SUBJECT: Proposal in Support of Contra Costa County's Service Integration Teams
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S):
SUPPORT the Greater Bay Area Family Resource Network's (GBAFRN) submission of a proposal
to the S.H. Cowell Foundation to provide technical assistance and training to Contra Costa County's
Service Integration Teams (SITs).
AUTHORIZE the Chair of the Board of Supervisors to execute a letter of support for the proposal to
the S.H. Cowell Foundation.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S):
This proposal marks the beginning of a partnership between Contra Costa County and the GBAFRN,
a family resource and training organization with great strengths in these areas. The proposal
requests $450,000 over three years. Training topics will cover the broad array of topics confronted
by the teams, including: high performance team functioning, defining roles and responsibilities,
cultural sensitivity and common case management. This training curriculum will strengthen and
support SIT functioning and will be replicable to future SIT sites in Contra Costa as well as family
service centers outside of Contra Costa.

Dear President Erickson and Members of the Board:
During these times of limited resources, innovative, collaborative models for delivering
human services to under-served and vulnerable populations play a vital role in meeting the
needs of families and children. During the past two years, Contra Costa County's Office
of the County Administrator, the County Departments of Health Services, Probation and
Social Service, the County Superintendent of Schools, the Mt. Diablo Unified School
District, County Employee Organizations and numerous community-based organizations
have joined together to develop and implement an integrated human service delivery
paradigm.
Ten months after implementation, Contra Costa County's Service Integration Teams (SITs)
have achieved a number of successes, including increased rates of employment among
SIT clients, a 100 percent immunization rate at North Richmond's Verde Elementary
School, the establishment of new relationships between clients and workers and high
levels of satisfaction for both clients and workers. However, this new service delivery
approach also has posed great challenges to the teams. Ensuring the success of this
model requires intensive training of workers who are undergoing the shift from the
traditional fragmented model of service delivery to this ground-breaking integrated
approach. Contra Costa County's Board of Supervisors believes that the SITS have the
potential to improve both the efficiency and effectiveness of human service delivery
systems, but recognize the need for further training of SIT workers.

The Greater Bay Area Family Resource Network's (GBAFRN) proposal to the S.H. Cowell
Foundation to provide technical assistance and training to Contra Costa County's SITs
would provide much needed support. Contra Costa County welcomes the opportunity to
develop a partnership with the GBAFRN in this endeavor.
