The status of the outreach program to the Multi-Cultural Development Association, Inc. and minority groups designed to generate additional minority foster homes and potential adoptive homes. C. The status of the administrative review of children in long-term foster care. D. The status of the implementation of each of the goals outlined in the paper entitled "Child Welfare Services Goals Related to Permanency Planning for Children consistent with the timeline proposed by Breck and Associates. HS03 BACKGROUND: On April 25, 1995, the Board of Supervisors approved a report from our Committee which included the following recommendations: REQUEST the Acting Social Services Director to report to the Family and Human Services Committee on July 10, 1995 on the following: 1. A cost estimate for a computer system that would allow the Department to assemble and maintain essential management statistics in terms of the status of each child in long-term foster care, the plans for each child and the status of the implementation of those plans for each child. 2. Copies of all relevant statistical reports so the Department, the Family and Human Services Committee, the Grand Jury and other interested parties can account for the number of children in various categories and insure that all interested individuals are working from the same statistical source documents. 3. The status of an outreach program with the Multi-Cultural Development Association, Inc. and minority groups designed to generate additional minority foster homes and potential adoptive homes. 4. A more detailed explanation of how Supplemental Security Income (SSI) revenue is handled in terms of allocating retroactive lump sum benefits, how funds are allocated to a trust fund for a child, how foster and adoptive parents are advised of the balances in a child's trust fund, and how plans are made for the expenditure of this revenue in order to insure that a child's trust fund does not exceed $2000. 5. The status of the administrative review of children in long-term foster care. 6. The report from Breck and Associates on the reality of the timelines proposed by the Department for implementing each of the recommendations contained in the report from the Child Welfare Research Center. 7. The status of each of the goals outlined in the paper entitled "Child Welfare Services Goals Related to Permanency Planning for Children". On July 10, 1995, our Committee received the attached report from the Acting Social Service Director. The Assistant County Welfare Director for Services, Danna Fabella, reviewed the report with us in some detail. The report responds to each of the above directions in detail. In addition, Mike Chernock from Breck and Associates presented our Committee with the detailed timeframe and each of the elements which must be undertaken in order to implement each of the recommendations made by the Child Welfare Research Center. He emphasized that the Department must have time in which to do its work and that any efforts from outside to micro-manage the Department do not further the Board of Supervisors' goals of improving the operation and management of the County's Adoption Program, Mr. Chernock noted that complete implementation will take until November of 1997, although probably 85% of the work will be completed early in 1996. The additional delay is the result of implementing the Fost/Adopt Program which will require a full year for implementation. He noted that this work will require the equivalent of four additional person-years of work from the Department's staff. Brian Simmons from the Child Welfare Research Center noted that they are beginning their administrative review of foster care cases but that it is too early to draw any conclusions at this time. Testimony from the foster and adoptive parents present indicated that there is still some confusion and suspicion regarding how the County handles SSI revenue which is received on behalf of foster children. Ms. Fabella noted that staff from the U.S. Social Security Administration were present and would be undertaking an audit of the manner in which the County handles SSI revenue. As a result, we are asking that the Department respond to the following points when it next reports to our Committee: 2 HS03 A. Where in the County Budget (in which revenue line item) is revenue from the Federal Government in the form of SSI payments on behalf of foster children reflected? Can this be documented in some manner through, for instance, copies of the documents the County uses when this revenue is received to credit it to a particular revenue account? It would be helpful to describe in writing how this revenue is physically received by the County and what identifying information the County receives to match the revenue to a particular child. B. Does the Department or the Auditor-Controller have available a printout of some type which shows the receipt of SSI revenue on behalf of foster children, a copy of which (or a copy of a portion of which) could be made available to interested individuals without violating confidentiality to demonstrate how the revenue is received and credited to the County? C. Can the Department or the Auditor-Controller describe in writing or demonstrate what is meant by the "trust fund" which is maintained for each foster child who is receiving SSI revenue and which shows credits to the "trust fund", withdrawals from the "trust fund" and the balance in the "trust fund" without violating confidentiality? D. Is it possible to provide the Family and Human Services Committee with a copy of a "trust fund" ledger showing the activity on a child's account while protecting the identity of the child in order to demonstrate what the County means when it says that it has a "trust fund" on each of these children? E. A copy of the report from the Social Security Administration on the results of its audit of the County's maintenance of SSI accounts should be forwarded to the Family and Human Services Committee upon receipt by the Department. As appropriate, the Department should comment on any findings, conclusions or recommendations which reflect the need for improvement in the County's procedures, policies or practices. Jim Fallis of the 1994-95 Grand Jury raised the following questions which we have agreed should be responded to by the Social Service Department in their next report to our Committee on October 9, 1995: 1. In the next to the last line of the first page of the Department's report to our Committee dated July 5, 1995, reference is made to "pilot counties" for the implementation of the CWS/CMS system. Which counties are pilot counties? 