SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION RECOMMENDATION(S): APPROVE and AUTHORIZE the Chair, Board of Supervisors, to execute a letter to apply for a Models That-Work Competition award for Service Integration (sponsored by the Bureau of Primary Health Care, U.S. Department of Health and Human Services). BACKGROUND/REASON(S)FOR RECOMMENDATION(S): This competition is part of the Bureau's Models That Work Campaign, aimed at identifying and sharing information about innovative and successful models for delivering primary health care to under served and vulnerable populations. Application to the Models That Work Competition will show support for Contra Costa's Service Integration Teams and provide opportunities for recognition at the national level. The winning projects will have the opportunity to showcase their work at the Models That Work Symposium in Washington, D.C., in November 1995. The five winning projects will be awarded $12,000 - $20,000 in addition to an expense paid trip to Washington, D.C. Contra Costa County's Office of the County Administrator, the County Departments of Health Services, Probation and Social Service, the County Superintendent of Schools and the Mt. Diablo School District have joined together over the past two years to create an integrated service delivery system that draws upon the resources not only of public agencies, but of communities as well. Innovative, collaborative models for delivering human services to under served, and vulnerable populations play a vital role in meeting the needs of families and children during these times of limited resources. Contra Costa County's Family Service Centers are an extremely creative and successful models for improving the efficiency and effectiveness of human service delivery to at-risk communities. Contra Costa Cdunty would appreciate the opportunity to share its successes with other localities I and practitioners at the Models that Work Symposium on November 20, 1995.
