SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his
designee (Wendel Brunner, M.D. ) , to execute on behalf of the
County, Novation Contract 128-540-4 with the City of Antioch,
Leisure Services Department, for the period from July 1, 1997
through June 30, 1998, to provide congregate meal services for the
County's Senior Nutrition Program. This contract includes
provision for an automatic three-month extension through September
30, 1998.
II. FINANCIAL IMPACT:
No County cost is included.
III. REASONS FOR RECOMMENDATION/BACKGROUND:
On November 19, 1996, the Board of Supervisors approved Contract
#28-540-3 with the City of Antioch, Leisure Services Department,
for congregate meal services for the County's Senior Nutrition
Program. The Contractor will provide an average of 90 meals per
day, five days per week, for senior citizens at the Antioch Senior
Center.
Approval of Novation Contract #28-540-4 will continue these
services through June 30, 1998 .

On December 17, 1996, the Board of Supervisors approved Novation
Contract 29-335-10 with the City of E1 Cerrito for congregate meal
services for the Senior Nutrition Program. This program provides
an average of 50 meals per day, five days per week, for senior
citizens in E1 Cerrito.
Approval of Novation Contract #29-335-11 will continue these
services through June 30, 1998.

Contract 129-285-15 with the City of San Pablo, for the period from
July 1, 1997 through June 30, 1998, to provide congregate meal
services for the County' s Senior Nutrition Program. This contract
includes provision for an automatic three-month extension through
September 30, 1998.
II. FINANCIAL IMPACT:
No County cost is included.
III. REASONS FOR RECOMMENDATION/BACKGROUND:
On Janurary 14, 1997, the Board of Supervisors approved Novation
Contract #29-285-14 with the City of San Pablo for congregate meal
services for the Senior Nutrition Program. This program provides an
average of 100 meals per day, five days per week, for senior
citizens at the Davis Park Community Center and the Senior Citizens
Center in San Pablo.
Novation Contract #29-285-15 replaces the automatic extension of the
prior Contract and allows Contractor to continue providing service
through June 30, 1998 .

Contract #24-700-39 with Contra Costa Crisis and Suicide
Intervention Services, for the period from July 1, 1997 through June
30, 1998, with a payment limit of $91,476, for crisis intervention,
suicide prevention, and mental health rehabilitative services.
II. FINANCIAL IMPACT:
This Contract is funded in the Health Services Department's Fiscal
Year 1997-98 Budget (Org. #5942) .
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On January 14, 1997, the Board of Supervisors approved Novation
Contract #24-700-37 (as amended by Administrative Amendment
Agreement #24-700-38) for the period from July 1, 1996 through June
30, 1997 to provide crisis intervention and suicide prevention
services, including a 24-hour hotline, grief counseling and
community mental health education for residents of Contra Costa
County.
Approval of Contract #24-700-39 will allow the. Contractor to
continue providing services through June 30, 1998.

III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
This Contractor supplies trained mental health workers to provide
bilingual/bicultural interpretation and language translation
services; non-therapeutic counseling; and information, referral, and
advocacy services for Southeast Asian refugees to assist the
County's Mental Health staff in providing services to this client
population at the West County Community Mental Health Center.
On December 10, 1996, the Board of Supervisors approved Novation
Contract #24-312-16 (as amended by Administrative Amendment
Agreement #24-312-17) with Asian Community Mental Health Services,
Inc. for the period from July 1, 1996 through June 30, 1997.
Approval of Contract #24-312-18 will allow the Contractor to
continue providing services through June 30, 1998 .

This Contractor has been providing mental health program services
for the County for many years. The Contractor' s services continue
to be a vital and important part of the County's continuum of care
for West County residents with problems of emotional and mental
illness.
On December 10, 1996, the Board of Supervisors approved Novation
Contract #24-308-20 (as amended by Administrative Amendment
Agreement #24-308-21) with Early Childhood Mental Health Programs,
Inc. for the period from July 1, 1996 through June 30, 1997 .
Approval of Contract #24-308-22 will allow the Contractor to
continue providing services through June 30, 1998.

provide mental health services for the Emergency Foster Care
Program.
Approval of Contract Amendment Agreement #24-883-1 will allow the
Contractor to provide additional services through September 30,
1997 .

Year 1997-98 Budget, and the funding source is County/Realignment
100%.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
Effective July 1, 1992 , State Mental Health Realignment Legislation
shifted responsibility for payment to providers from the State to
the Counties and required Counties to assume direct responsibility
for contracting with sub-acute skilled nursing facilities known as
Institutions for the Mentally Diseased (IMD's) .
On December 17, 1996, the Board of Supervisors approved Novation
Contract #24-646-8 with Villa Maria Management Corporation (dba
Westwood) for the period from July 1, 1996 through June 30, 1997 .
Approval of Contract #24-646-9 will allow the Contractor to continue
to provide services through June 30, 1998 .