2. The Family and Children Services Advisory Committee submitted a report to Supervisor Powers in April, 1990 on the "Retention of Foster Parents in Contra Costa County'. In the section titled "Suggested Improvements" there are a number of recommendations for action by the County. What actions have been taken to respond to or implement those recommendations? 3. On page C-19 of Attachment C to the Department's report to the Family and Human Services Committee, it appears that 91% of the children for whom the case plan goal is long-term foster care with non-relatives have been in placement for in excess of 24 months. Why is this the case and what steps are being taken to reduce this time period? 4. On page C-22 of Attachment C to the Department's report to the Family and Human Services Committee, it appears that a significant number of the children have been in five or more placements. Given that the Department and our Committee agree that this is a troubling statistic, what steps are being taken or can be taken by the Department to reduce the need to replace children so many times? 5. On page 5 of the report from the Department, in the second paragraph reference is made to "data elements". What are those data elements? 3 HS03 Our Committee believes that the series of hearings we have conducted on the subject of the County's Adoptions Program has led to a change in attitude on the part of staff. We are not interested in micro-managing the Social Service Department. We are, however, interested in insuring that the Department is being responsive to legitimate requests for information from the concerned public and that where problems are identified that changes are planned and undertaken. Our Committee intends to continue to pursue this subject and will make further reports to the Board of Supervisors following the October 9, 1995 report to our Committee from the Department. 4 SOCIAL SERVICE DEPARTMENT Contra Costa County TO Family and Human Services Committee, DATE July 5, 1995 Board of Supervisors FROM Robert Hofmann, CC Phil Batchelor, Acting County Welfare Director County Administrator Clyde Parkhurst, Foreman, Grand Jury SUBJ FOLLOW UP TO THE APRIL 11, 1995, REPORT TO THE FAMILY AND HUMAN SERVICES COMMITTEE ON THE ADOPTIONS PROGRAM This report is submitted to the Family and Human Services Committee as directed by the Board of Supervisors on April 11, 1995. The Committee requested information in the following seven areas. 1. A cost estimate for a computer system that would allow the Department to assemble and maintain essential management information in terms of each child in long-term foster care. Attached please find a report by Jim Takahashi for the Social Service Department (attachment A). In summary it would cost approximately $395,000 to redo the State's FCIS (Foster Care Information System) into a relational data base PC system. Another alternative is to redo the SSRS (Social Services Reporting System) into a PC platform, and a relational data base would be approximately $260,000 for programming and $350,000 for equipment. Other "hidden' costs are the training of staff.and the normal maintenance for the system. The applications and hardware would require 100% County general funds since state and federal funds have been committed to the statewide CWS/CMS (Child Welfare Services/Case Management System) which has been in development for several years. We reported at the last Family and Human Services Committee meeting that the CWS/CMS was stalled due to litigation between IBM/ISSC and the California Department of Social Services (CDSS). We were recently informed that, through Governor Wilson's intervention with IBM, CWS/CMS is back on track with IBM agreeing to absorb the disputed budget overrun. The State's projected time frame is to begin roll out in the pilot counties in August, 1995, with roll out in all other counties to begin in January, 1996. Family and Human Services Committee, Board of Supervisors July 5, 1995 Page 2 Steven Steinbrecher, Director, Data Processing Services, reviewed our Department's start-up estimate and found it "on target." However he cautioned that we might be underestimating the maintenance costs which he projects at approximately $12,000 per desktop unit. Because all of these costs would be general fund expenditures, he does not believe an interim system is a lucrative proposition for the County. Therefore, our Department strongly recommends that we continue to wait for the State's CWS/CMS computer database. We should note, however, that our Department is continuing to meet with our Systems staff to develop a database for our administrative review which will track minors who have received adoptive reviews and track their outcome. We are also working on a database for our adoption program which will track information after a minor is referred and is case managed by adoptions. 2. Copies of relevant statistical reports. As you know our Department receives FCIS reports from the California Department of Social Services (CDSS). These reports are aggregate numbers and are not very useful for case management purposes. We also ask for ad hoc reports when we want inter-relational information and that, again, is in aggregate numbers. I have attached a list and copies of the regular FCIS reports we receive as well as the most recent ad hoc reports (attachments B through G). Additionally, there is a table describing the FCIS reports (attachment H). 