DIRECT the Health Services Director to obtain from other urban counties in California a sense of what model each county uses in providing services to the homeless in an effort to insure that this County does not have to unnecessarily "reinvent the wheel" and provide a report on the various models which are in use to the Family and Human Services Committee on April 24, 1995 . 6 . ENCOURAGE the Homeless Advisory Committee to sponsor a forum at which it can gather information on various models for providing services to the homeless in other communities . 7 . REQUEST the County Administrator to schedule a date, place and time when the Family and Human Services Committee can meet with the members of the Homeless Advisory Committee in order to jointly plan for the direction in which the Board of Supervisors should move in developing and implementing a comprehensive plan for services to the homeless in this County. 8 . In preparation for the joint meeting between the Family and Human Services Committee and the Homeless Advisory Committee, DIRECT the Health Services Director to develop at least the outline of a continuum of services which need to be in place in order to adequately meet the needs of the homeless in this County and report it to the Family and Human Services Committee when it meets with the Homeless Advisory Committee. 9 . DIRECT the Health Services Director to provide the members of the Family and Human Services Committee by March 24, 1995 with copies of the proposed revised rules for admission, residence and exclusion from the shelters which are operated by Shelter, Inc. 10 . APPROVE a month-to-month contract between the County and Shelter, Inc. for the continued operation of the shelters, effective April 1, 1995 . BACKGROUND: On November 8, 1994, the County Administrator presented a report to the Board of Supervisors which contained 22 recommendations, all of which were approved by the Board of Supervisors. The last of these recommendations read as follows : 22 . DIRECT the County Administrator and Health Services Director to provide a progress report to the 1995 Internal Operations Committee in March, 1995 on the status of each of these recommendations and for this purpose REFER this matter to the 1995 Internal Operations Committee, REMOVING it as a referral to the 1994 Internal Operations Committee. On December 20, 1994, this issue was removed from the Internal Operations Committee and referred to our Committee by the Board of Supervisors. F&HS-01 -3- On March 13, 1995, our Committee met with Dr. Wendel Brunner, staff from the Health Services Department, staff from the Housing Authority, members of the Homeless Advisory Committee and other advocates for the homeless. Dr. Brunner presented our Committee with the attached report outlining the actions which had been taken on the 22 recommendations since November 8, 1994 . Dr. Brunner emphasized that it is essential to get the ongoing shelter operation working on a sound basis before turning to the longer-term issues having to do with the homeless . For this purpose, the Health Services Department has temporarily reassigned Brenda Blasingame from the tobacco control project to work full- time under the direction of Dr. Brunner and Chuck Deutschman, Substance Abuse Program Director on the homeless programs, with specific attention to working out the operation of the shelters . She has been working primarily on evaluating some of the problems with the dual case manager/case counselor system, which is simply not operating in an effective manner. Funding for homeless programs continues to be a problem. Dr. Brunner reported that we only have enough funds to guarantee continuation through the end of April, 1995 . He hopes, however, that there will be sufficient funds to continue through the end of June, 1995 . Dr. Brunner noted that HUD has set a deadline of April 6, 1995 for the submission of a grant application for Supportive Services RFP funds which should assist in supporting the continuum of services . Dr. Brunner' s report identifies the immediate objectives of the County' s homeless programs to be completing the restructuring of the case managed shelter program, clarifying funding for and financing of the homeless programs and hiring a Homeless Coordinator. Longer term goals are identified as working with PACT to identify and catalog the existing services for homeless people in the County, developing a single, seven-day-a-week homeless referral system and formalizing the planning process for the continuum of care and developing a comprehensive homeless plan. We received a number of comments and questions from those who were present. In response to a question from Susan Prather about how staff could obtain input from the homeless themselves, Dr. Brunner agreed that this was a weakness since the only mechanism for obtaining such input was through the Homeless Advisory Committee. Ms . Prather also referenced a letter from the Human Relations Commission urging that the investigation of Shelter, Inc be continued. She also asked why the contract with Shelter, Inc. was being continued without having gone out with an RFP to determine what other providers might