The Board of Supervisors, RESOLVES THAT:
The County of Contra Costa on June 28, 1994, contracted with Younger-
Wunar, Inc. , for Renovation and Addition to 30 Muir Road, Martinez,
Budget Line Item No. 4423-4221, Authorization No. 0928-WH221B, with
Amwest Surety Insurance Company as surety, for work to be performed
on the grounds of the County; and
The Director of General Services reports that said work has been
inspected and complies with the approved plans and specifications,
and recommends its acceptance as complete as of May 16, 1995; and
recommends the withholding of $4,800 . 00 pursuant to a letter dated
May 3, 1995;
Therefore, said work is accepted as recommended above, and the Clerk
shall file with the County Recorder a copy of this Resolution and
Notice as a Notice of Completion for said contract; and the Director
of General Services is directed to withhold monies otherwise due
pursuant to the letter dated May 3, 1995 .
Time extension to the date of acceptance is granted as the work was
delayed due to unforeseeable causes beyond the control and without
the fault or negligence of the Contractor.

SUBJECT: RENOVATION AND ADDITION TO 30 MUIR ROAD, MARTINEZ, FOR SOCIAL SERVICE
DEPARTMENT (WH221B); Withhold Agreement and Acceptance of Contract
Before we recommend to the County Board of Supervisors that they accept the Contract
for the subject project as complete, we must have your concurrence with the
following:
The County may withhold from monies otherwise due, the amount of $4,800.00
for materials and labor to complete the items listed on the attached
"Withhold Summary." If any item is not satisfactorily completed by June 15,
1995, the County may use the money withheld, in the amount designated at the
respective line item, to complete the work.
If you are in agreement with these conditions, please sign on the line provided at
the bottom of this letter and return it to us to serve as notice of your agreement.
We will need your response by May 10, 1995, in order for the Board of Supervisors to
act on acceptance of the contract at their meeting on May 16, 1995.

1. Clean carpet and base. $ 200.00
2. Install carpet base at stair. 500.00
3. Clean window frames and glass. 100.00
4. Install supporting wall at upper-level gate of handicapped lift
(see Change Order No. 3) . 1,600.00
5. Test and adjust handicapped lift for trouble-free operation. 400.00
6. Submit certifications and/or warranties for building insulation, 150.00
roofing system, and sealant and caulking.
7. Secure all access panels, covers, and grilles at roof-top 100.00
HVAC unit.
8. Furnish and install electronic ballasts and specified lamps 1,000.00
in lighting fixtures.
9. Install on-off switch for roof-top exhaust fan in junction 200.00
box adjacent to fan.
10. Replace 15A receptacles (2) with 20A. 50.00
11. Complete HVAC balance work and submit report. 500.00
