SUBJECT: REPORT ON THE COUNTY'S SMOKING CESSATION PROGRAM

RECOMMENDATIONS:
1 . AUTHORIZE the Healthy Lifestyle Committee to coordinate lunch
time Brown Bag seminars where representatives of various
private and community-based smoking cessation programs could
visit several worksite locations throughout the County and ,
present specific information on their programs, thereby
allowing employees to make an educated decision as to the type
of program they would like to try.
2 . AUTHORIZE the Healthy Lifestyle Committee . to begin
disseminating smoking cessation program and referral
pamphlets .
3 . AUTHORIZE the Healthy Lifestyle Committee to begin tracking
and awardingincentive prizes and special recognition awards
for those employees who are successful at their quest to kick-
the-habit.
4 . AUTHORIZE the Healthy Lifestyle Committee to begin recruiting
non-smoking volunteers to "sponsor" smokers who are attempting
to break the', habit.
5 . AUTHORIZE the Healthy Lifestyle Committee to make additional
recommendations to the Board of Supervisors as the Committee
feels such recommendations are timely and appropriate.
6 ., REMOVE this item as a referral to the 1994 Internal Operations
Committee.

BACKGROUND:
On November 2, 1993, the Board of Supervisors approved a report
from the 1993 Internal Operations Committee on this subject. Among
the recommendations approved by the Board of Supervisors was one
to:
4 . REQUEST the Labor/Management Healthy Lifestyle
Committee to prepare proposed criteria and
guidelines for County employees to attend smoking
cessation classes which include having the County
provide a portion of the time needed to attend such
classes, and return their recommendations to the
Internal Operations Committee on January 3, 1994 .
The Board of Supervisors subsequently referred this subject to the
1994 Internal Operations Committee and authorized the Acting
Personnel Director to make a report on behalf of the Healthy
Lifestyle Committee later than had been contemplated.
On February 28, 1994, the Acting Personnel Director met with our
Committee and reviewed the attached report with us . As can be
seen, most smoking cessation programs require a commitment of 11 to
2 hours and meet in the evening. The number of employees who are
still smoking and are interested in attending a smoking cessation
class is few enough and sufficiently spread out across the County
that it is not practical to consider a class just for County
employees . The Healthy Lifestyle Committee has concluded,
therefore, that there is no need for the County to provide
employees any County time to attend smoking cessation classes.
The Healthy Lifestyle Committee has made the above recommendations
for how they would like to proceed and our Committee is pleased to
forward these recommendations to the Board of Supervisors for
action.

variety of local smoking cessation programs designed to help
smokers kick their smoking habit. Offering a variety of smoking
cessation programs allows you,to choose the type of program
which will best suit your needs. In addition, this Resource l
Guide provides information regarding what you can expect when
you quit smoking such as, the benefits of quitting smoking,
understanding the withdrawal symptoms of quitting smoking, and
smoking and weight gain.

Please take a few moments to look through this brochure and see
which smoking cessation methods are available to you. Pick
which ever program you feel will help you to succeed and make
the decision today that you're going to use it to help you quit.
Remember that successful methods are as different as the people
who use them. What may seem silly to others may be just what
you need to quit, so don't be embarrassed to try something new.
ORGANIZATIONS

American Cancer Society

This is a 6-week program which meets once a week for 2 hours.
Classes focus on education, group support and behavior
modification. Classes are held at Kaiser Hospital in Richmond
and at the American Cancer Society Offices in Walnut Creek, as
well as other locations throughout the County. Contact the
American Cancer Society for locations and starting dates. Cost
for this program is a $20.00 suggested donation.
The American Cancer Society also has maintenance meetings for
the "I Quit" Smoking Programs, which are designed for those who
wish to remain smoke-free. These meetings are free and are on
the third Thursday of every month from 7:30 - 9:30 p.m. at the
American Cancer Society Office.

The American Lung Association coordinates the "Freedom From
Smoking" program at Kaiser Permanente Medical Center in i
Antioch. This is a 6-week program consisting of 7 sessions
.which meet for 1 1/2 hours per session. Classes focus on
education, group support and behavior modification. The cost
for this program is $24.00 for Kaiser members and $108.00 for
non-Kaiser members.