3. The status of.an outreach program with the ministerial alliance and minority groups designed to generate additional minority foster homes and potential adoptive homes. The Department has been working with Reverend Curtis Timmons in an effort to recruit additional minority foster homes and adoptive homes. Our recruiter appeared on Reverend Timmons' radio talk show with the purpose of discussing our County's needs and to answer questions relating to foster care and adoption. Our understandingis that this was well received. We have also contracted with Ministerial Alliance to do five 60-second recruitment spots. Reverend Timmons has also.written an article stressing the need for African- American foster and adoptive homes which has appeared in a local newspaper. Our recruiter will continue to work with Reverend Timmons to reach this population. Family and Human Services Committee, Board of Supervisors July 5, 1995 Page 3 We are also actively recruiting foster and adoptive homes through involvement in Community Task Force on Homes for Children which supports the Brian's Kids' spot on KPIX. We recently obtained court orders from Judge Haight to allow filing on several minority children for recruitment purposes. They will be featured on Brian's Kids in the near future. 4. a) A more detailed explanation of how Supplemental Security Income (SSI) revenue is handled in terms of allocating retroactive lump sum benefits; b) how funds are allocated to a trust fund for a child; c) how foster and adoptive parents are advised of the balances in a child's trust fund; and, d) how plans are made for the expenditure of this revenue in order to insure that a child's trust fund does not exceed $2000. a) When a child comes into foster care who appears to be eligible for SSI, the Social Service Department makes an application on his/her behalf. If the application is approved, the Department receives an award from Social Security that goes back to the date of the SSI application. We take that award and repay "welfare" for the cost of the child's care. For example: • On 2/26/94 Susie was removed from her mother's home and placed in a foster home that received $500/month in AFDC-Foster Care payments. • On 5/24/94 the Juvenile Court made a dispositional order. The care and maintenance section of the court order included: "County to pay costs of placement, including clothing, transportation and medical costs. Social Service authorized to receive monies coming to minor. Excess monies over maintenance shall be placed trust for minor." • On 6/1/94 the Social Service Department applied for SSI for Susie. • On 12/15/94 Social Security advised the Department that it had awarded benefits. They advised us that we would be receiving a check for $5236 to cover the time from 6/1/94 through 12/31/94 and that the continuing benefit effective 1/1/95 would be $760/month. • The Department repays welfare (AFDC-FC) for the cost of care expended from the date of the SSI application. The Department paid out $3500 for the period from 6/1/94 through 12/31/94. This $3500 would be subtracted from the award and the remainder ($1736) would be put into a trust account for the child. (NOTE: The Department had been paying $500/month for Susie's care since 2/26/94.) Family and Human Services Committee, Board of Supervisors July 5, 1995 Page 4 Attached are two tables showing how revenue is handled. One shows what happens when the cost of care is less than the revenue (attachment I) and the other shows what happens when the cost of care is more than the revenue (attachment J). b) If the child's cost of care is less than the child's income from SSI, the difference is put into the child's trust account. If the child's cost of care is more than the child's income, there is no money to put into the child's trust account. See the attached tables (attachments I and J). c) Currently it is up to the social worker to inform the caregiver. We have recently hired a Children's Services Specialist Clerk to apply for these funds and to monitor the accounts. Previously we worked with a consultant group to apply on behalf of the minor. We have developed two form letters that will go out to the caregiver when we apply for SSI and when we receive notification that the SSI benefits have been granted. We are revising our policy to make it clear to the social worker to include in the child's.case plan consideration of current or potential trust monies and identified special needs for these monies. Additionally, that the foster caregiver is to be involved in planning what the child's special needs are and planning for utilizing the child's money to his/her best benefit. d) The trust accounts are kept by clerks in Fiscal operations who prepare a monthly Trust Account Report which is sent to the Child Welfare Unit Supervisor. We have started sending a copy to the Child Welfare Division Managers so that they can monitor their staffs compliance. We are also informing social workers that they must monitor the child's trust account balance to determine if there are sufficient funds in the account to meet the child's special needs and to ensure that the balance does not go over the $2000 property limit for AFDC-FC, SSI and Medi-Cal. Trust account funds should not accumulate unnecessarily to the extent that the child becomes ineligible for either AFDC-FC or SSI/SSP. Our Department manual on this section is being revised to reflect the above changes. 