be able and willing to provide shelter services to the homeless. Dr. Brunner responded that it was difficult to contemplate an RFP when the County had no idea where funding would be coming from in the future. David Ammann complained that no one is allowed to go on-site to the shelters and talk with the residents about what problems or complaints they may have. Dr. Brunner agreed that representatives from the Homeless Advisory Committee should be able to visit the shelters on a planned basis and talk to those residents who wish to talk to someone. He emphasized, however, that this is the residence for the homeless and we must observe their privacy. Mary Lou Laubscher suggested that the County needs to build an oversight program with the Homeless Advisory Committee that is positive and sensitive. She expressed concern about whether there is enough staff to get everything done and deal with problems as soon as they come up. Dr. Brunner agreed that there were staffing problems and noted that there might not be enough money to run the program through June 30, 1995. F&HS-01 -4- Staff also clarified in response to a question that Pat Pinkston's primary role was to write grant applications and seek funding for the homeless programs . In response to questions from Committee members, Dr. Brunner indicated that a complete report on the homeless would require having a Homeless Services Coordinator on scene to undertake the needed planning and that might take three months . Our Committee indicated to those present our frustration that seemingly so little had been accomplished in the four months since November. It appears that little has been done except to extend the contract with Shelter, Inc. It was noted that the Board of Supervisors had made a conscious decision that a comprehensive plan was needed. It appears that little if any planning has been done in the meantime. It was suggested that our Committee may have to establish the Task Force and meet with it in order to make some progress . Brenda Blasingame, in response to a question from the Committee, noted that it was difficult to make permanent changes in the operation of the shelters when she is only interim. The most important thing appeared to be getting the shelter under control and addressing the dual case management system which is simply not working. She suggested that the Homeless Coordinator or Program Manager needed to be on board before the need for additional changes in the system could be evaluated and implemented. Dr. Brunner indicated that a decision on a homeless program manager would be made within a week or so, although the individual might not be on the job for as long as a month. Our Committee believes that it is essential and feasible to undertake the changes in the operation of the shelters as well as the longer-term comprehensive planning for the homeless at the same time on parallel tracks . Merlin Wedepohl suggested that a lot of what our Committee is talking about is being done. It just has not been pulled together in a cohesive form. Attached is a draft chart outlining each of the County Administrator' s recommendations approved by the Board of Supervisors on November 8, 1994, along with the individual responsible for implementing the recommendation and the action which was required. Our Committee went through each of these recommendations with Dr. Brunner and concluded that the current status was as is identified in the last column headed "Status as of March 13, 1995" . Our Committee emphasized our concern that we must address the underlying causes of homelessness, we have to deal with people who are at-risk of becoming homeless and not limit our efforts to operating transitional shelters. Shelters are not the long-term solution to the problem. We do not believe that we need to have a Planner on board before we can begin to gather the information needed for a plan. We have carefully outlined the above. recommendations in an effort to move things forward and insure that we accomplish the objectives set down by the Board of Supervisors four months ago. F_` •. tl"l Administrative Offices 597 Center Avenue Suite 200 --------- ----- - G4' Martinez,California 94553 TO: Supervisor Jeff Smith Supervisor Mark DeSaulnier FROM: Wendel Brunner, M.D. Assistant Health Services Director for Public Health DATE: March 10, 1995 SUBJECT: REPORT ON CONTRA COSTA COUNTY HOMELESS PROGRAMS Since the overall responsibility for the homeless programs in Contra Costa County was transferred to the Health Services Department December 1, 1994, a number of activities have been accomplished to evaluate and improve county contracted homeless services, facilitate improved communication and collaboration between homeless service providers and the county program, and plan for developing the most appropriate continuum of care for homeless persons in Contra Costa. Much more needs to be accomplished; and we outline also some of our near and longer range goals. STAFFING FOR COUNTY HOMELESS PROGRAMS The Health Services Department has assigned Wendel Brunner, M.D., Director of-the. Public Health Division, and Chuck Deutschman, Director of the Substance Abuse Division, to head up