Faustina Egoian (dba Baldwin Manor)

a. Approve and authorize the Health Services Director or his designee (Donna M.
Wigand) to execute on behalf of the County Emergency Residential Care
Placement Agreement #24-086-104 (1) with Faustina Egoian (dba Baldwin Manor) ,
for the period from July 1, 1998 through June 30, 1999, to provide emergency
residential care to mentally disturbed adults who are potentially eligible for
SSIfSSP, with a maximum payments of $703 per client per month through December
31, 1998, and a maximum payment of $726 per client per month beginning January
1, 1999; and
b. Authorize the Auditor-Controller to pay a maximum of $93 per month through
December 31, 1998, and a maximum of $96 per month beginning January 1, 1999,
to clients who are placed in the board and care facilities under said
Emergency Residential Care Placement Agreements for incidental expenses and
personal needs.
FISCAL IMPACT:
Under this program, the County pays the State-established rates for the residential
care of mental health clients who are not vet receiving SSI/SSP (but who are
potentially eligible and have made formal application to the Social Security
Administration for SSI/SSP) , contingent upon the client's signing an agreement with
the County to reimburse the County from the SSI/SSP money, once it is finally
granted by the federal government (usually 3 to 6 months retroactively back to the
month of application) .
For those mental health clients who are not granted SSI/SSP (i.e. , are found
ineligible) , the County covers the residential care payments to the facility
operators through the County's established Mental Health Program Funding.
Initially, ten to twenty percent of the clients are found ineligible by the Federal
Social Security Administration, depending on changes in Federal guidelines, and then
some of these are ultimately found eligible through appeal processes.

This Program provides residential care for 40 to 5o clients per year. Approval of
Agreement #24-086-104(1) will ,allow the Contractor to continue providing services
through June 30, 1999.

I . RECQb=NDZD ACTION:
Approve and authorize the Health Services Director,: or his designee
(Milt Camhi) , to execute on behalf of -the County, Contract #27-381
with Deborah Sedberry, M.D. , for the period from May 1, 1998 through
April 30, 1999, to be paid in accordance with the rates provided in
the Medi-Cal Schedule of Maximum Allowances in effect on the date
professional health care services are rendered to Contra Costa
Health Plan members.
II . FINANCIAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums'.
Costs depend upon utilization. As appropriate, patients and/or
third party payors will be billed for services .
III . REASONS FOR RECOMMENDATIONS/BACKGROUND:
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of
their Individual and Group Health Plan membership contracts with the
County.
The Health Plan is also required under the terms of its Local
Initiative contract with the State, to contract with community
physicians and other providers, called "Safety Net" and
"Traditional" Providers, for the provision of medical care to
Medi-Cal recipients .
This Contract is necessary to meet State mandates to expand the
number of community providers for the Local Initiative, along with
a recent Department of Corporations audit finding that requires
formal contracts with law volume providers.
Approval of this Contract will allow the Contractor to provide
professional health care services to Health Plan members through
April 3'0, 1999 .

I. RECOMMENDED ACTION:
Authorize the Executive Director, Contra Costa County Private Industry Council, to
execute on behalf of the County a standard form contract (County #19-2038-0) with the
Contra Costa County Office of Education for the operation of a summer of service
program contract for $64,473 for the term March 1, 1998 through September 30, 1998 to
operate a community service program for middle school students.
IL FINANCIAL IMPACT:
None, one hundred percent federal funded.
III. CHILDREN IMPACT STATEMENT:
This contract is to provide vocational training to appropriate DTPA eligible participants.
This will support the third outcome--Families that are economically self sufficient.
IV. REASONS FOR RECOMMENDATION:
The Contra Costa County PIC has received a grant from Work/Family .Direction, Inc. to
provide summer youth community service program for middle school age children. Up to
50 activity slots are to be available of which 36 are reserved for the children fo the
corporate donors. The balance of the activity slots will be filled by Title IIB Summer
Youth Employment and Training participants, which will co-fund the program. The
County Office of Education is the service provider for the Summer 1998 SYETP, this
contract will provide the payment for the non SYETP participants that participate in the
program.

ISCA:
These contracted services are claimable as Foster Care
recruitment costs which are 75% Federal and 25% State. No
County Cost.
BACKGROUND;
Contra Costa is one of eleven Bay Area Counties who
participate in the financial support of a "Help Line"
designed to handle inquiries from individuals and families
who have seen the program "Brian's Kids, " which encourages
Foster Parenting for hard to place children. In addition to
the collaborative resources of the 11 county agencies, the
state Department of Social Services Adoptions Branch, the
Zellerbach Family Fund, KPIX-TV, and other private funding
sources also support the Help Line.