5. The status of the administrative review of children in long-term foster care. As you know we are undertaking a review of children ages 12 and under who have been in foster care two years or more and have a case goal of long-term foster care. These cases (approximately 900) include children who are placed with their relatives. Family and Human Services Committee, Board of Supervisors July 5, 1995 Page 5 We are reviewing these cases on a flow basis and have divided cases equally among the Managers on the Children's Services Administrative.Team (CSAT). We have just completed cases that had a court hearing in April and are arranging for the Child Welfare Research Center (CWRC) to complete a random sampling of our review. It is too.soon to report specific information. We have identified data elements to assist us in tracking the information and quantify results. Our programmer has been working on this collaboratively with program staff and clerical staff. 6. The report from Breck and Associates on the reality of the timelines proposed by the Department for implementing each of the recommendations contained in the report from the Child Welfare Research Center. We have been working with Mike Chernock with Breck and Associates on developing realistic time frames for implementing CWRC's report. He has met with our Division Managers to gain an understanding of the task, has spent an entire day with managers and supervisors breaking the recommendations into specific tasks and assigning time to the tasks and has worked with our Systems staff and Program Analyst both installing and training staff on the project- scheduling software. Mr. Chernock is available to provide further information and answer any questions. 7. The status of each of the goals outlined in the paper entitled "Child Welfare Services Goals Related to Permanency Planning for Children." a) Within the next six months, complete an administrative review of all children who have been in long-term-foster or relative care over 24 months and who are under the age of 12. • CSAT finished their first month review with the CWRC. CWRC will begin their sample review this month. b) Develop a formal fost/adopt program in Contra Costa County. • Sent for and reviewed the Washington material recommended by the Youth Law Center. • Contacted the author of the material and are arranging a presentation to staff in October. • Adoption manager and supervisors visited San Mateo County to learn their process. Family and Human Services Committee, Board of Supervisors July 5, 1995 Page 6 • Attended presentation on fost/adopt by CWRC at Contra Costa County's Juvenile Bar meeting to discuss fost/adopt in general and discuss our Department's plans to develop a program. c) Create a standard decision-making-criteria regarding family reunification and the placement of children in long-term foster care or adoption. And, d) Clarify the roles and responsibilities as related to adoptions and permanent planning between family reunification workers and adoptions staff; and implement policies and procedures that ensure timely, effective decision making. • Both these goals are related to fost/adopt and are part of the development of that program which is in the initial phase. e) Create an adoption worker's desktop manual. • We have identified this as an end product of the redesign of the adoption program and will be completed after the program redesign has been completed. f) Develop standardized home study guidelines. • We have obtained copies of CDSS's guidelines as well as San Mateo County's. We have also requested copies from other counties and will use those to adapt our process accordingly. .g) Promote mutual understanding with the Juvenile Court around roles and responsibilities in planning for children. • We continue to meet monthly with the Court, Public Defenders and Alternate Defenders offices to discuss issues that arise between the Department the legal community. We also keep them informed of programmatic changes in an attempt to create a collaborative environment. • We have been in contact with the Administration on Children and Family, Region IX, and have received approval for technical assistance from several resource centers on legal issues and permanency planning. We have preliminarily identified the adversarial climate of the Juvenile Family and Human Services Committee, Board of Supervisors July 5, 1995 Page 7 Bar as an hindrance to fost/adopt planning and have asked for assistance in this area. h) Develop a common conceptualization of the foster parent role within the Contra Costa County Social Service Department system. . The Department has completed its first session of Foster Adopt/Foster Pride training. We believe this training is a model for developing mutual understanding of the roles between foster/adoptive parents and social workers. We are contracting with the community colleges to continue this training and will be assessing how we can include this into staff training curriculum. Our recruiter has made herself available to foster/adoptive parents. She will continue in this role and will continue to collaborate with the community colleges on the Foster Adopt/Foster Pride training. Our adoption supervisor attended the state-wide Annual Foster Parent Retreat in order to gain an understanding of the issues that foster parents have. i) Set up a continuing education curriculum and training for all Department child welfare workers. • We are pleased that our Training Supervisor began her assignment on July 5th. We received permission in April to hire and completed the interviews in May. However, after we selected the Supervisor, it was necessary to hire behind her because we could not leave a vacancy in the position she was leaving--Supervisor of the Intake/Court Unit in Central County. • We have identified training classes which U.C. Davis will be providing through our contract with them. These classes include Negotiating Case Plans, Interviewing Young Children, Expert Witness Testimony, and others. We will be working with our Training Supervisor to develop training on Department policies