SUBJECT: Approval of Administrative Amendments to Provide Cost-of-Living
Adjustments for Mental Health Contracts
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I . RECOMMENDED ACTION:
Approve and authorize the County Administrator, or his designee, to
execute on behalf of the County, Administrative Amendments, as
specified in the attached "Listing of Administrative Amendments for
Mental Health Contract Programs" , to increase the salary components
by . 7580 for Fiscal Year 1997-98, to make modifications in
corresponding payment and budget provisions, and to ' incorporate
State or County required changes in other provisions (if any) .
II . FINANCIAL IMPACT:
These Amendments provide a . 758•°a cost--of-living increase to the
salarycomponents of the Mental Health Contracts on the attached
list, and increase the FY 1997-98 Contract Payment Limits based on
the attached calculations. Furthermore, these increases are also
reflected in the Payment Limits for the corresponding automatic six-
month contract extensions in those Contracts which contain this
provision.
III. REASONS FOR RECOMMENDATIONS1BACKGROUND:
These Administrative amendments carry out the standing policies and
directives of the Board of Supervisors regarding' cost-of-living
increases for contract programs which are providing a vital and
important part of the County's continum of dare and services for
persons with mental illness . The cost-of-living increase for each
contract is based on a distribution of available County funds in
proportion to the relative size of personnel costs of each contract
payment limit (excluding any one-time-only amounts)

SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMMMED ACTION
Approve , and authorize the Health Services Director, or his
designee (Milt Camhi) , to execute on behalf of the County,,
Contract #27-309 with Muir Primary Care Medical Group, for the
period from March 1, 1997 through June 30 next ensuing and from
year to year thereafter until December 31, 1999, unless sooner
terminated as provided in the Contract. Contractor will be paid
in accordancewith the rates set forth in the agreement, for the
provision of professional health care services for the Contra
Costa Health Plan.
II. FINANCIAL IMPACT:
This Contract is funded by Contra Costa Health Plan member
premiums. Casts depend upon utilization. As appropriate,,'
patients and/or third party payors will be billed for services.
III. REASCI B FOR REC OMMENDATIONSIBACKGRCIUND
On February 1, 1997, the Local Initiative for Medi-Cal managed .
care was implemented in Contra Costa County. Local Initiatives
are required to include traditional Medi-Cal providers from the
community in their provider networks. j
This Contract is necessary to meet State mandates to expand the
number of community providers for the Local Initiative, along with
a recent Department of Corporations audit finding that requires
formal contracts with low volume providers.

I . RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his
designee (Milt Camhi) , to execute on behalf of the County, ,'
Contract #27-228-1 with Mindy Werner-Crohn, M.D. , ; for the period'
from March 1, 1998 through February 28, 1999, to be paid in
accordance with the rates set forth in the agreement, for the
provision of professional mental health services for the Contra
Costa Health Plan.
II . FINANCIAL IMPACT:
This Contract, is funded by Contra Costa Health Plan member'
premiums . Costs depend upon utilization. As appropriate, ,;
patients and/or third party payors will be billed for services.
IIT . REASONS FOR RECOMMENDATIONS/BACKGROUND:
The Centra Costa Health Plan (Health Plan) has an obligation to
provide professional psychiatric services for Health Plan members '
with mental health therapy services as a covered benefit . This
population includes Medi-Cal, Medicare and Commercial members
enrolled in the Health Plan.
Approval of this Contract will allow the Contractor to continue '
providing mental health services to Health Plan members through'
February 28, 1999 .

(.Donna Wigand) to execute on behalf of the County, Contract
Amendment Agreement #24-648-6 with Telecare Corporation, effective
April 1, 1998, to amend Contract #24-648-5, to increase the Payment
Limit by $55,918, from $93,877 to a new total contract payment limit
of $139,795 and to increase the automatic six-month contract
extension payment amount by $27,959, from $41,939 to a new automatic
extension payment amount of $69, 898.

This Contract is included in the Health Services budget and is
funded. by County/Realignment funds 100%.

On January 6, 1998, the Board of Supervisors approved Novation
Contract #24-648-5 with Telecare Corporation, for the period from
July 1, 1997 through June 30, 1998, to provide mental health
treatment services to Contra Costa County geriatric clients.
Approval of Contract Amendment Agreement #24-648--6 will allow the
Contractor to provide additional units of service through June 30,
1998.
