Susan Swift
SPECIFIC REQUESTS) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee
(Wendel Brunner, M.D. ) to execute on behalf of the County, Contract
Amendment Agreement #22-348-7 with Susan Swift, effective June 1,
1994 , to amend Standard Contract #22-348-5 (effective July 1, 1993
through June 30, 1994) , to modify the Contract Payment Provisions with
no change in the Contract Payment Limit.
II. FINANCIAL IMPACT:
None. This Amendment modifies the payment provisions only and does
not change the Contract Payment Limit.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On June 18 , 1993 , the Board of Supervisors approved Standard Contract
#22-348-5 with Susan Swift to provide consultation and technical
assistance to the Department's Prevention Program regarding public
relations and media work related to various grant projects during
Fiscal Year 1993-94 .
Approval of Contract Amendment Agreement #22-348-7 will allow the
Contractor to provide additional units of service through June 30,
1994 , while reducing the Contractor's expense reimbursement to reflect
actual need.

AMENDMENT SPECIFICATIONS

In consideration of Contractor's agreement to provide additional
hours of service under the Contract identified herein, County
agrees to modify the Contract Payment Provisions. County and
Contractor agree, therefore, to modify said Contract as specified
below, while all other parts of the Contract remain unchanged and
in full force and effect.
1. Modification of Payment Provisions. Paragraph 1. (Payment
Amounts) , subparagraph d. is hereby modified to read as follows:
" [X] d. (1) $ 23.25 per hour, NOT TO EXCEED a total of
1,905 hours of service; and
(2) In addition, County will reimburse Contractor
an amount not to exceed $2, 008.751 as
follows: a) for travel on behalf of the
County, and/or training fees, approved in
advance by the Assistant Health Services
Director--Public Health; b) for miscellaneous
expenses actually incurred in the provision of
services hereunder during the term of this
Contract; and c) at the rate of 28 cents per
mile for the use of a private automobile
required in the provision of services
hereunder. "