the Homeless Program efforts for the Health Services Department. The Department has assigned Brenda Blasingame to work full-time under the direction of Dr. Brunner and Mr. Deutschman on the homeless programs, on an interim 30-90 day assignment. Those three work closely with Pat Pinkston, from the Housing Authority; to constitute the four-person team primarily responsible for homeless issues for county government. Recruitment is well underway for a permanent Homeless Services Coordinator. Sixty-seven applications have been narrowed down to a short list, and a final selection will be made shortly. EMERGENCY CASE MANAGED SHELTER PROGRAM Our staff have worked very closely with Merlin We.depohl, Executive Director of Shelter, Inc., in what has been a very productive and collaborative relationship. Brenda Blasingame has devoted much of her time to visiting the shelters, meeting with shelter staff, clients, and county Supervisors Jeff Smith/Mark DeSaulnier March 10, 1995 Page 2 case managers. She has confirmed and more thoroughly evaluated some of the problems outlined in our previous report on the shelter system, particularly the problems with the dual case manager/case counselor system. Chuck Deutschman and I have decided, and Merlin Wedepohl has concurred, that on an interim basis and pending a final decision on the case management program and structure, we would institute weekly case manager/case counselor joint meetings at the shelter sites to review clients' individual progress. Prior to implementation of these weekly meetings, we have scheduled a retreat for shelter and county line staff to plan for procedures and programs to institute a more effective, collaborative working relationship in the shelters. The vast majority,of Shelter, Inc. and county staff are clearly committed to the welfare of the clients, and are eager to do their difficult work in a more effective manner. A shelter appeals procedure was adopted by the Board of Supervisors in January and has been implemented by Shelter, Inc. and the Health Services Department. Brenda Blasingame has been hearing routine appeals, which has also given her additional insight into the issues with the shelter program, case managers, and clients. Dr. Brunner has heard appeals in several .of the more significant cases. It is our expectation that as the overall shelter program improves, of between county and Shelter, Inc. staff increases, and greater clarity about the goals and direction of the program is developed, the shelter appeals issue will become less controversial and contentious. In the meantime, we are thoroughly investigating appeals and complaints to ensure fairness and to gain insight into the underlying problems which may need to be resolved. There is some understandable frustration on the part of the Homeless Advisory Committee that the shelter rules still have not been revised. I have discussed this issue with some members of the committee. It is our intention to rationalize the shelter case management program and clarify its direction, before developing new shelter rules. The rules should flow out of the purpose and goals of the program and be understood by both clients and staff, who are implementing them. We will report to the Homeless Advisory Committee on our progress at the next meeting, and solicit their advice and input, before proposing any new draft shelter rules. HOMELESS FUNDING ISSUES The Health Services Department fiscal office has set up a homeless cost center to handle the county managed homeless contracts and funding sources. Health Services.Department fiscal staff have met with fiscal staff from the Housing Authority, arranged for a transfer of fiscal contract management. We have assured Shelter, Inc. that there will be no interruption of payments in this transfer, as we understand that cash flow is an important issue to a community-based nonprofit. Our conservative projection is there is at least.enough funding through April for the emergency ' shelters, and we expect that a number of private fund raising efforts and grant proposals will provide funding through the fiscal year. PLANNING FOR THE CONTINUUM OF CARE We still anticipate that HUD will require a county-wide plan for a continuum of care for homeless services, and will provide (insufficient) block grant funding to support the continuum Supervisors Jeff Smith/Mark DeSaulnier March 10, 1995 Page 3 needs. Two weeks ago HUD announced that they will require a community planning process and a community identified continuum of care as part of the grant application for the Supportive Housing RFP which is due to the Feds on April 6. Fortunately, much work has already been done in this county through the PACT, Association of Homeless and Housing Service Providers, the Homeless Advisory Committee, and others. We have formed a joint PACT/MC Steering Committee to coordinate these efforts and develop the continuum of care description which all the grant applicants from Contra Costa County will include in their application. We believe Contra Costa County is in a good position to respond to the'HUD RFP, despite the extremely short notice we have been given. SOME FUTURE GOALS Some immediate