Agreement #24-660-5 with First Hospital Corporation (dba First
Hospital Vallejo) , for the period from July 1, 1997 through June 30,
1998, with a payment limit of $50, 000, for Hospital ' s provision of
fixed price beds for acute inpatient psychiatric hospital services
for mentally-ill children and adolescents.
II. FINANCIAL IMPACT:
Funding for this Contract is included in the Department's Fiscal
Year 1997-98 Budget. Utilization will vary, depending upon the
availability of hospital beds, the unique treatment needs of each
minor, and the availability of alternative sources of health care
coverage (e.g. , Medicare or private insurance) . The Contractor will
be used only as a last resort when no other financial resources are
available.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
Under the Statutes of 1987 (Assembly Bill 1362, Bronzan) , Welfare
and Institutions Code Section 5751. 7 established a prohibition
(except for special State-granted waivers) against admission of
minors into the same psychiatric treatment wards as certain adults
who might threaten the physical safety of minors.
On December 17, 1996, the Board of Supervisors approved Hospital
Agreement #24-660-4 with First Hospital Corporation (dba First
Hospital Vallejo) , for the period from July 1, 1996 through June 30,
1997 , to provide acute inpatient psychiatric hospital services for
mentally-ill children and adolescents so that minors need not be
hospitalized on the adult psychiatric units at Merrithew Memorial
Hospital.
Approval of Hospital Agreement #24-660-5 will allow the Contractor
to continue providing services through June 30, 1998 .

II. FINANCIAL IMPACT:
This contract is 100% Federally funded by the Ryan White CARE Act,
Title I, Formula. No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
Under Contract #22-623, Jennifer Marttinen will provide consultation
and technical assistance to the Department' s Public Health Division
with regards to the AIDS program through June 30, 1998.

II. FINANCIAL IMPACT:
Under this program, the County pays the State-established rates for the residential
care of mental health clients who are not yet receiving SSI/SSP (but who are
potentially eligible and have made formal application to the Social Security
Administration for SSI/SSP) , contingent upon the client's signing an agreement with
the County to reimburse the County from the SSI/SSP money, once it is finally
granted by the federal government (usually 3 to 6 months retroactively back to the
month of application) .
For those mental health clients who are not granted SSI/SSP (i.e. , are found
ineligible) , the County covers the residential care payments to the facility
operators through the County's established Mental Health Program Funding.
Initially, ten to twenty percent of the clients are found ineligible by the Federal
Social Security Administration, depending on changes in Federal guidelines, and then
some of these are ultimately found eligible through appeal processes.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
For several years, the County's Mental Health Adult Services Program has used these
Agreements as an interim financing mechanism to facilitate the residential care of
mentally disabled clients and movement of clients, who are deemed eligible for
SSI/SSP, from State or local inpatient facilities to community-based facilities.
This Program provides residential care for 40 to SO clients per year. Approval of

#23-158-7 with Battered Women's Alternatives, for the period from
July 1, 1997 through June 30, 1998, with a payment limit of
$159, 679, for provision of shelter services for battered women and
their children.
II. FINANCIAL IMPACT:
This Contract is included in the Health Services Department's budget
and is funded by County discretionary funds and a $23 . 00 fee per
marriage license issued by Contra Costa County.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On July 16, 1996, the Board of Supervisors approved Contract #23-
158-6 with Battered Women's Alternatives for the period of July 1,
1996 through June 30, 1997 to provide shelter services for battered
women and their children.
Approval of Contract #23-158-7 will allow the Contractor to continue
providing services through June 30, 1998.

II. FINANCIAL IMPACT:
This Contract was funded through the State Department of Health
Services, Office of AIDS.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On July 9, 1996, the Board of Supervisors approved Contract #22-
267-16 (as amended by Contract Amendment Agreement #22-267-17)
with AIDS Project of Contra Costa, for the period from July 1,
1996 through June 30, 1997, for the provision of AIDS education
and prevention services to men at increased risk of HIV infection.
The Contractor recently notified the Department that it wished to
terminate its contract because the inability to support the
financial cost of continuing this contract. Therefore, in
accordance with General Conditions Paragraph 5. (Termination) of
the Contract, the Department and Contractor have agreed to a
mutual cancellation of this Contract. Approval of Cancellation
Agreement #22-267-18 will accomplish this termination.

rates set forth in the agreement, for the provision of
professional health care services for the Contra Costa Health
Plan.
II. FINANCIAL IMPACT:
This Contract is funded by Contra Costa Health Plan member
premiums. Costs depend upon utilization. As appropriate,
patients and/or third party payors will be billed for services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms
of their Individual and Group Health Plan membership contracts
with the County.
The Health Plan is also required under the terms of its Local
Initiative contract with the State, to contract with community
physicians and other providers, called "Safety Net" and
"Traditional" Providers, for the provision of medical care to
Medi-Cal recipients.
This Contract is necessary to meet State mandates to expand the
number of community providers for the Local Initiative, along with
a recent Department of Corporations audit finding that requires
formal contracts with low volume providers.
Approval of this Contract will allow the Contractor to provide
professional health care services to Health Plan members through
May 31, 1998.

