (CWS) non-federal Title IV-E funding, (70% State, 30% County)
and are included in the Social Service Department budget.
CHILDREN'S IMPACT STATEMENT:
This contract supports one of Contra Costa County's community outcomes: "Families that are Safe, Stable
and Nurturing," by helping parents address their substance abuse issues.
BACKGROUND:
On February 28, 1999, the Social Service Department issued a Request for Proposal; (RFP #1051) to provide
court ordered random substance abuse testing. The only response to RFP 1051 was from PharmChem meets
the objectives outlined in the RFP.

I. RECOIVVYffiNDED ACTION:
APPROVE and AUTHORIZE the Community Services Director or designee to execute Interagency
Agreement Amendment #39-811-9 with the Contra Costa Community College District for an Inflation
Adjustment to increase the contract amount by $25,323 to a neva contract payment limit of$817,317 for
expanded state preschool program services for the period from January 1, 1999 through June 30, 1999;
and,
APPROVE and AUTHORIZE the Community Services Director or designee to execute Interagency
Agreement Amendment #39-850-3 with the Contra Costa Community College District for an Inflation
Adjustment to increase the contract amount by $4,245 for a new contract payment limit of$137,002 for
full-day preschool program services for the period from July 1, 1998 through June 30, 1999.
II, FINANCIAL LMPACT:
No County funding is required. The Contra Costa Community College District receives its funding for
this service from.the California Department of Education.
III, CHILDREN'S LMPACT STATEMENT:
The Community Services Department Child Development Program supports two of Contra Costa
County's community outcomes: "Children Ready for and Succeeding in School" and "Families that
are Safe, Stable and Nurturing." These outcomes are achieved by offering comprehensive services,
including high quality early childhood education, nutrition, and health to low-income children
throughout Contra Costa County.

IV. REASONS FOR RECONLMENDATIONS/BACKGROUND:
On August 11, 1998 the Board approved Interagency Agreement #39-811-8 with the Contra Costa
Community College District (College District) for expanded state preschool services. On that same
date, the Board approved Interagency Agreement #39-850-2 with the College District for a full-day
preschool program.
As a result of the Inflation e COLA/Minimum Wage adjustment passed by the State Legislature and
approved by the Governor, state preschool programs are entitled to the above increases for the 1998-99
fiscal year.
