SUBJECT: Approval of Contract Amendment Agreement #24-649-2 with
Evaluation, Management and Training Associates, Inc.

I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, to execute on
behalf of the County, Contract Amendment Agreement #24-649-2,
effective June 30, 1994, to extend Novation Contract #24-649-1 with
Evaluation, Management and Training (EMT) Associates, Inc. from June
30, 1994 through August 31, 1994, with a payment limit increase of
$4, 000, from 26, 400 to a new total Contract Payment Limit of $30,400.
II. FINANCIAL IMPACT:
This Contract is funded under a Grant Award from the U.S. Department
of Health and Human Services, Center for Substance Abuse Prevention
(CSAP) . No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On December 7, 1993 , the Board of Supervisors approved Novation
Contract #24-649-1 with EMT Associates, Inc. , for the period from July
1, 1993 through June 30, 1994, for independent evaluation services for
the Department's Born Free Project, as required by the Federal CSAP
Grant for this Project.
Approval o1. Contract Extension Agreement #24-649-2 will enable the
Contractor to analyze Project data and prepare a final report as
required by the Federal Grant.

AMENDMENT SPECIFICATIONS

In consideration for Contractor's agreement to provide additional services
under the Contract identified herein, County agrees to extend the term of
the Contract and to increase the Contract Payment Limit. County and
Contractor agree therefore to amend the Contract as set forth below while
all other parts of the Contract remain unchanged and in full force and
effect.
1. Extension of Term. The term specified in Paragraph 3 . (Term) is
hereby extended from June 30, 1994 to August 31, 1994
2. Payment Limit Increase. The payment limit set forth in Paragraph 4.
(Payment Limit) are hereby increased by $4, 000, from $26,400 to a new total
Contract Payment Limit of $30,400.
3 . Modification of Payment Provisions. The payment provisions set forth
in Paragraph 1. (Payment Amounts) , subparagraph d. , of the Contract Payment
Provisions is hereby modified by the addition of sub-subparagraphs (3) and
(4) , to read as follows:
11 (3) $2, 000 upon demand on or after the first day of the month of
August: 1994; and
(4) $2,000 upon demand on or after the last day of the month of
August: 1994, following acceptance by the Health Services Director
or his designee of Contractor's final report as set forth in
Service Plan Paragraph 2. (Service Activities) . "
4. Modification of Service Activities. Service Plan Paragraph 2 .
(Service Activities) is hereby deleted and replaced with the following new
paragraph:
112 . Service Activities. Contractor shall provide professional
consultation and technical assistance to complete evaluation of the
Department's Born Free Project, as required under the County's Federal
Grant Award (#29-458) from the U.S. Department of Health and Human Services
(Office for Substance Abuse Prevention) . Contractor's service activities
hereunder shall. include, but may not be limited to, the following:
"A. Describing socio-demographic, prenatal care, and alcohol and
drug use characteristics for Born Free and non-Born Free high risk, low
income patients to differentiate between populations for. outcome analysis.

"B. Analyzing data collection from Born Free client files,
Healthy Start, Merrithew Memorial Hospital and Family Stress Center to
determine the impact of participation in the Born Free Project on the
following:
113.) Birth outcomes;
2) Labor and delivery costs;
3) Family functioning;
4) Alcohol and drug abuse recovery; and
5) Retention.
"C. Meeting with the Project Director and Project Coordinator,
as required by the Born Free Project Director, to plan and review ongoing
evaluation activities.
"D. Preparing and submitting to the Health Services Director or
his designee, no later than August• 31, 1994, a Final Report of the CSAP
Evaluation, in the form and manner required by County and CSAP; and
"E. Other activities deemed appropriate by the Project's
Principal Investigator (Hope Ewing, M.D. ) or by the CSAP Project Director. "