Contract #22-316-15 with STAT Nursing Services, in the amount of
$180, 000, for the period from July 1, 1997 through June 30, 1998,
for provision of in-home attendant care to AIDS and ARC patients.
II. FINANCIAL IMPACT:
This Contract is funded by Federal Ryan White (Care Act, Title I)
and AIDS Medi-Cal Waiver funds. No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
This Contractor has provided in-home attendant care to County's AIDS
and ARC patients since February 1989, as part of the AIDS Case
Management and Home/Community Based Care Project funded by the State
Department of Health Services.
When a person with AIDS or ARC is unable to secure attendant care
which would allow them to remain in the home rather than in an acute
care hospital, this project provides funding for in-home attendant
care services. The Contractor must attempt to collect from all
other sources of payment (e.g. , private health insurance held by the

I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee
(Frank Puglisi) to execute on behalf of the County, Unpaid Student
Training Agreement #26-209-2 with the University of the Pacific--
Department of Physical Therapy, for the period from July 1, 1997
through June 30, 2000, for provision of clinical and field experience
for physical therapy students.
II. FINANCIAL IMPACT:
None.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On February 14, 1995, the Board of Supervisors approved Unpaid Student
Training Agreement #26-209-1 with the University of the Pacific--
Department of Physical Therapy to provide the University's students
with field education in physical therapy at Merrithew Memorial
Hospital and Health Centers. Supervised clinical and field work
experience is considered to be an integral part of both educational
and professional preparation. As a teaching hospital, Merrithew can
provide the requisite field education, while at the same time, taking
advantage of the students' services to patients.
Approval of Unpaid Student Training Agreement #26-209-2 will provide
supervised clinical experience for students enrolled at the University
of the Pacific--Department of Physical Therapy through June 30, 2000.

I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) , to execute on behalf of the County, Novation
Contract 128-530-5 with the City of San Ramon, for the period from
July 1, 1997 through June 30, 1998, for the provision of congregate
and home-delivered meal services for the Senior Nutrition Program.
This document includes provision for an automatic three-month
extension through September 30, 1998 .
II. FINANCIAL IMPACT:
No County cost is included.
III. REASONS FOR RECOMMENDATION/BACKGROUND:
On March 18, 1997 , the Board of Supervisors approved Contract #28-
530-4 with the City of San Ramon for meal services for the Senior
Nutrition Program for the period from July 1, 1996 through June 30,
1997.
This program will provide an average of 30 congregate meals per day,
two days per week for senior citizens at the San Ramon Senior
Center, and 15 home delivered meals per day, to senior citizens in
the San Ramon area.
Approval of Novation Contract #28-530-5, will allow the Contractor
to continue providing services through June 30, 1998 .

I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director to execute on
behalf of the County, Contract Amendment Agreement #23-073-10 with
Accountemps, effective June 1, 1997 , to increase the Contract
payment limit by $5, 000, from $65, 000 to a new total of $70, 000.
II. FINANCIAL IMPACT:
Funding for this Contract is included in the Department's
Enterprise I budget and is funded by third party revenues.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In November, 1995, the County Administrator approved and the
Purchasing Services Manager executed Contract #23-073-6 (as
amended and extended by subsequent Amendment Agreements) with
Accountemps, through June 30, 1997, to provide qualified temporary
accountants to assist the Department's accounting staff during
peak work loads, temporary staff absences or vacancies.
Approval of Contract Amendment Agreement #23-073-10 will allow the
Contractor to continue to provide temporary help through June 30,
1997 .

AUTHORIZE Executive Director, Private Industry Council or his designee
upon receipt of the California Department of Aging Title V Standard
Agreement for funding year 1997-98, to conduct contract negotiations and
EXECUTE contracts with the following Title V Senior Employment
Program host agencies for FY 1997-98. These contracts are funded through
federal funds under the Title V of the Older Americans Act.

V for the 1996-97 program year. PIC will continue to subcontract with

(1) execute on behalf of the County standard form Vocational
Training Agreements with Vocational Training Institutions named
on the attached "Vocational Training Institutions Listing", to
provide training, at the tuition rates set forth in said
agreements, to eligible enrollees under the Job Training
Partnership Act (JTPA) Individual Referral Program, for the
period beginning July 1, 1997 and ending June 30 1998; and
(2) execute additional standard form agreements and amendment
agreements with said Vocational Training Institutions as may be
needed for periodic revisions, such as adding or deleting a
training course, training fees and/or compliance revisions to
State or Federal regulations/guidelines.
IT. FINANCIAL IMPACT:
None. 100 percent Federal funds.
ITT. REASONS FOR RECOMMENDATION/BACKGROUND:
By its Order dated April 27, 1997 the Board approved the County's
Private Industry Council Job Training Plan for 1997-98 which includes
funds for this training activity. The Individual Referral Training
Program will provide vocational training to approximately 300 JTPA-
eligible Contra Costa County residents during the period July 1, 1997
through June 30, 1998.
