I. RECOMMENDATION:
Approve the providers (listed on the attachments)recommended by the Contra
Costa Health Plan Quality Council at the meeting on March 26, 1999,
II. FINANCIAL IMPACT:
None.
III. BACKGROUND:
The National Committee on Quality Assurance(NCAA)has requested evidence
of Board Approval for each Contra Costa Health Plan provider be contained
within the provider's credential file. The recommendations were made by the
Contra Costa Health Plan Credentialing Committee and approved by the Contra
Costa Health Plan Quality Council,

BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
The Hazardous Materials Commission consists of 13 members, in various categories. With the resignation of
Mr. Felon Holloway as alternate to the Labor Seat#2,the Paper,Allied-industrial, Chemical &'Energy Workers
International Union 8-5 (PACE) has nominated Steve Sullivan as Mr. Holloway's replacement. The Hazardous
Materials Commission is recommending that the Board of Supervisors take the necessary action to concur.

RECOMMENDATION:
DECLARE vacant the seat on the Fish and Wildlife Committee held by Charles
Morehouse, who is deceased, DIRECT the Clerk of the Board to apply the Board's
policy to filling the vacancy, and REFER the vacancy to the Internal Operations
Committee.
BACKGROUND:
Staff has recently become aware that Mr. (Morehouse, a member of the Fish and
Wildlife Committee, has died. It is, therefore, appropriate for the Board to declare
his seat vacant so the process can go forward to recruit a replacement for this seat.

BACKGROUND:
There are currently several vacancies on the Women's Advisory Committee. The
Advisory Committee has recruited and interviewed several applicants and has
recommended the appointment of four individuals to currently vacant seats. Our
Committee interviewed these four individuals on April 26, 1999 and were very
impressed with the background, interests and commitment of these four women. We
are pleased to recommend the appointment of these four individuals to the Women's
Advisory Committee.
