In some cases, capital improvements are necessary to brine the program up to federal standards. Mr. C$1on noted that, in one case, federal authorization and funding had been given to purchaseand install neve classroom modules in November. However, the modules have stili not been put out to bid by General Services. Delegate staff were also invited to attend this workshop. The in- service covered the reasons for obtaining and evaluating these indicators,the correct methods for obtaining heights and weights and methods for graphing. The evaluations from this training indicate a high level of understanding with regard to charting heights and weights and when to refer. Heights and Weights were recorded on growth charts for all children by January 1$, 1998. The next round of growth charting will be accomplished by March 16, 1998 using the computer-graphing program developed for ChildPlus. The Medical Quarterly Report along with file reviews will be used by the Family Services Managers to monitor and ensure completion of this task. Comp 09t n Date 'bUrch I6, 1998 .. Item#88: Active involvement orf parents in pkmnrng and implementing the individual mental health needs of their children.(8) Deficiency: Parents are mailed a needs survey which they are asked to fill out. This is not a very effective way of discovering real family mental health issues or needs'(eg. Lack of parenting skills,abuse, drug use,etc). Also there was not much evidence of parental inclusion in mental health. Action: We recognize and value the importance of family involvement in mental health issues, and in order to accomplish family-focused early intervention, AtAU response.docAprH B, 1998 we have established the following sequence of activities. These specific activities relate to planning, implementing, and evaluating family services in general,with the implementation of mental health support activities as necessary. ♦ The Family Partnership Appraisal tool,implemented this year,is used by Family Service Workers with each family on a one-to-one basis. Many dimensions of family functioning can be assessed with this tool such as parent-child interaction, child variables relevant to family function,and family needs for support in various areas of their lives. ♦ The Family Partnership Appraisal generates information about each family which is examined,with high-priority needs being identified. r The Family Service Worker then conducts a focused interview to get more parental input and clarify their needs. ♦ A Family Partnership Agreement is developed based on the above,formalizing specific goals and needed services. The Disabilities/Mental Health Manager will participate,if necessary,at this point,working with the family and the Family Service Worker to identify the services or strategies necessary to respond to identified needs within the family. ♦ Services are implemented with Head Start staff serving a facilitators or case mangers of services. t Each family is continually evaluated as to the effectiveness of services.New goals and strategies are established as necessary. NUTRITION Item#9o: The nutrition assessment data(height, weight, hemoglobinJhematocrit) ohtwned,for each child(1):[see Health#441 Deficiency: Seven out of twenty-eight files reviewed did not have height and weight graphs and some Family Service Workers did not understand the reason for obtaining and evaluating these indicators. Action: Same response as above in Item#44. (3) Deficiency: Food served for lunch at Fairgrounds,the only site where lunch was observed in the Delta Division,was not appropriate. Sandwiches, canned half apricots and celery were adult size and Children had a hard time serving themselves and eating this food. Action: Results of the review have been shared with our food service vendor. Classroom staff will have a review of appropriate serving sizes and will be encouraged to Cut food into smaller pieces through site meetings. Serving sizes were covered in an in- service in the Bay Division on February 18, 1399. Additional emphasis will be given to appropriate serving sized in fall pre-service. COm letion Date: Febrsia 1$, 199$ Item# 103: Chairs, tables, and eating utensils are suitable for the size and developmental level of the children, with special consideration for meeting the needs of chiA*en with disabilities (S) Deficiency: At Fairgrounds,the only utensils used for eating were sporks. Children had a very hard time eating their apricots.Forks, spooks and knives are not available at all.Plates used were flimsy and very light weight. Action: The food vendor was contacted immediately after the review. Children are now provided thicker plates and forks and spoons. Sporks are no longer used. Cam lotion Date: xnna Item# 104: Opportunity is provided for the involvement of children in activities related to meal service(for example:family style service). (7) Deficiency: Children did not participate in any activities involving meal set-up. At all sites observed Children were asked to pick up their plate,napkin,utensils,and cups and walk over to the dining table. This was a difficult task for most Children who kept dropping one or more of the items. Action: Education staff has been trained by the education coordinator to use meal service as an opportunity to develop self-help skills. The method for Children setting the table has been modified since the review to minimize the dropping of items. The topic for an in-service in the Bay Division on February 18, 1998,was family style meals. Item# 110: The nutritional status of the children is discussed with the parents. (2) Deficiency; Did not see any evidence that this was done(verbal interview and file review). Action: Training will be provided to the Family Service Workers on how to share the nutritional status of children with parents. Appropriate educational materials will also be shared. (AI) [1304.4-2(a) (6),j Deficiency: There is no tracking system of referrals and follow-up activities to ensure that family needs identified in the Family Needs Assessment are being met. Action: With the assistance of Diane Harkins of UC Davis,a tracking system has been developed to ensure needs are being met as identified in the Family Partnership Appraisal. Training on this tracking form will be accomplished on March 13, 1998. Vacancies arefilled as �05- occur, unless 60 calendar days or less remain in the program Year. [ (b)] Deficiency: The Bay District is under-enrolled in two centers. Vacancies re difficult to fill because parents are either looking for full-time day care or are $ openings at another