Approve and authorize the Health Services Director or his designee
(Frank Puglisi, Jr. ) to execute on behalf of the County, Contract
#26-158-18 with Antonio G. Britto (dba Concord Transportation
Services — Says Taxi of Martinez) , in the amount of $140, 000, for
the period from July 1, 1998 through June 30, 1999, to provide
taxicab transportation services for Contra Costa Regional Medical
Center patients .
FISCAL IMPACT:
This Contract is included in the Health Services Department ' s
Enterprise I budget .
BACjtGROTJND/REASON(S) FOR RECOMMENDATION(S) :
On August 12, 1997, the Board of Supervisors approved Contract
#26-158-16 (as amended by Contract Amendment Agreement #26-158-17)
with Antonio G. Britto (dba Concord Transportation Services - Sal ' s
Taxi of Martinez) to provide taxicab services for Contra Costa
Regional Medical Center to provide transportation for patients to
and from the Hospital and other health facilities, and for
transporting Mental Health patients to court, Board and Care homes,
and recovery programs .
Approval of Contract #26-158-18 will allow the Contractor to
continue providing services through June 30, 1999 .

(Frank Puglisi,) , to execute on behalf of the County;, Unpaid Student
Training Agreement #26-211.-2 with Regents of the University of
California, on behalf of University of California, San Francisco, School
of Medicine, for the period from June 1, 1998 through May 31, 2001 for
the provision of clinical experience for physical therapy students.
FISCAL; IMPACT:

BACRt t3UNDIREAB N(S) FOR ItECOMMENDATION(S) :
The purpose of this agreement is to provide physical therapy students
with the opportunity to integrate academic knowledge ; with application
skills and attitudes at progressively higher levels of performance
requirements and responsibility. Supervised field work experience in
physical therapy is considered to be an integral part of both the
educational and professional preparation. As a teaching hospital, Centra
Costa RegionalMedical Center and the Contra Costa Health Centers can
provide the requisite field education, while at the name time, taking
advantage of the students' services to patients.
Approval of Unpaid Student Training Agreement #26-211-2 will allow
University of California, San Francisco students to continue to receive
field instruction and experience in physical therapy at Contra Costa
Regional Medical Center and the Contra Costa Health Centers through May
31, 2001.

MMENDET3-ACTION:
Approve and authorize the Health Services Director or his designee
(Wendel. Brunner, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #22-219-24 with Compass Group USA, Inc.
(formerly Canteen Corporation) effective June 1, ; 1998, to amend
Contract #22-219-22 (as amended by Administrative Amendment.
Agreement #22--219-23) , to increase the Contract Payment Limit by
$14, 602 from $915, 577 to a new total payment limit of $930, 179 .
This amendment will also increase the three-month automatic
extension payment limit by $3 , 551, from $228, 894 to a new total
extension payment limit of $232,545 .
FINANCIAL IMPACT:
This Contract is Federally funded through the State under Title
EI C-1 and Title TTT C-2 of the Federal Older Americans Act of 1965 .
No County funds are required.
REASONS FOR RECQMMENDAT10NS/BACKGR0UND:
On July 22, 1997, the Board of Supervisors approved Contract
#22-219-22 (as amended by Administrative Amendment Agreement #22'-
219-23)with Compass Group USA, Inc. for the period from July 1, 1997
through June 30, 1998, to provide 10 congregate and home-delivered
meals for the Senior Nutrition Program.
This amendment reflects an increased demand for home-delivered meals
for the elderly and AIDS/HIV diagnosed citizens of Contra Costa
County. Approval of Contract Amendment Agreement #22-219-24 will
allow the Contractor to provide additional meals through June 30>.,
1998 .

I. RECOMMENDED ACTION:
Authorize the Executive Director, Contra Costa County Private industry Council, to execute a standard
form Title V Senior Community Services Employment Program Contract Amendment/Extension
Agreement with agencies listed below to extend the term of the contracts from June 30, 1998 to August
30, 1998,

Contra Costa Hospice
City of Richmond
Family and Community Services
II. FINANCIAL IMPACT:
None,one hundred percent federal funds.
III. CHILDREN IMPACT STATEMENT:
One CIS goal is addressed: Communities that are Safe and Provide a High Quality of Life for Children
and Families. Employment of seniors contributes to self-sufficient families and healthy communities.
IV. REASONS FOR RECOMMENDATION:
By its order June 24, 1997, the Board approved standard form contracts (19-5000-0 through 19-5004-0)
with Senior Community Services Employment Program agencies to provide Title V Employment Services
to seniors. The purpose of the amendments is to allow agencies more time employment seniors and
expend all funds for Program Year 1997-98.
The purpose of the amendments is to extend current year contracts through August 30, 1998 to allow
expenditure of all budgeted funds.
