BACKraROUND/REASON(,S)FOR REQ0_NM[tiD 0 (S)• On June 8, 1998, the Family and Human Services Committee met with Nina Goldman, Program Manager for Service Integration. Also present were Katherina Zulliger, Bay Point Works Project Manager; Stephanie Gantt, North Richmond Employment Collaborative Project Manager; Annie King-Meredith, North Richmond Employment Collaborative Assistant Project Manager; Lorraine Gauli, Employment Specialist; and Sandy Marsh, Mental Health Division Manager. Ms. Goldman reviewed the attached report with the committee in some detail. Both Supervisor Donna Gerber and Supervisor Mark DeSaulnier were pleased with the outcome data, which demonstrate the results of the program. In addition, the high level of participation in the community was seen as very valuable. Ms. King-Meredith distributed a draft copy of the North Richmond Community Resource Guide, which had received considerable community input concerning its format and its content. The guide is to be distributed as a refrigerator magnet to all households in North Richmond. Community involvement was also a tribute to the success of AC Transit Route 376, serving North Richmond. The route now averages between 150 and 178 riders every evening. Similarly, in Bay Point, community involvement has resulted in high levels of participation at the Bay Point Community Career Center with 210 visitors in the first three weeks of operation. Kim Belshe, Director of Health Services for the State of California, recently visited the Bay Point Family Service Center and Wellness Center. Introduction The Contra Costa County Service Integration Program is a multi-disciplinary collaboration of staff from the County Departments of Wealth Services, Probation and Social Service,the Fount Diablo Unified School district, community-based organizations and community members. Located in Family Service Centers in two of Contra Costa's most distressed communities, Bay Point and North Richmond, the Service Integration Teams (SITeams) provide holistic, community-based, family-centered services. The SITeam approach focuses on family strengths, and builds upon these strengths to help families achieve their goals. The Service Integration Program strives to achieve the following three goals: )9- increase family economic self-sufficiency; >- increase family stability; >► expand community capacity to support children and families. This report presents program outcomes for the Service Integration Program for calender year 1997, as well as for 1998 year-to-date. The report then provides an overview of the Service Integration Team case management process. The final sections of this report provide Service Integration Program developments and progress notes since our last report to the Family and Human Services Committee in December 1997; these issues are framed in terms of the three Service Integration Program goals. Page 9 11, The Service Integration Case Management Process The SIT case management process is the heart of the SlTeams' work with families. Over the past year, the SlTeams have worked closely with a training team from the Greater Bay Area Family Resource Network (GBAFRN) (supported through a grant from the S.H. Cowell Foundation)to refine the Service Integration Case Management process. Designed specifically for and with the SlTeams,the case management process ensures that families receive integrated services tailored to help meet their specific needs and goals. Each family who volunteers to be a part of the Service Integration Program must sign an authorization form that allows SlTeam members to share information regarding this family. After this document is signed, a SlTeam member completes a Family Assessment Record (FAR) with family members (see Attachment A). The FAR guides the SIT case management process by assisting family members and team members to jointly. define goals; identify barriers or problems in achieving goals; identify strengths and resources; agree on service outcomes; plan actions; acquire useful tools, services and resources; carry out their plans; and evaluate outcomes. In an intensive conversation or series of conversations, the SlTeam member completes the FAR with family members to assess how the family is doing in a range of inter-related life domains, such as employment, child care, child and adult health, transportation, social support, etc. This process captures a holistic picture of the family and helps get the SlTeam members and the family onto the same page. Each family then completes a Family/SlTeam Action Agreement, which helps the family identify goals and lay out the steps to progress. This document is then used as a reference tool for the family and SlTeam members to remember their commitments, follow up with one another and celebrate achievements. Each time a family achieves its stated goals, a new Family/SlTeam Action Agreement is completed. Every two weeks, the SiTeam members come together for a SlTeam'"Triage Session." Triage Sessions provide an opportunity