Approve and authorize the Health Services Director; or his designee (Wendel Brunner, M.D. ) , to execute on behalf of 'the County, Standard Agreement #28--538-5 (State #97-1.2016) with the State Department of Health Services', in the amount of $5, 000, for the period from March 1, 2998 through June 30, 1998, for the On The Move With <Foodwise Project . FINANCIAL IMPACT: This Agreement 'provides $5, 000 of State funding for the On The Move With Foodwise Project . No County funds are required. CHILDRMI S IMPACT S'T'ATEMENT: The "On the Move with Foodwise"" Project supports two of Contra Costa County's community outcomes: (1) "Children Ready for and 'Succeeding in School" and (2) "Children. and ; Youth Healthy and Preparing for Productive Adulthood" , by improving the physical activity of low income children and families in selected Contra Costa communities. It is anticipated that the program. outcomes under this Project would relate to children becoming physically healthier, increasing their ability to stay in school, and leading a healthier adult life. REASONS FOR RECOMMEMATIO S/BACKGRgUM: This Contract will allow the Department' s Community Wellness and Prevention Program to promote physical activity messages through local media channels and host physical activity events in regions of Contra Costa County with high risks for cardiovascular disease. The goal of this project is to promoted the benefits of physical activity and an active lifestyle to Contra. Costa County' s population, who are at an increased risk of cardiovascular disease. Three certified and sealed copies of the Board Order should be returned to the Contracts and Grants Unit for submission to the State Department of Health Services . RECQMMENDED ACTION: Approve and authorize the: Health Services Director,; or his designee (Donna Wigand, ;LCSW) , to execute, on behalf of the County, Contract #29-499 with Santa Clara County, for the period from March 19, 1998 through June 30, 1998, to pay the County $10, 500, for provision of professional treatment services for duallydiagnosed (developmentally disabled and emotionally disturbed) adolescents. ', II . FI ANC14 ZMPACT: Approval of this Agreement will result in a total payment to Centra Costa County of $10, 500 . III . REASONS FOR RECC3MMEMDATIONS./BAC GGRC>UND: On February 3, 1998, the Board of Supervisors approved Standard Contract #24-920 with Fred Finch Youth Center to provide intensive day treatment program and medication support services for seriously emotionally disturbed children (dually diagnosed) at Fred Finch Youth Center Residential/Day Treatment Programs. Contra Costa: County is acting as the host county for the Fred Finch Program, therefore, counties needing services will need to contract with Contra Costa for those services . Under this Contract, Santa Clara County will pay the County $100 per day for the use of one bed through June 30, 1998 . REC�Qj0MEXDATI0 Approve and authorize the Health Services Department to a issue 90-day advance written notice to the Regents of the University of California (U.C. Davis School of Medicine Neurology Department) to terminate the Agreement #26--151-10 (as amended by Amendment Agreement #26-151-11) , effective close of business on June 30, 1998 . II . FISCAL IMPACT Nene. Neurology services provided under this Agreement will be contracted directly with individual Neurologists and services are billable to patients and appropriate third party payors. The actual cost to the County depends upon utilization. III . HACKGRC?� REASON(S) FOR RECQZ--= DATIQ_ (S) On November 8, 1994 , the Board of Supervisors approved. Contract #26- 151-10, (University #94-05872V) as amended by Amendment Agreement #26- 151-11, with the University of California (U.C. Davis School of Medicine Neurology Department) , for the period from October 1, 1994 through September 30, 2000, for the provision of professional neurology and neuropsychology services at Centra Costa Regional MedicalCenter. Since 1986, the County has been contracting with U.C. Davis for professional neurology and neuropsychology services! Effective July 1, 1998, the Department will be contracting directly with individual Neurologists. Approval by the Board of Supervisors will allow the Health Services Department to issue a ninety-day advance written notice to the Contractor, in accordance with Paragraph C.2 (Term) of the Agreement #26-1.51-10 las mended by Agreement #26-151.-11) , that the agreement is terminated effective June 30, 1998 . Approval of Contract #27-378 will allow the Contractor to provide ' professional health care services to Health Plan members through ' April '30, 1999 .
