MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: April 12, 1994 SUBJECT: Approval of Contract Change Order No. 5 for the Oakley Road Improvement project, Project No. 0662-6R4235-92, Oakley area. Specific Request(s) or Recommendation(s) &BackgroundJustification I. Recommended Action: APPROVE Contract Change Order No. 5 in the approximate amount of $13,000.00 and AUTHORIZE the Public Works Director to execute it in order to compensate the contractor, Vintage Grading and Paving, Inc. for work performed, including a cost reduction incentive payment. II. Financial Impact: The project contingency funds are sufficient to cover this change order cost. The project is funded by Developer Contribution (12%), Deferred Improvement Agreement (7%), Oakley/North Brentwood Area of Benefit (55%), State Transportation Partnership Program (15%), and Diablo Water District (11%). III. Reasons for Recommendations and Background: The cumulative total of contract change orders Nos. 1 through 5 exceeds $25,000.00, requiring Board approval. IV. Consequences of Negative Action: Without approval the contractor will not receive payment for work required.