to engage the resources of the whole team; to troubleshoot, brainstorm and help each other strategize about how to work most effectively with SIT families; to assure coordination of SlTeam efforts; and to keep all SlTeam members abreast of the progress of SIT families. At the Triage Sessions, SlTeam members formally present new cases to the team and update each other on achievements and barriers facing families discussed at past sessions. During the Fall of 9998, the GBAFRN Training Team will be working with the SlTeam members on refining SIT Family Conferences. At Family Conferences, SlTeam members come together with family members and friends to develop a support network to help them achieve their goals. Family Conferences are reserved for those families who are in crisis, who are receiving services from multiple providers, who are facing particularly daunting issues and who are open to this type of approach. Page 2 In addition to these formal collaborative processes, the SlTeam members work together through a number of informal mechanisms. SITeam members serving the same families conduct joint home visits, particularly in cases where one team member has particularly good rapport with the family and a new team member is entering the case. SITeam members exchange numerous resources and tap into each others'',areas of technical expertise. For example, when a family receiving public assistance has a teenage son who is at risk of entering the juvenile justice system, the Employment Specialist may consult with the Juvenile Probation Officer and ask him to informally provide some prevention counseling to the family. SITeam members update each other regularly on a day-to-day basis regarding common families. Our experience with Service Integration thus far indicates that these informal mechanisms are as critical to effective teamwork as are the more formalized collaborative processes. III. Program Outcomes for Calender Year 1997 and 1998 Year-to-Date The Service Integration Program has an excellent staff comprised of Employment Specialists, Employment Specialist Supervisors, Senior EmploymentSpecialists, Child Welfare Workers, Mental Health Clinicians, a Public Health Furse, Juvenile Probation Officers, Clerical Staff, Public Safety Officers and, at the Bay Point Center, Family Resource Specialists from the Mt. Diablo Unified School District's Healthy Start Program. Staff members have worked extremely hard over the past year to refine the SITeam case management process and revise the Family Assessment Record (FAR), the central case management tool. Their hard, high-quality work is reflected in the many measurable improvements we have seen in the lives of the families we serve. While we acknowledge that only a portion of the impact of the SITeams' work is presently captured in trackable outcomes, we are striving to become more sophisticated both in terms of how we track data and in the types of data tracked. This section focuses on some of the outcomes achieved in 1997 as well as year-to-date outcomes for 1998. *** BAY POINT SITeam OUTCOMES *** rl There continues to be a marked increase in employment among Bay Point SIT AFDClrANF participants. As illustrated in Figure 1, a comparison of three points in shows notable growth in the number of AFDCITANF participants reporting incomes at the Bay Point SIT, the Antioch District Office and Countywide over the past two years. While this trend is reflected across the County, the increase has been particularly pronounced for those served by the Bay Point SIT. In April 1998, the Bay Point SII`hit a team record of 27.6 percent of all AF©CITANF participants reporting earned Incomes. of An estimated 117 AFDC/T'ANF participants served by the Bay Point SITeam joined the paid work force in 1997. Of the AF©CITANF recipients who got jobs In 1997, an estimated 31 percent received no welfare check In June 1998. Wages ranged from $5.15 an hour for a job at Taco Bell to $12.50 an hour for a Claims Examiner position. el During 1997, a monthly average of 93 percent of all youth on probation at the Bay Point SlTeam were either enrolled or had graduated from school. V In a partnership with the Mt. Diablo Unified School District's Bay Point elementary schools and Foster Youth Services, the SITeam's Mental Health Counselor has co-facilitated two Anger Management groups this year; a third group is currently in session. Pre- and post- test assessments were conducted on all of the 25 9-11 year olds who participated in the program. The Teacher-Child Rating Scales indicated significant behavioral improvements in areas such as Acting Out, Peer Social Skills, Learning Skills, Frustration Tolerance and Task Orientation. ** NORTH RICHMOND SlTeam OUTCOMES'' V/ 20.7 percent of all AFDCITANF participants served by the North Richmond SlTeam reported earned Income in April 1998. This share is up from 14.1 percent in April 1996 and 19.2 percent in April 1997 (See Figure 2). t`' An estimated 58 AFDCITANF participants served by the North Richmond SlTeam obtained jobs in 1597. The 1997" annual figure for the number of North Richmond SIT AFDCITANF participants obtaining employment is 146% of the North Richmond SITeam's 9996 annual figure. 