SUBJECT: Approval of Contract Change Order No. 5 for the Ygnacio Valley Road Overlay, Project
No. 0662-6R4221-92, Concord area.
SpecificRequest(s)or Recommen ation(s)&Background Justification
I. Recommended Action:
APPROVE Contract Change Order No. 5 in the approximate amount of $12,000.00 and
AUTHORIZE the Public Works Director to execute it in order to compensate the contractor, Oliver
de Silva, Inc. for unforseen extra work.
II. Financial Impact:
The Project Contingency Funds are sufficient to cover the change order cost. The project is funded
by Measure °C" Funds (95%) and the City of Concord (5%).
III. Reasons for Recommendations and Background:
The cumulative total of Contract Change Order Nos. 1 through 5 exceeds $25,000.00, requiring
Board approval.
IV. Consequences of Negative Action:
Without approval the contractor will not receive payment for work required.
