CONTRACT FOR BID PACKAGE NO. 2 - SITE PREPARATION AT MERRITHEW
MEMORIAL HOSPITAL REPLACEMENT, 2500 ALHAMBRA AVENUE, MARTINEZ

. I. RECOMMENDATION
A. APPROVE Change Order No.. 6 to the Contract dated October 5,
1993 , for Site Preparation - Demolition, Relocation and
. Modifications of Existing Structures and Features, and the
Widening of Street No. 4 at Merrithew Memorial Hospital, 2500
Alhambra Avenue, Martinez, and AUTHORIZE the County
Administrator to execute the Change Order, which provides for
a new twenty four inch (2411) storm drain and associated man
holes on "B" Street, and increases the contract guaranteed
maximum cost.
II. FINANCIAL IMPACT
Funds in the capital account established for the project are
sufficient to cover the amount encumbered by this contract, from
bond proceeds specified to the project. Bond financing for the
total project cost was secured in May, 1992 in accordance with .the
Board -of Supervisors direction.

III. REASONS FOR RECOMMENDATIONS
On October 5, 1993, the County awarded the contract to the lowest
bidder, Joe Carone, Inc. with a guaranteed maximum cost except for
the asbestos removal. The new guaranteed maximum cost will exceed
the total estimated cost stated in . the construction contract
agreement approved by the Board on October 5, 1993 . After
executing the original contract, five change orders were made for
extra services and listed as follows: Change Order No. 1 dated
February 1, 1994, Change Order No. 2 dated April 12, 1994, Change
Order No. 3 dated April 26, 1994, Change Order No. 4 dated May 3,
1994, and Change Order No. 5 dated May 11, 1994. The amount
listed in the construction contract agreement was $11, 124, 644 . 00.
The new guaranteed maximum cost will be $1, 319, 009.46, an increase
of $194, 365.46.
