FISCAL IMPACT: This program will have no County General Fund impact. One-year operating costs estimated at $104,720 and start-up costs estimated at $13,808 (total of $118,528) will be covered under CSA EM-1 (Measure H). BACKGROUND: As a result of the community's remote location, paramedic ambulance response times to Bethel Island are among the longest in the county. For 1995, the average ambulance response time in Bethel Island was 13.7 minutes compared with a countywide average of 7.0 minutes. Only Discovery Bay and Byron had longer response times, averaging 16.3 and 13.8 minutes, respectively. However, to better serve those communities, a Measure H funded first responder paramedic program has been in place since 1992. That program uses a single American Medical Response paramedic in a non-transporting unit stationed at the Byron fire station to respond to medical emergencies in the Byron/Discovery Bay area. The attached Proposal of the Bethel Island Fire Protection District will pilot an alternative model using paid-on- call firefighter/paramedics to provide first responder paramedic service. Under the proposed program, 10 part- time paramedic personnel will be hired to augment the District's existing paid-on-call staff. Preference will be given to hiring paramedics who have completed Firefighter I training. All paramedic personnel hired will complete a Fire District orientation course (16 hours for Firefighter I trained personnel or 40 hours for those without Firefighter I training). One paramedic/firefighter will then be assigned on duty during each shift and will respond to medical emergencies as well as perform other firefighter duties. Measure H funding will initially be committed for a one-year period during which time the program will be evaluated from an operational and emergency medical response perspective. Continued operation beyond the first year will be dependent upon the program's success and the availability of ongoing funding. RICHARD OORNFEED fi3tRY DAVIS CHRISTIAN SANDUS DALE WONG PROPOSAL P47t-777me Parame&&Tirefighters in the Bethel Island Fire Protection District The Bethel Island Fire Protection District provides fire and rescue emergency services to a rural six square mile area at the far northeast corner of Contra Costa County (see map, appendix A). The permanent population of 3,500, predominately of retirement age, increases seasonally due to the influx of recreational boaters and owners of summer vacation homes. Though outside actual fire district boundaries, emergency services are also provided throughout Contra Costa County's eastern portion of the San Joaquin Delta via marine fireJrescue apparatus at the request of the U.S.Coast Guard The District's single fire station responds to approximately 360 calls annually. About 70% of these are medial emergencies, 15% are fires, and 15% are hazardous conditions or public service calls. The District maintains three sworn positions, each with suppression .and administrative responsibilities. Additionally, eighteen (18) paid-on-call firefighters provide emergency response and, on a volunteer basis, share program management responsibility. The station is currently staffed from 8:00am to 5:00pm seven days per week with a minimum of two(2)paid and/or paid-on-call personnel. Upon review of the 1993 Annual Program Report published by the Contra Costa County MAS Agency, the District found itself to be one of the very few areas within the County that does not receive paramedic service within the standard response time of 10 minutes. Further research by District staff showed that virtually none of the 200+code 3 emergency medical responses met this common standard. Over the past six months, a variety of options have been explored to address the extended paramedic response times in the District. The most promising of these, from the standpoint of delivering the highest possible standard of advanced life support within the confines of current fiscal resources, is presented in this document. The concept is unique in that it combines the resources of the Bethel Island Fire Protection District with program management skills of EMS Agency staff and Measure "H" funding to produce a cost- effective solution to a service deficiency in the community of Bethel Island, that neither agency could fully resolve on its own. County Emergency Medical Services (EMS) Agency, implement a pilot program, to continue for a minimum of one (1) year, utilizing part-time paramedic/firefighters as non-transporting first responder personnel in the Bethel Island Fire Protection District (BIFPD) This program would provide one paramedic/firefighter as part of the engine crew responding to all requests for assistance in the Bethel Island Fire Protection District. Data will be collected during the first year of operation in order to evaluate the program's cost-effectiveness, its impact: upon the identified shortcomings of current service levels, and how closely the objectives and intent of Measure `H' are fulfilled. The paramedic/firefighter will provide Advanced Life Support Fust Responder medical assistance as needed, as well as respond to fire emergencies, hazardous materials and rescue incidents, and other public service requests. Other job responsibilities va include fire prevention and public education activities. PROGRAM SPECMCA.'I'IONS