and other identified training needs. CONTRA COSTA COUNTY June 19, 1995 Tel: (510)313-1654 FAX 3-1575 TO: DannFabella CC: Bob Hofmann Don Cruze FROM: Jim Takahashi RE: Automation of a Foster Care Services System �`c 7— �!Lv BACKGROUND: This is in response to the Grand Jury request to install an automated information children services tracking system in the Social Service Departhnent and what would be needed in the way of costs. The primary concern today is complexity and cost in comparison to, or in competition to the State's CWS/CMS system now in development. There is a wide range of automated information systems and automated processing systems that could be brought into the county. We could contract to develop a fully automated clone of CWS/CMS which has had a roller coaster ride over the last three years and another 2 years to go before completion. The system has been promised to be brought into California at a cost of $95 million. At the other end of the spectrum in cost and information is the -low cost State's Foster Care hnformation System, FCIS. This system exists today but is document fed by workers and posted into the state's data base for reports. Contra Costa can ask for reports and obtain information sorted and extracted from the data base from the state at no costs and a wait of several weeks. Looking for a middle ground is still expensive. We can offer to duplicate the State's FCIS on a PC based LANIWAN or change the CDS SSRS system by moving from the mainframe and to a relational data base PC system. I have not gone into great study as to what the FCIS system would have in the number of elements for programming the system other than the single input document now being used for the FCIS initiation and change. I also have not looked into with other ramifications or saving brought about by these systems if they were to produce high worker impact and efficiencies (cost savings) in the placement or reunification of children. What I did do was to take an extrapolation of the costs from the county SIT project information and referral tracking system under review for development. In my initial talks with the SIT and Data Processing staff the cost to develop a data base program for SIT was around $35,000 which I thought might be low. Assured by DP that development of PC data bases was much faster than those for main frame and therefore probably only 1/5th or 1/4 of what it would cost on the mainframe. The number of SIT screens are currently limited to a very few screens. The other costs for SIT being extrapolated was equipment which would be the bulk of our initial system acquisition costs. FCIS A LAN/WAN SOLUTION: For a FCIS PC based system the assumptions are that each ER, FM, FR, PP, adoptions, CRPU, licensing, IM foster care unit supervisor would have a PC; and at least two free standing PCs are available for SWs in those units; and that one machine for each unit clerk would be available for each unit. The total machines necessary would be less those that we now have for clerks, except for the 2 IM foster care clerks. PC work stations required 71. Plus 5 stand alone CWS PC servers, one for each office. These costs do not include duplicate labor used to make data entry, or labor used to run machines and provide hardware maintenance. This does not include wiring for Antioch which still has coax and requires twisted pair. SSRS AUTOMATED TO PC/RELATIONAL DATA BASE Another alternative is to choose to redo SSRS and take up the EDS offer to move SSRS onto to a PC platform and a relational data base. The programming costs are $260,000. This is the maximum programming charge only. If one other county were to chose to do the same the costs would be 1/2 should a single county decide that this is a viable option. If all WCDS counties joined in, our costs would be 1/19 or $20,549. This would not include maintenance required by changes in legislation or for department needs. Equipment would be at least $350,00,_plus costs for additional workstations to bring all SW up to a 1-1 ration would have to be figured in at $3,417/ worker. Clearly this is not a viable option unless other federal and state funds are available which would not be available now that CWS/CMS is back on track. HIDDEN COSTS: On any of the above options the costs will not be looked upon as normal maintenance and will be seen as a substitute for CWS/CMS with no-state or federal funds allowed to offset costs. I also want to say that the costs most likely would be low since we have not done an in depth review. These costs do not include duplicate labor used to make data entry, or labor used to run machines and provide hardware maintenance. This does not include wiring for Antioch which still has coax and requires twisted pair. SSRS AUTOMATED TO PC/RELATIONAL DATA BASE Another alternative is to choose to redo SSRS and take up the EDS offer to move SSRS onto to a PC platform and a relational data base. The programming costs are $260,000. This is the maximum programming charge only. If one other county were to chose to do the same the costs would be 1/2 should a single county decide that this is a viable option. If all WCDS counties joined in, our costs would be 1/19 or $20,549. This would not include maintenance required by changes in legislation or for department needs. Equipment would be at least $350,00, plus costs for additional workstations to bring all SW up to a 1-1 ration would have to be figured in at $3,417/ worker. Clearly this is not a viable option unless other federal and state funds are available which would not be available now that CWS/CMS is back on track. HIDDEN COSTS: Y On any of the above options the costs will not be looked upon as normal maintenance and will be seen as a substitute for CWS/CMS with no state or federal funds allowed to offset costs. I also want to say that the costs most likely would be low since we have not done an in depth review.