objectives of the county homeless program include: • Completing the restructuring of the case managed shelter program with Shelter, Inc.; • Clarifying the funding and financing of the homeless programs and establishing a clear accounting system; • Hiring a Homeless Coordinator. Intermediate range goals include: s • Working with PACT to identify and catalog the existing services for homeless people in Contra Costa County; • Developing a single, seven-day-a-week homeless referral system in collaboration with the appropriate community-based organizations; • Formalizing the planning process for the continuum :of care, and developing a comprehensive homeless plan based on data, service needs, and community input. That plan will serve as a basis for seeking funding and prioritizing existing resources. ASSIGN to the Health Services Director the primary responsibility to provide services to homeless individuals and families and to coordinate those services with the Contra Costa Housing Authority and other appropriate agencies. 7. DIRECT the Health Services Director to work with Shelter, Inc. to prepare amendments to the contract between Shelter, Inc. and the County for the operation of the North Concord, Brookside and Shell Family Transitional Shelters in order to place appropriate responsibility and authority for program operations with the contractor, with program evaluation, oversight and monitoring being provided by the Health Services Department. 8. REFER to the Health Services Director for review the rules for the residents in the shelters. 9. AGREE to extend the existing contract with Shelter Inc. for operation of the North Concord and Brookside Shelters through March 31, 1995, with the understanding that those contracts can be amended as soon as all parties are prepared to recommend amendments to the Board of Supervisors. 10. Recognizing that there are insufficient funds available to expand the current level of services to the homeless, DIRECT the County Administrator, Health Services Director and Social Services Director to evaluate the County's capacity to assist as many homeless.persons as possible during the coming winter by undertaking a funding raising effort to be named "HOUSE THE HOMELESS FOR THE HOLIDAYS", designed to encourage cities, individuals, families and organizations to sponsor a bed at the shelter and thereby to raise sufficient funds .to cover the cost of adding an additional 40 beds at the North Concord Shelter for the period of December 1, 1994 - March 31, 1995. This should be accomplished by: 2 ❑ Requesting the 18 Mayors and City Councils to make contributions to sponsor a Shelter bed. ❑ Requesting private businesses to make contributions to sponsor a Shelter bed. ❑ Requesting service clubs to make contributions to sponsor a Shelter bed. • Requesting churches to make contributions to sponsor a Shelter bed. ❑ Issuing a press release urging citizens to contribute to the sponsorship of a Shelter bed. ❑ Utilizing the community bulletin board on Contra Costa TV (CCTV) to urge citizens to contribute to the sponsorship of a Shelter bed. The cost of a Shelter bed is: ✓ 20.00 per day. ✓ $ 140.00 per week. ✓ $ 620.00 per month. ✓ $2480.00 Jor the entire four month period from December 1, 1994 through March 31, 1995. 11. RATIFYthe decision of the County Administrator to immediately provide two meals a day at the shelters, APPROVEan interdepartmental service agreement between the Sheriff and the Health Services Department for this purpose. 12. DIRECT the Health Services Director to explore the feasibility of providing the residents of the North Concord and Brookside Shelters with facilities for the preparation and storage of food so that the residents can prepare themselves a sack lunch. 13. ACKNOWLEDGEthat the County Administrator has established an account at the Wells Fargo Bank which,can be used by citizens and organizations to deposit their contributions toward sponsoring a Shelter bed. AUTHORIZE the creation of one position in the Health Services Department to serve as Homeless Program Manager to manage all of the County's programs for the homeless. This will include the responsibility to: ✓ Oversee, monitor and evaluate the contracts with Shelter, Inc. and Rubicon Programs, Inc., ✓ Supervise the preparation of a comprehensive, integrated plan for homeless services, and ✓ Coordinate with all other involved Divisions within the health Services Department, all other involved County Departments, all concerned advisory boards, committees and commissions, and all concerned individuals and organizations in the community to insure that services are provided to homeless individuals in Contra Costa County in an appropriate, humane, and cost effective manner, given the funds which the Board of Supervisors is able to make available or which can be generated from other governmental and private sources,. and 3 15. AUTHORIZE the creation of one position in the Health Services Department to serve as a Homeless Planner and staff to the Task Force established pursuant to Recommendation #4, to be supervised by the Homeless Program Manager. This individual will be expected to: ✓ Contact the organizations named in Recommendation #4 as well as other interested bodies, ✓ Convene the Task Force, and ✓ Work with the Task Force in determining: o An appropriate continuum of services to meet the needs of the homeless in Contra Costa County, o Identify gaps in that continuum, o Prioritize filling those gaps, and o Assist in seeking funding to fill those gaps, with a view to returning a comprehensive, integrated plan for providing services to the homeless in Contra Costa County to the Board of Supervisors by June 30, 1995. 