RECOMMENDED ACTION: Approve and authorize the Chair, Board of Supervisors, to execute, on behalf of the County, Standard, Agreement (Amendment) #28-523-4 (State #94-19872-A1) , effective February 1, 1995, with the State Department of Health Services to increase the contract payment limit. by $64, 300, from $192,529 to a new total of $256, 829 during FY .1994/95, for the County's HIV CARE Consortium Project. II. FINANCIAL IMPACT: Approval of this Standard Agreement (Amendment) will increase the maximum reimbursable amount of State funding (Ryan White CARE Act, Title 2) for FY 1994/95 by $64, 300, for a new total of $256, 829. No County match is required. III. REASONS FOR RECOMMENDATIONIBACKGROUND: On September 27, 1994, the Board of Supervisors approved Standard Agreement #28-523-3 with the State Department of Health Services, for the County's local HIV CARE Consortium, to improve the quality, availability and organization of health care and support services for individuals with HIV Disease and their families. Standard Agreement (Amendment) #28-523-4 will provide an increase' in the maximum reimbursable amount from the State for activities conducted by the HIV CARE Consortium during FY 1994/95. The Board Chair should sign eight copies of the agreement. Seven signed copies of the agreement and three sealed/certified copies of this Board Order should be returned to the Contracts and Grants Unit for submission to the State Department of Health Services.