site that is outside the immediate neighborhood. Action; A small wrap around program was established on February IS, 1998 with one of the Greater Richmond Child Care sites one mile proximity of the Silver and as fMar h 4, 1998 both PM and Las Deltas sites.The response was overwhe & sites were filled. The selling point was the availability of transportation to those parents who were working or in school who could not pick their children up in mid day. -4 - AAACF response.docAprif 6, 1998 The wrap around also provided a reduction in childcare costs for the families. As a result of this small program, a waiting list of eight(8)children is ready to fill any vacancies that occur. �Compi on Dkte 1!Carch 20, 1998 f { t ADMINISTRATION Item # 191: Grantee has an internal monitoring procedure for delegate program operations which: (1i, ) Deficiency: Grantee does not have in place procedures for monitoring delegate agency contracts and program requirements, correction plans and for provision for technical assistance. Action: The monitoring procedures approved by the Policy Council on 5-9-96 provided for a team, composed of component managers and program monitors,to perform full and limited program monitoring. In September 1997 the responsibility for program monitoring was shifted to Area Managers and the Management Teams. Subsequently, each management team member has been assigned to monitor its respective subject matter area at the Delegate Agencies. The monitoring activity will be conducted in accordance with the approved monitoring plan. In the event that technical assistance is needed,the grantee shall provide it upon request of the Delegate Agency. 777 Cam tet oh Date: Un oin Item# 192: Grantee has a procedure for providing copies of regulations guidelines, policies, notices, and general information to delegate agencies. a) Ddiciency: Grantee does not have in place procedures for providing policies,guidelines, and general information to delegates. Action: An Administrative Task Matrix has been developed and implemented to assign clerical staff to perform specific duties. information from the ACF regional office will be copied and mailed to the Delegate agencies as the information becomes available. The Head Start Secretary will be responsible for assuring the mailing of this information. Pr _ A ACF response.docAprii 6, 1998 S'T'AFFING Item #205: A coordinated component wide training model needs to be developed for community volunteers. Deficiency: Volunteer recruitment and training from the wider community is very weak and uncoordinated. A management level staff person is needed to develop closer relationships to colleges,universities,training agencies,businesses,and non-profit organizations. Action: The Family Services Managers are responsible for volunteer recruitment and training and overall coordination from the wider community. The Grantee has been a GAIN Prep Site whereby we utilize GAIN program participants in a variety of areas in the program. This program has been very well coordinated since its inception in 1996. Community volunteers are now screened with a newly developed volunteer application and are required to provide fingerprint clearance and documentation of current negative TB test.The Grantee has also purchased a"Resource Hit for Managers of Volunteers" from assisting the program in advertising for volunteer opportunities. Com etioa Dates March 9,:.998 FISCAL Item#226: Previous audit deficiencies have been addressed and corrective actions have been implemented Deficiency: The June 30, 1996 audit finding addressing the need to prepare a cost allocation plan and/or indirect cost rate proposal that provides a basis for allocating indirect costs to Federal programs has not been completed. Action: Carl Mason, Supervising Accountant for Community Services Department,is developing an Indirect Cost Rate Proposal{ICRP}for the Community Services Department to allow a more accurate basis for allocation of indirect costs to Federal Programs, including Head Start. �i Li 0 rt �' 1 ., ✓i • t "t d_. .. '.,_.s � MACF response.docApril 6, 1998 Item:#242.All contract/agreements are available,appear to be legally sounrt and appear to be in the best interests of the grantee. Deficiency; There are six grantee leases and three delegate agency leases that have not yet been finalized by the Contra Costa County Board of Supervisors. Action: The following three grantee leases and two delegate agency leases have been finalized and approved. 1. Bridgemont 2. Cambridge 3. Gehringer 4. Castlewood (First Baptists) 5_ Lido Square (First Baptist) Fairgrounds:and Sunset are in negotiations. Head Start currently has leases on these sites that have expired,but contain language which allows month to month tenancy as per the "hold over clause",which allows us to stay on indefinitely on a month to month tenancy. Both of these facilities require us to vacate during the summer,one of them floods each winter,and the other has a less than desirable play area. Therefore we continue an intensive search for alternative locations for these facilities, so it is in our best interest to continue in this way until new sites are identified. The West Contra Costa County School District currently owns the Balboa site. The Northern California Community Council,representing our interests,has concluded an agreement for sale with them,and when it closes escrow,we will move into a rental agement with them without a gap in usage. The purpose of the QIP Analysis is to provide clarification and any additional information to support the goals, objectives and activities described in the plan. Agency Purpose for Quality Improvement Plan: The QIP is designed as a working plan. It will be used to a guide by all members of First Baptist Head Start Program; the director, parents, board of directors, fiscal staff, service area specialist and managers,including the teachers and food service staff Although each activity has an achievement date, the QIP as a whole will be monitored and documented (for achievement and/or targeted timelines) by the managers and the director on a monthly basis throughout the program year(97/98). Understanding the Quality Improvement Plan: The QIP is self-explanatory. It Contains the goal, objective, activity, person responsible, measurement of achievement, dates to be accomplished and achieved, staff and parent involved and resources needed. The goal of each non-compliance cited is to meet and perhaps exceed the performance standard as indicated in the OSPRI. The objective is to correct the non-compliance.