01 Of the 58 North Richmond SlTeam AFDCITANF participants who obtained jobs in 1997, 33 percent received no welfare check for June 1998. These North Richmond residents obtained jobs including a $13.00Ihour position as a Customer Service Representative and a $13.501hour job as a Maintenance Worker. In 1997, 76 families with a total of 197 children received SIT Child Welfare Services from the North Richmond SlTeam. only eleven of these children were removed from their parents' homes in 1997. (One of these children was actually moved from one foster home to another.) The 1997 share represents a slight decrease from 1996, when 7.2 percent of NR SIT CPS children (13 children)were put in out-of-home placement.' 1 information for december 1997 is not available and is not included in these totals. Also, because the SlTeam Child Welfare Workers practice early intervention services, there is no comparable data from the Social Service district Offices. Service Integration Program Developments and Progress Notes GOAL#1: INCREASE FAMILY ECONOMIC SELF-SUFFICIENCY In the Fall of 1997, the Zellerbach Family Fund awarded the Service integration Program with a $201,000 grant to support the goals of Bay Point Works (BPW) and the North Richmond Employment Collaborative (NREC), two community-wide employment coliaboratives aimed at helping community residents obtain jobs and move up the employment ladder to economic self-sufficiency and security. By December 1997, the BPW and NREC Project Managers were on board, beginning to build the foundations of community-wide employment systems. This weaving together of public, private and community resources is already proving synergistic. During the early months of 1998, the Bay Point Works Project Manager, SITeam staff and three dedicated community residents came together to form the ',Bay Point Works Community Career Center Design Committee. The Design Committee worked diligently to develop the Bay Point Community Career Center,a neighborhood resource room where all residents would have access to computers,telephones, a fax machine, a copier, many job listings and other employment-related information and services. The resign Committee planned the "look" of the Center, the hours of service, the staffing and the grand opening ceremonies. On Saturday, May 9th, Bay Paint Works hosted the grand opening of the Community Career Center (see Attachment B). Over 200 people attended this festive community event. At the event, a number of agencies and individuals were 'honored for their contributions to the establishment of the Career Center. In particular, the Contra Costa County Social Service Department and the Zellerbach Family Fund received special acknowledgment for providing the funding that made the Career Center'possible. Among those attending the event were Cindy Rambo, Executive Director of the Zellerbach Family Fund, Supervisor Joe Canciamilla and John Cullen, Director, Social Service Department. During the first 3 weeks (16 business days) following the May 9th'celebration, 210 people visited the Bay Point Community Career Center. On average, 14 visitors came to use the Center each day. Already 67 individuals have signed up for the Career Center membership, which allows them to regularly use the Center's resources. The Career Center is staffed by four Bay Point residents. We are in the process of developing a formal system for tracking jobs obtained by people who utilize the Career Center. Page 5 In addition, Bay Point Works has completed two six-week "Keys to Success" employment workshops to date (in collaboration with St. Vincent de Paul) and is in the process of completing its first four-week "Job Club/Job Search" workshop. Both workshops will be offered at regular intervals throughout the year. Of the 31 people who have received direct services from BPW in 1998, 17 have gotten jobs. In June and July, BPW staff will initiate the Working Parents Network., which will be run jointly by a community member and SlTeam staff. The Working Parents Network will provide mutual support opportunities for families who are struggling with the challenges of balancing the responsibilities of their jobs with their family lives. In addition, the BPW Job Troubleshooting Hotline will provide a 24-hour/day service to Bay Point residents having trouble on the job; new and returning workforce members will be able to reach a trained BPW staff member any time of the day or night to help work through on-the-job challenges. North Richmond EmplQyment C: l a r C • Moving Term Economic mic Success During the summer and fall of 1997, NREC worked closely with AC Transit officials to establish the North Richmond Night Jobs Shuttle (AC Transit Line 376). November 24, 1997, marked the launching of Line 376, which provides hourly bus service that connects North