Personnel The paramediclfirefighters would be incorporated into an existing paid-on-call program in the Bethel Island Fire Protection District, and required to work a minimum number of shifts per month to remain in good standing. Due to the relatively low number of medical responses in the district, and the fact that the paramedic will be worldrig without a partner, the pilot program will seek paramedics with a minimum of one (1) year full time field experience. To assist with skM retention, paramedics, would be required to maintain active,fiilltime employment with an approved AIS provider agency. Other requirements would consist of current paramedic licensure in the State,of California, current Advanced Cardiac Life Support (ACLS) certification and Basic Trauma Life Support (BTLS) or PreHospital Trauma We Support (PHTLS) training prior to working in the District. Accreditation in Contra Costa County is not a requirement to enter the selection process, but must be completed prior to beginning shift work- Ten orkTen (10) paramedics will be selected from an eligibility list created by the Selection Process (see appendix B). The most desirable candidates for the pilot program are paramedics currently accredited in Contra Costa County, with more than 1 year full-time field experience and current employment with an approved ALS provider agency. Although Firefighter I training would not be required to participate in the pilot program, paramedic personnel meeting the above requirements, who also have Firefighter I training would be the preferred candidates. Persons hired with the desired qualification of Firefighter I training will complete a 16- hour Fire District Orientation course. Topics will include familiarization to District Rules and Regulations, Policy and Procedures Manual, Infection Control Program, and Contra Costa County Personnel Management Regulations; use of safety equipment such as firefighter's turnout clothing and self-contained breathing apparatus; firefighting tools, Bethel Island Rre part-time Param ediclFrefigh ter Proposal 2 equipment and communications systems; fire apparatus including the District's two fire/rescue boats; engine company performance standards; and familiadzation.with District streets,water supply sources and levee system. Upon successful completion of an eighteen (18) month probationary period, the District will, at the employee's expense, apply to the Office of the State Fire Marshall for the employee's Firefighter I Certification Persons hired without the desired qualification of Firefighter I training will complete a 40- hour Fire District orientation course. The course will consist of an expanded version of the topics mentioned above. Employees without Firefighter I ttaining will be expected to complete the next available Firefighter I Academy on their own time, with the direct costs of the Academy being borne by the Fire District.. When the employee is hired, s/he will enter into an agreement with the District to remain as an employee for a 'minimum of eighteen(18) months from completion of the Academy training. If the employee resigns prior to completing either the Academy or the eighteen(18)month post-Academy period, s/he will reimburse, on a prorated schedule,the direct costs of the Fire Academy and pre- Academy training incurred by the District(see appendix Q. Upon successful completion of an eighteen(18) month probationary period(which begins at the end of the Academy training), the District will, at the employee's expense, apply to the Office of the State Fire Marshall for the employee's Firefighter I Certification. As firefighterswith the Bethel Island Fire Protection District, employees will be bound by current Rules and Regulations of the District and shall be subject to Contra Costa County Personnel Management Regulations, as they apply to part-fm employees. Immediate supervision shall be provided by Officers of the District for all matters other than those concerning medical control. Communications/Documentation As First Responder ALS personnel,the paramediclfirefighters will be expected to perform appropriate BIS and ALS skills while awaiting the arrival of the transport unit. A MEDARS portable radio supplied by the Contra Costa EMS Agency, and a portable cellular phone supplied by Bethel Island Fire Protection District will provide field-to- hospital communications with Mt.Diablo Medical Center, which will be the assigned base hospital for the unit. The paramedictfirefighters will be expected to follow all current patient treatment guidelines and policies, as outlined the Contra Costa County Prehospital Care Manual and County Policies. For purposes of continuity of patient care, the first paramedic on the scene will be in charge of patient care, until s/he turns the patient over to a transporting unit or a receiving facility. If time permits, the Bethel Island Fire Protection District paramedic/firefighter will complete a Contra Costa County Prehospital Care Report (PCR) prior to transport and give a copy to the transporting unit. If it is not possible to complete the PCR prior to transport, the PCR will be completed and faxed to the receiving hospital when the first Bethel Island Fire part-time ParamediclFirefighter Proposal 3 r responder returns to the Bethel Island Fire Protection District station. A verbal report shall by