16. APPROVE the assignment of a position in the Housing Authority to prepare and submit grant applications seeking funding from any and all governmental and private sources in order to maintain and expand the homeless programs which are in place now and seek to fund gaps in the continuum, of services which may be identified in a comprehensive, integrated plan for homeless services. 17. APPROVEthe transfer of funds from the Housing Authority to the Health Services Department which are now being used within the Housing Authority to support the staff person who oversees the emergency shelter programs and transitional shelter programs, along with the responsibility for the McKinney Transitional Housing Program grant. These funds total approximately $58,000. 18. APPROVEthe transfer of funds from the Social Services Department to the Health Services Department which are. now appropriated within the Social Services Department for the emergency shelter programs. 19. APPROVEthe transfer of funds from the Social Services Department to the Health Services Department which are now appropriated within the Social Services Department for the McKinney Transitional Shelter Program. 20. DIRECT the County Administrator to identify appropriate costs which must be covered to accomplish these recommendations. 21. DIRECTthe County Administrator to return any necessary documents to the Board of Supervisors for further consideration as appropriate in order to accomplish any of the above Recommendations. 22. DIRECT the County Administrator and Health Services Director to provide a progress report to the 1995 Internal Operations Committee in March, 1995 on the status of each of these recommendations and for this purpose REFER this matter to the 1995 Internal Operations Committee, REMOVING it as a referral to the 1994 Internal Operations Committee. 4 BACKGROUND: On September 13, 1994, at the request of Supervisor DeSaulnier, the Board of Supervisors referred to the Internal Operations Committee the pending contract between the County and. Shelter, Inc. for operation of the single adult's homeless shelter. On September 27, 1994, the Board of Supervisors accepted a report from the Internal Operations Committee asking Dr. Wendel Brunner to review the operation of the homeless shelter programs and provide a report to the Internal Operations Committee on October 31, 1994. As a separate but related referral, on October 4, 1994, the Board of Supervisors adopted recommendations from the Internal Operations Committee regarding development of a comprehensive and integrated plan for homeless services in Contra Costa County. In addition, the Board of Supervisors requested that the County Administrator prepare recommendations on how the various interests could be coordinated in a Task Force and how the equivalent of a full-time staff person could be financed. The Internal Operations Committee met October 31, 1994 to receive the report from the Public Health Director on the results of his and his staffs review of the homeless shelter programs in the County and the contract with Shelter, Inc. for the operation of those programs. In addition, the County Administrator's Office presented recommendations on staffing and funding the preparation of the comprehensive, integrated plan for homeless services and establishing a Task Force to coordinate, oversee and advise the Board of Supervisors on the preparation of the plan. As a result of the October 31, 1994 meeting, the Internal Operations Committee has prepared a number of recommendations which would direct the County Administrator's Office to take a number of steps to propose changes in the way the programs for the homeless in this County are organized, funded and managed. In reviewing the reports of the Internal Operations Committee, it is clear that this subject and the policy direction being recommended by the Internal Operations Committee are too important to wait for another two months or even another 30 days before addressing them in a substantial manner. As a result, staff from the County Administrator's Office and the involved departments have met with the Homeless Advisory Committee and Shelter Inc. As a result of those meetings, we are prepared to recommend steps which we believe will address the fundamental issues which are covered in the Internal Operations Committee's report. Acting on our report today will allow progress to begin on making some of the needed changes. We believe this will allow us to move in the direction sought by the Internal Operations Committee and the full Board of Supervisors immediately, rather than waiting to make an initial progress report to the Committee in January.
