Approval of Contract #26-925-3 with Martha E. Corcoran, M.D.
SPECIFIC R9QUEST{S}OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
I . RECQMENDBD ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Contract #26-
915-3 with Martha E. Corcoran, M.D. , (Specialty: Otolaryngology) for
the period from July 1, 1998 through June 30, 1999, in the amount of
$120, 000, for professional. Otolaryngology (Ears, Nose, and Throat)
services to patients at Contra Costa Regional Medical Center and
Contra Costa Health Centers.
II . FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payors will be billed for services .
III . REASONS FOR RECOMMENDATIONS/BACKGROUND:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and health centers .
On June 3 , 2997, the Board of Supervisors approved Contract #26-915-2
with Martha Corcoran, M.D. , for the period from July 1, 1997 through
June 30, 1998, for the provisions of Otolaryngology services.
Approval of Contract #26-925--3 will allow Dr. Corcoran to continue
providing ENT (Otolaryngology) services through June 30, 1999 .

RBCQN.XENDXD ACTION:
1. Acknowledge receipt of notice from La Cheim Residential Treatment Center
requesting termination of Contract #24-351-26 (as amended by Amendment Agreement
#24-351-27) , effective at the close of business on April 28, 1998; and
2. Approve and authorize the Health Services Director or his designee (Donna
Wigand LCSW) to execute on behalf of the County, Contract Amendment Agreement
#24-133--41 with La Chem Residential Treatment Center, to amendment Contract #24-
133-38 (as amended by Amendment Agreements #24-133-39 and #24-133-40) , effective
May 1, 1998, to increase the contract payment limit by $94,596 from $475,379 to a
new total of $569,975. This amendment will increase the six month automatic
extensions payment limit by $136,759, from $473,241 to a new total of $610,000.
FINANCIAL IMPACT:
There is no increase in County cost.
REASONS FOR R8C0NDATIONs/BACKGROi3ND:
On December 9, 1997, the Board of Supervisors approved Contract #24--351-26 (as
amended by Amendment Agreement #24-351-27) with La Cheim Residential Treatment
Center, for the period from January 1, 1998 through June 30, 1998 for operation
of a residential group home program for mentally disturbed adolescents in Contra
Costa County. On December 2, 1997, the Board of Supervisors approved Contract
#24-133-38 (as amended) with La Cheim, for the period from January 1, 1598
through June 30, 1998, for intensive day treatment services for mentally or
emotionally disturbed children.
The purpose of this Board Order is to advise the Board that the Department and
the Contractor have mutually agreed to terminate the residential fee-for-service
Contract #24-351-26 (as amended) , effective at the close of business can April 28,
1998, and to add the remaining dollars and federal Medi-Cal billable amount to La
Cheim's Contract #24-133-38 (as amended) . In this way, La Chem can increase its
services to Contra Costa residents and increase Medi-Cal billing.
Effective May 1, 1998, the services previously provided under #24-351-26 will be
continued under Contract #24-133-38.

Approve and authorize the Health Services Director to execute on behalf
of the County, Contract ,#23-012M-23 with KPMG Peat .Marwick, for the period
from July 1, 1998 through June 30, 1993, with a payment limit of $50,000,
for consultation and technical assistance with regard to financial
issues.

This Contract is included in the Health Services Department's Enterprise
1 Budget and is funded by third party revenues.

The Department has contracted with KPMG Peat Marwick for several gears
for provision of fiscal expertise on various audit appeal issues;
representation at appeal hearings; and technical research and advice on
class action litigation.
On June 3, 1997, the Board of Supervisors approved Contract #23-01.2-22
with KPMG Peat Marwick for the period from July 1, 1997 through June 30,
1998.
Approval of Contract #23-012-23 will allow the Contractor to continue
providing its expertise with regard to Medicare and Fedi-Cal
reimbursement issues through June 30, 1999.

RECI_MMENLMD ACTION:
Approve and authorize the Health Services Director, or his designee
(Frank Puglisi, Jr. ) , to execute on behalf of the Country, Contract ##26-
331 with Airia Sasser, for the period from June 1, 1598 through May 31,
1959, with a payment limit of $37, 500, for consultation and technical
assistance to the Department with regard to administrative and
statistical analysis for the Ambulatory Care Centers.
FINANCIAAL IMPACT:
Funding ' for this Contract is included in the Health Services
Department' s Enterprise I budget .
REASONS FOR RECOMMEIMA,TIONS/SACKGRC3UND:
Under Contract ##26-331., Airia Sasser will provide consultation and
technical assistance to the Department with regard to administrative
and statistical analysis for the Department' s Ambulatory Care
Administrator, Specialty Access Steering Committee and Patient' s Choice
SteeringCommittee. These committees are responsible for quality
improvement projects which will improve access for patients in
ambulatory care. Reports produced by the Contractor will be used by
the Ambulatory Care Administrator and the committees to guide
operatio .al. decision-making.
Contractor shall 'provide consultation and technical assistance to the
Department through May 31, 1599 .
