SUBJECT: Approval of Contract Amendment Agreement #22-407-3 with the AIDS
Project of Contra Costa
SPECIFIC REQUEST(S) OR RECOMMENDATIONS) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #22-407-3 with the AIDS Project of Contra Costa,
effective April 1, 1994, to amend Contract #22-407-2 (effective August
1, 1993 through June 30, 1994) , to increase the payment limit by
$40, 000, from $3, 749 to a new Contract payment limit of $43, 749, and
to extend the term of the contract through March 31, 1995. This
Contractor provides HIV/AIDS case management services for Contra Costa
residents diagnosed with HIV Disease, and their families.
II. FINANCIAL IMPACT:
This Contract is funded by State Department of Health Services, Office
of AIDS. The additional funds are Federal funds from the Ryan White
CARE Act, Title I, Formula Grant, through an intergovernmental
agreement with Alameda County, who is the Grantee of these funds. No
County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In September, 1993, the County Administrator approved, and the
Purchasing Services Manager executed, Short Form Service Contract #22-
407-2 with the AIDS Project of Contra Costa to provide HIV/AIDS Case
Management service to County residents diagnosed with HIV Disease and
their families.
Approval of Contract Amendment Agreement #22-407-3 will allow the
Contractor to continue to provide services, through March 31, 1995.

AMENDMENT SPECIFICATIONS

In consideration of the Contractor's willingness to provide additional HIV/AIDS case
management services as set forth in the Contract identified herein, County and Contractor
agree to amend said Contract as specified below while all other parts of the Contract remain
unchanged and in full force and effect.
1. Extension of Term. The Term of the Contract set forth in Paragraph 3. (Term) is hereby
extended through March 31, 1995.
2. Payment Limit Increase. The Payment Limit set forth in Paragraph 4. (Payment Limit) is
hereby increased by $40,000 from $3,749 to a new total Contract Payment Limit of $43,749.
3. Modification of Payment Provisions. Payment Provisions Paragraph 1. (Payment Amounts)
subparagraph d. , is hereby modified to read as follows:
"[R] d. $ 340 monthly for services performed during the months of August,
1993 through March, 1994;
$ 3.673 monthly for services performed during the months of April,
1994 and May, 1994;
$ 3,682 for services performed during the month of June, 1994;
$ 3,333 monthly for services performed during the months of July, 1994
through February, 1995; and
$ 3.337 for the month of March, 1995; provided that final contract
payment will be withheld from Contractor pending receipt of a
properly documented final demand and approval by the Health
Services Director, or his designee, of Contractor's final
activity report specified in Paragraph 3. (Progress Reports)
of the Service Plan.
.4. Modification of Service Activities. Additional Provisions Paragraph 1. (Service
Specifications) is hereby modified to read as follows:
"1. Service Specifications. Contractor shall provide HIV/AIDS Case Management
services for Contra Costa residents diagnosed with HIV Disease, and their families,
including, but not limited to, emergency housing, coordination of food services and
transportation assistance, in accordance with the Work Plan which is on file in the County's
AIDS Program Office, 597 Center Avenue, Suite 200, Martinez, and which is incorporated herein
by reference. "
5. Submission of Progress Reports. Contractor's Final Activity Report as specified in
Service Plan Paragraph 3. (Progress Report) shall be submitted to County's Health Services
Director, or his designee, on or before April 1, 1995.
