RECOMMENDATION:
CONSIDER and ACCEPT report from the Health Services Director or designee
(Milt Camhi) regarding the Community Health Education Grants and the recipients
of this year's awards.
BACKGROUND:
Contra Costa Health Plan earmarks a portion of its marketing budget for Community
Health Education Grants to help improve the health of all Contra Costa residents.
This year the Managed Care Commission reviewed each application and
recommended three organizations for the award: the Family Stress Center, the
Pittsburg Pre-School Coordinating Council and the YWCA.
The Family Stress Center received a grant to help support their Latino Family
Project/Perinatal Health and Wellness Program. The Pittsburg Pre-School
Coordinating Council was awarded a grant to help fund its Perinatal Education
Project. The YWCA was awarded a grant to help fund its Perinatal and Dental
Health Programs.

Dear Contra Costa County Non-Profit Organization:
We are delighted to announce the second funding cycle of the Contra Costa Health Plan's
Community Health Education Grants.The Contra Costa Health Plan has again committed
five percent of its annual advertising budget to provide funding for health education for
young families and this year's grant is for$15,000.The 1948 grants,ranging from$5,000
to$7,000,are intended to provide support for community based non-profit organizations,
school groups and the faith community to foster assroots health ed at' n'activites.
Working with the Contra Costa Health Plan's Managed Care Commission,we are
accepting applications for grants that specifically conduct health education activities
directly related to the high priority areas of diabetes,dental health, asthma, and perinatal
health.
This is an excellent opportunity for organizations that already deal with families and
special populations to focus their efforts on health education. We hope these grants will
encourage you to take a look at where health education might be included in other
services you might provide to young families.
Enclosed is the Request for Proposals briefly describing the program and application
process. Please share this RFI'with those individuals in your organization responsible for
planning,delivery and or evaluation of health education for low income families.
Grant applications are dere no later than the end of business on March 16, 1998.

CONTRA COSTA HEALTHPLA ''S COMMUNITY EDUCATION GRANT
FOR M:EDI-CAS,AND LOW INCOME YOUNG FAMILY
The Centra Costa.Health Plan(CCHP)is a twenty-five year old publicly sponsored HMO
serving county employees,small businesses,individuals and families in Contra Costa
County.Service is provided primarily at the Contra Costa Regional Medical Center and
14 health centers located throughout the county.
The Contra Costa Managed Care Commission provides guidance to CCHP and will be
involved in the awarding of grants.The Commission will support the goals and outcomes
of the projects that are chosen.
Proposals for an education grant are doe in the office of CCHP"no later than the end
of business on.Monday,March 16, 15198.Grants will be awarded April 1, 1998 and
project funds are expected to be expended by March 31, 1999.
This year the Commission identified the following four priority areas for health education
grants:
1. Perinatal issues including early care,breast feeding support and early
parenting shills.
2. Diabetes juvenile and/or adult onset.
3. Dental health.
4. Asthma.
Application Requirements
• Proposals of more than two pages(excluding documentation and;appendices)
will not be accepted.
• Nott-profit (501(c)3 organizations applying for funding must serve
Medi-Cal and low-income young families.
• Proposals should include descriptions of: '..
Current health education program goals and how this project fits within
the current programming
"l he project description including time line,brief budget description,of
the clients and potential numbers to be reached.
• A copy of your most recent annual report and audited financial statement
and IRS identification number indicating your 501(c)3 status.
Selection Criteria:
• What are your expected outcomes and how will you measure and evaluate
there.
Clarity of project description
• Breadth of proposed intervention.
• Potential to improve the health of those it serves
• innovation and ability to improve delivery of health education
• Potential future contribution to other organizations who serve this population

• The reasonableness of the proposed budget and the feasibility of achieving
project objectives within the proposed timeline and budget
• .Projects where CCHP Health Education funds can be matched or leveraged
with other health education efforts to the target population will be given high
priority.
This Project is directed by the Executive Director of the Contra Costa Health Plan and the
Marketing and .Education Committee of the Managed Care Commission. The Managed
Care Commission will assist CCHPin evaluating and reviewing proposals. The timeline
for the grants is:
TIMELINE
March 15, 1398 - Proposals due at 595 Center Street, Suite 100 Martinez by end of
business -Spm.
April 15, 1998-Grant recipients announced. Awardees will be invited to give a
presentation to the Managed Care Commission of progress on their projects in October
1998.
March 31, 1999 -A Final report, no longer than two pages giving a summary of project
activities and outcomes is due.
PROJECT EXAMPLES
Single Agency: A small non-profit after school program observes that more children are
experiencing asthma with exertion. They develop a program to work with a physician and
nurse to teach parents and children about exercise and asthma to control episodes and
teach all the family what exercises are beneficial.
Collaboration: Three community based organizations, a day care center, a senior center
and a neighborhood church collaborate to develop a mentoring program for young
families who have children in the day care center. Seniors from the center and church
members are trained by the day care nutritionist,and activities coordinator of the senior
center to help young families plan meals that are nutritious and low in fat content.
Monthly meetings are held at the church to discuss nutrition, a potluck dinner is served,
using nutritious recipes from the seniors. Church members provide baby sitting during the
"class"portion:of the monthly meeting.The expected outcome is better understanding of
nutrition needs of young children, lessons in cooking nutritious foods and a chance for
seniors to become"mentors" or"grandparents" to young families. In addition, because so
many families have both parents working or single parents that are rushed at meal time an
emphasis will be placed on quick, nutritious meals.

I am sorry that we are not able to fund all of your proposals. However,T am very pleased to inform
you that you have been awarded a grant for$5,000 for your innovative perinatalEducation project.
The proposals we received were thoughtful and highlighted the many unmet health education needs
of our Contra Costa Community.
The Managed Care Commission and Contra Costa Health plan are grateful for the community
interest that was sparked by the health education RFP. As usual,there are unlimited needs and a
limited pool of funds.
Grant recipients will be announced to the public on April 15, 1998, at the Managed Care
Commission meeting at 4:30 p.m. at the Holiday Inn, 1050 Burnett Avenue,Concord. In addition,
you will be invited to the Contra Costa County Board of Supervisors meeting the morning of May
12, 1998, for a special presentation. We will call to confirm both dates and times.
In October,grantees will be invited to present a progress report to the Managed Care Commission.
A final report,no longer than two pages,giving a summary of project activities and outcomes is due
March 31, 1999.
All printed materials,educational as well as allied reports,must acknowledge and credit the receipt
of a Community Health Education Grant from the Contra Costa Health Plan.