Richmond with key employment centers in the greater Richmond Area between 7.30 p.m. and 1:00 a.m. NREC staff have continued to work closely with AC Transit to ensure high ridership levels. Currently, night ridership on Line 376 averages a healthy 150-175 people per night. AC Transit also reports that the 376 is now the most popular route requested by AC Transit drivers (contradicting previous fears for driver safety). On April 20th, 1998,staff From the Federal Transit Administration visited the North Richmond Family Service Center to meet with AC Transit and NREC staff to learn more about the innovative community-based transportation planning model implemented in forth Richmond. AC Transit recently received a grant of $20,000 from the Zellerbach Family Fund to conduct a marketing campaign to increase ridership on the 376. NREC; staff are working closely with AC Transit to develop and run the marketing campaign at the grassroots level. In early June, NREC will be hiring an estimated 24 North Richmond residents to distribute flyers, bus schedules and free bus tickets to potential riders. Forty-three North Richmond residents have submitted applications for these positions; all 43 of these individuals have been invited for interviews.. NREC's Transportation Liaison was recently invited to join AC Transit's Ridership Advisory Panel, and has participated in numerous forums representing the public transportation needs of poor communities. The current North Richmond Night Jobs Shuttle costs an estimated $300,000 per year. Clue in part to the advocacy of wide range of public officials, including Congressman George Miller, Assemblywoman Dion Aroner and Supervisor Jim Rogers., as well as to the hard work of staff from a number of Contra Costa County and other public agencies Fuge 6 (including the Contra Costa County Departments of Community Development and Public Works,the County Administrator's Office,the Metropolitan Transportation Commission,the Contra Costa Transit Authority(OCTA)and the West County Transportation Authority),AC Transit was able to secure $88,000 in Community Development Block Grant funds and $63,000 in CCTA funds to support the coming year°s service. AC Transit's recommended FY 98-99 budget allocates an additional $130,000 to support the 376. The current challenge is to cover the $20,000 shortfall as well as to continue to loop at the possibilities for more frequent service. NREC staff have worked with SlTeam staff to revitalize an employment resource area in the North Richmond Family Service Center. The North Richmond Missionary Baptist Church recently joined the NREC partnership. The church has offered NREC space for establishing the North Richmond Community Career Resource Center as well as for running employment workshops for community residents. A committee comprised of S1Team and NREC staff, church members and community members are currently in the process of designing the center. NREC and the North Richmond Missionary Baptist Church hope that this endeavor will provide an innovative model for public agencies and community-based organizations to form new partnerships with the faith community in helping move people from welfare to work. Starting this summer a wide range of NREC work experience and job skills building opportunities will be offered to North Richmond residents by Rubicon Programs' Project OPEN! Opportunities will include subsidized employment in the private sector as well as at Verde Elementary School and community organizations located in North Richmond. Other upcoming NREC activities include: i On June 22nd, NREC will be conducting a hiring workshop;jointly with United Parcel Service (UPS) in North Richmond for positions that are currently available. * Through its partnership with Greater Richmond Social Services, Inc., NREC will be able to offer community members work experience opportunities in the local child care center, while increasing the availability of child care slots for children ages 3-5. • NREC is also in the process of finalizing a comprehensive Neighborhood Resource Guide which will be distributed to all 700 'North Richmond households as well as to local service providers. Page 7 GOAL#2: IMPROVE FAMILY STABILITY Substance Abuse Services r Family Centers` The Service Integration Program has entered into a partnership with Community Substance Abuse Services, Health Services Department, and New Connections, a community-based organization,to provide limited substance abuse services to the families served by the SITeams. Community Substance Abuse Services has made a one-year, $10,€300 commitment to supporting these services. In addition, in April 1998, the Community Development Block Grant awarded the Service Integration Program with $15,000 for substance abuse services to clients of the Family Service Centers. The SITeam substance abuse counselor is expected to come on board in late summer 1998. Service prevision will focus on pre-treatment substance abuse services to families as well as technical