MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: April 26, 1994 SUBJECT: Approval of Contract Change Orders Nos. 21 and 22,for the Assessment District 1992-1 project, Project No. 4500-6X5181-92, Danville area Specific Request(s)or Reoommendation(s)&Background&Justification I. Recommended Action: APPROVE Contract Change Orders Nos. 21 and 22, in the approximate amounts of $1,200.00, and $3,500.00, respectively, and AUTHORIZE the Public Works Director to execute them in order to compensate the contractor, Oliver de Silva, Inc., for work resulting from utility conflicts. II. Financial Impact: The project contingency funds are sufficient to cover the change order costs. The source of funds is Assessment District 1992-1. III. Reasons for Recommendations and Background: The cumulative total of Contract Change Orders Nos. 1 through 22 exceeds$25,000.00, requiring Board approval. The sequence and scope of work was altered because of underground utility conflicts. These unforseen conditions resulted in extra work. IV. Consequences of Negative Action: The lack of approval would preclude payment for work that was performed in good faith.