given to the transporting unit in all situations. ` If a BLS unit arrives to provide transport, and the patient requires ALS care during transport, or if ALS care has been provided prior to transport, the paramedic/firefighter will accompany the patient to the receiving hospital. ' Housing/Uniforms The District's fire station is equipped with three fold-up beds, lockers, a Bull kitchen and restroom with a shower. Structural turnout clothing will be supplied by the Fire District and is to be maintained by the employee. The Diistriict has ten (10) complete sets of turnouts in various sizes. The employee may purchase a station uniform per District guidelines at his/her own expense, or may order a Top Notch fire-resistive cotton jumpsuit, which will be supplied by the District. ,v- Scheduling As a condition of employment, each paramediclfirefighter will be required to work no less than three (3) 24-hour"shifts per month. Shift charkge will occur at 0700 hours, to accommodate employees leaving to work shifts with thew full-time employers. Each month, on a date and time to be determined, employes will sign up for shifts in the following month, until all days in that month are staffed (this process may he modified depending upon the needs of the program). Unless approved in advance by the District, an employees inability to comply with this obligation may be considered as a voluntary resignation. In the event shifts are available at the end of the sign-up period, employees may fill more than their three shift minimum Supplies/Equipment Paramedic supplies and equipment will be purchased and maintained through an agreement with American Medical Response West. All required ALS equipment will be carried on the engine, with back-up stocks of medications and other supplies maintained in a locked cabinet at the station(see appendix D for listing of supplies/equipment). Narcotics will be stored in the cab of the fire apparatus, using a double lock storage system The storage container that houses the narcotics will be secured inside a larger cabinet, and will utilize the Fire District's KNOX--BOX security system. The KNOX-BOX key, as well as the key to the larger cabinet will be passed from paramedic to paramedic at shift change. In the event that the off-going paramedic must leave prior to the arrival of the on-coming paramedic, the off-going paramedic will secure the KNOX-BOX key in the Sentralock system on the fire apparatus. The on-coining paramedic will contact the Contra Costa County Fire Dispatch Center and request: that they release the Sentralock system, allowing access to the key(see appendix E for additional information). Bethel Island Fire part-time ParamedidFirefighter Proposal 4 A process is being developed to allow the first responder paramedic to "swap" narcotics with the transporting ALS unit. The transport unit would then restock at a facility approved by the Contra Costa County EMS Agency. Costs Costs for the pilot program have been divided into two parts. The start-up costs include initial training/orientation of personnel, uniforms and purchase of equipment and supplies needed to put the unit into service. These costs are estimated at $13,808.00. On-going costs throughout the one year pilot period include personnel costs, equipment maintenance (Lifepak 10), and medical supply restock costs, and are estimated to be $104,720.00. Total cost for the one year pilot is$118,528.00(see appendix F for cost break-downs). Quality Improvement For the pilot program, Quality Improvement (Ql) and medical oversight activities will be coordinated by the Contra Costa County EMS Agency. Operational QI oversight will be provided by Bethel Island Fire Protection District. The program would be funded through County Service Area EM-1 and evaluated at the end of one year to determine its cost effectiveness, its impact upon the identified shortcomings of current service levels, and how closely the objectives and intent of Measure H are fulfilled. The specific goal of the program would be to "enhance the provision of advanced life support services to the community." Listed below are seven measurable objectives that will be tracked throughout the one year pilot period. 1. Reduce the current average response time of fourteen (14) minutes for the arrival of advanced life support personnel by at least 50%. 2. Reduce on-scene time of transporting ALS units, thereby reducing overall time taken to deliver patients to hospital emergency rooms. 3. Hire and schedule certified and experienced part-time paramedics to ensure service availability 24 hours per day and 7 days per week. 4. Utilize paramedic employees to train existing EMT firefighters to assist paramedics when performing advanced life support skills. 5. Dispatch Bethel Island paramedic engine on an auto-aid basis to the immediately surrounding area where a delayed response or need for multiple ALS units may be indicated. 6. Provide service with as little impact as possible on Measure H funds by using part-time employees at no more than fire district paid-on-call firefighters' salaries. 7. Deliver paramedic first responders via Bethel Island fire/rescue boat directly to scene of waterborne accidents or medical emergencies. (Currently victims of boating accidents or illness are transported to land based ALS units by private or BLS fire district boats.
