MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: February 6, 1996 SUBJECT: Approval of Contract Change Order No. 4 for the San Pablo Dam Road Slide Repair at Station 252+00, Project No. 0672-6R6523-93, EI Sobrante area. Specific Request(s)orRecommendation(s)&Background&Justification I. Recommended Action: APPROVE Contract Change Order No. 4 in the approximate amount of $8,000.00 and AUTHORIZE the Public Works Director to execute it in order to compensate the contractor, Kulchin-Condon and Associates, for changes in contract bid quantities. If. Financial lmpact: The project contingency funds are sufficient to cover the change order costs. The source of funds is Federal and Storm Damage Repair Funds (65.8%) and County Road Funds (34.2%). III. Reasons for Recommendations and Background: The cumulative total of Contract Change Orders Nos. 1 through 4 exceeds $25,000.00, requiring Board approval. Contract''Change Order No. 4 balanced estimated contract quantities. IV. Consequences of Negative Action: The lack of approval would preclude payment for work that was performed in good faith.