assistance to SlTeam members who are working with families with such issues. We greatly appreciate the support of Community Substance Abuse Services and the Community Development Block Grant program to help provide this much needed service to the communities of Bay Point and North Richmond. Family h The North Richmond SITeam has partnered with the West County Child Welfare Services Division to establish a beautiful new Family Room at the North Richmond Family Service Center. District Child Welfare staff needed an accessible and comfortable location where families that are attempting to reunite could meet together. The North Richmond location proved a good fit. Child Welfare Services supported the transformation of a formerly dingy office space into a warm, family-friendly environment complete with a sofa, an armchair, carpeting, artwork and children toys. This shared space provides a positive environment that supports both programs' work with families. GOAL#3: EXPAND COMMUNITY CAPACITY TO SUPPORT CHILDREN ANIS FAMILIES North. Richmond r;mplgyment Collaborative Undem X In January 1995, NREC established the Program Manager Mentorship Initiative (PMMI), which has the goal of building indigenous community capacity to lead the NREC Project. The NREC Assistant Project Manager is a North Richmond resident who was chosen by a hiring committee comprised predominantly of North Richmond community members. She Page 8 will be formally mentored over a 15 month period by NREC's lead Project Manager. In the Spring of 1999, she is expected to transition into the Lead Project Manager position. This exciting initiative strives to provide a model for developing local leadership skills in North Richmond, as well as in other distressed urban neighborhoods. This position is funded by a $21,100 grant from the James C. Penney Foundation and $4,1€I4 grant from the East Bay Community Foundation. FamilyNQrth Richmond i Qgnter to C In the Fall of 1997, the Service Integration Program was approached by staff from the Community Housing Development Corporation of North Richmond (CHDC) about the possibility of leasing commercial space at a new facility being built on the corner of 3rd and Chesley in North Richmond. While the current North Richmond Family Service Center has many benefits, including lavish garden space, the location is on the outskirts of North Richmond and is not easily accessible to North Richmond residents. Consequently, members of the North Richmond SITeam strongly endorsed such a move. In early 1998, the SIT Executive Oversight Committee, comprised of the Directors of the Departments of Health Services, Probation and Social Service and the County Administrator, approved the recommendation that the North Richmond Family Service Center re-locate to the 3rd and Chesley site. The new facility will be adjacent to the new North Richmond Center for Health, which should farther promote inter-agency collaboration. The ground-breaking for this new facility is scheduled for the Fall of 1998. The North Richmond SITeam's tentative move-in date is late summer 1999. Bay Point and N!Qrtb Richmond Comm1inity M!gm_b_grsi The addition of community staff to the SITeams has greatly enriched the day-today work of the Service Integration Program and broadened our understanding of the communities we serve. The SITeams have expanded to include staff from Bay Point Works (BPW) and the North Richmond Employment Collaborative(NREC), two employment projects that are being funded largely through a grant from the Zellerbach Family Fund'. These new staff include: Project Managers, an Assistant Project Manager, a Community Career Center Coordinator, Community Resource Specialists, a Childcare and Working Parents' Network Facilitator and a Transportation Liaison. V. Other Updates Earnify Fund Board of DirectorsVisit the SIT Employment Prplegk On July 17, 1998, the Zellerbach Family Fund Board of Directors will be conducting a site visit to the Bay Point and North Richmond Employment Collaboratives to view the progress of the collaboratives. The site visits will include tours of the facilities where BPW and NREC activities are underway and meetings with BPW and NREC staff and participants. Page 9 Minicuccl Aasocla.es Evaluatlon Underway Minicucci Associates, a Sacramento-based consulting firm, has initiated an evaluation of the Service Integration Program. The Evaluation Team developed a proposal, which was reviewed by the members of the SIT Executive Oversight Committee. The Oversight Committee members had a number of suggested changes, which have now been incorporated into the evaluation plan, which will focus on assessing the impact that the program has on the families it serves. The evaluation is expected to be completed by early 1999. Second Issue of SIT Insights Published The Spring 1998 issue of the Service Integration Program's new quarterly newsletter, SIT Insights, encapsulated many program updates for a readership thatincludes funders, community members and other service providers.
