OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION RECg MENDED ACTION Approve and authorize the Health Services Director, or his designee (Frank Puglisi, Jr. ) , to execute on behalf of the County, Contract #26-244-2 with Marriott Management Services/Lohmar, for the period from July 1, 1998 through June 30, 2001, in the amount of $1, 600, 000, for linen rental and management services for Contra, Costa Regional Medical Center and Contra Costa Health Centers . FINANCIAL 1KPAGT: This contract is included in the Health Services Department Enter'- prise I budget . REASONS FOR R.ECg MENDA'TIfJNS/BACRGRt}'CTNb Hospital and clinics linen rental and management services have been provided under contract since 1984 . On August 1, 1995, the Board of Supervisors approved Contract #26- 244-1 with Marriott Management Services/Lohmar (formerly: Lohmar) for the period: from July 1, 1995 through June 30, 1998, for the provision of hospital and clinics linen rental services, including delivery of clean garments and other linen items ; and pick-up of soiled linen for the Health Services Department . The Contractor provides the personnel, facilities, tools, equipment, materials and supervision necessary for daily linen services. Approval of Contract #26-244-2 will continue the Contractor' s services through June 30, 2001. Approval of this Contract Amendment Agreement #24-751-62 will allow the Contractor to provide additional services, including Dual-Diagnosis services, through June 30, 1998 . SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOBMENDED ACTION: Ratify purchase of services from Crestwood Hospitals, Inc. , through December 31, 1997, and authorize the County Auditor-Controller to pay the $48,168.34 outstanding balance for the admission of and treatment for mentally disturbed persons in need of subacute skilled nursing care in a facility known as an institution for the mentally diseased (IMD) . FINANCIAL IMPACT: This Contract was included in the Health Services Department's Budget, funded by County/Realignment. REASONS FOR RECODIMMMATIONSZBACKGROUND: On December 10, 1996, the Board of Supervisors approved Novation Contract ##24- 373-12 with Crestwood Hospital, Inc. , for the period from July 1, 1996 through June 30, 1997, for the admission of and treatment for mentally disturbed persons in need of subacute skilled nursing care in a facility known as an institution for the mentally diseased (IMD) . The Contract included a six-month automatic extension through December 31, 1997. Due to a reorganization of the company, this Contract was not renewed, but a new contract (#24-933) with Crestwood Behavioral Healthcare, Inc. , was approved by the Beard of Supervisors on March 10, 1998. Services were requested and provided beyond the payment limit and by the end of December 31, 1997, charges had been incurred in excess of the Contract Payment Limit, of which $48,168.34 remains outstanding. Services were both requested by County staff and provided by Crestwood Hospital, Inc. , in good faith. Because of an administrative oversight by both the County and Contractor, and the reorganization of Crestwood Hospital, Inc. , use of the Contractor's services exceeded the authorized limits. The Department is requesting that the amount due to the Contractor be paid. This can be accomplished by the Board of Supervisors ratifying the actions of the County employees in obtaining provision of services of a value in excess of the contract payment limit. This will create a valid obligation on the part of the County, retroactively authorizing all payments made by the Auditor- Controller uditorController up to now, and authorizing payment of the balance. Acknowledge receipt of notice from La Cheim Residential Treatment Center requesting termination of Contract #24-351-26 (as amended by Amendment Agreement #24-351-27) , effective at the close of business on February 28, 1998; and 2. Approve and authorize the Health Services Director or his designee (Donna Wigand LCSW) to execute on behalf of the County, Contract Amendment Agreement #24-133-41 with La Cheim Residential Treatment Center, to amend Contract #24-133- 38 (as amended by Amendment Agreements #24-133-39 and #24-133-40) , effective March 1, 1996, to increase the contract payment limit by $94,596 from $,475,379 to a new total of $569,975. This amendment will increase the six month automatic extension payment limit by $136,759, from $473,241 to a new total of $610,000. FINANCIAL IMPACT: There is no increase in County cost. REASONS FOR RECOMMENRA_TIONS/BACKGROUND: On December 9, 1997, the Board of Supervisors approved Contract #24-351-26 (as amended by Amendment Agreement #24-351-27) with La Cheim Residential Treatment Center, for the period from January 1, 1998 through June 30, 1998 for operation of a residential group home program for mentally disturbed adolescents in Contra Costa County. On December 2, 1997, the Board of Supervisors appzoved Contract #24-133-38 (as amended) with La Cheim, for the period from January 1, 1998 through June 30, 1998, for intensive day treatment services for mentally or emotionally disturbed children. The purpose of this amended Board Order is to advise the Board that the Depart- ment and the Contractor have mutually agreed to terminate the residential fee- for-service Contract #24-351-26 (as amended) , effective at the close of business on February 28, 1998 (stated as April 28, 1998 in the prior Board order) , and to add the remaining dollars and federal Medi-Cal billable amount to La Cheim's Contract #24-133-38 (as amended) , effective March 1, 1998 (stated as May 1, 1998 in the prior Board order) . Approval of Novation Contract #24-810-4 replaces the six-month automatic extension under the prior Contract and allows the Contractor to continue to provide services through June 30, 1999.