The American Lung Association also has many self-help materials
available. In addition to numerous books and pamphlets, they
have audio and video tapes available for purchase.

Kaiser Permanente Medical Center offers several different
programs. They offer the "Thinking About Quitting" program
which is a one session, 1 1/2 hour introductory workshop about
quitting smoking. Motivation, barriers, strategies and resources
will be discussed. Quitting methods, including nicotine
replacement therapy will be explained. At this class,, members
may initiate the evaluation process for the "Group Program with
Nicotine Replacement". The fee for this introductory session is
$3.00 for Kaiser members and --- for non-Kaiser members
The "Smoking Cessation" program is a 6-week, 8 session group j
program that addresses the physiological, behavioral and
psychological process of becoming,a non-smoker. Each session
meets for 1 1/2 hours. The fee for Kaiser members is $24.00 and
$108.00 for non-Kaiser members. i
The "Group Program with Nicotine Replacement" is a 7-week, 8
session program for smokers who have attended the "Thinking
About Quitting" and "Smoking Cessation" programs. A
behavioral and medical evaluation will be conducted before
enrollment. Nicotine patches will be prescribed'only for those
smokers most likely to benefit from them. The fee for this
program is $35.00 plus the cost of the prescription which will
vary according to health care coverage.
Kaiser also offers a "Stop Smoking Maintenance" program which
is an ongoing monthly support group for non-smokers. This
program is free and meets on the fourth Monday of every month
from 7:00 p.m. - 8:30 p.m.

The "Kick The Habit" program combines nicotine replacement and
behavior modification techniques in a medically supervised
program., Participants meet at 7:30 p.m. on Tuesday, j
Wednesday, and Thursday during the first week. Meetings will
be held once a week on Tuesdays at 7:30 p.m. after the first
week. The cost for this program is $275.00 per person. An j
ongoing support group is available.

"Smoke Stoppers" is a 3-part program. The first part is a free
introductory session which will explain how the program works.
The second part is the 5 Days To Freedom phase. This is the
intense part of the program which meets for 4 consecutive days.
The last part of the program is a 3-week support phase which
meets once a week. These support meetings discuss various
topics such as learning how to deal with food and food cravings, i
the importance of exercise, weight gain and substituting healthy
behaviors. The cost for this program is $160.00 per person.

Smoke-A-Non is a 3-week, 6 session behavior modification
program which meets twice a week from 7:30 - 9:30 p.m. This
program teaches participants how to identify smoking habits and
how to use this knowledge to learn how to successfully cope with
the desire to smoke. This program costs $50.00 per person and
is also available at Kaiser Permanente Medical Center in
Pleasanton.

phone consultation is required. It typically requires one to two,
1 hour individual sessions depending upon the patients response
to the therapy. Price varies between $75.00 - $95.00 per session
depending upon health care coverage. A recorded audio tape is
supplied for ongoing support. Appointments are taken Monday
through Friday from 9:00 a.m. - 7:00 p.m.
THE BENEFITS OF QUITTING SMOKING
Smoking is an instance where your body, even after decades of
smoking, will forgive you if you stop. The cardiac benefits start
to accrue almost immediately such that, in two years, much of
your risk of heart disease will have disappeared. Within five to
ten years, your risk will be no greater than if you had never
smoked. The risk of lung cancer and other malignancies begins
to decrease steadily after you quit, and after ten years your risk
is almost as low as that of nonsmokers, even if you have smoked
for years.
UNDERSTANDING THE WITHDRAWAL SYMPTOMS OF QUITTING
SMOKING
Smoking is addictive, both physically and psychologically and
when you quit, you are very likely to go through withdrawal.
You may experience some of the following symptoms; mood
swings, lightheadeness, fatigue, restlessness, difficulty
sleeping, and poor concentration, among other symptoms. These
physical symptoms last for approximately one week. Knowing
what to expect beforehand and understanding that your
symptoms will get better in just a few days can help you to get
through nicotine withdrawal.
SMOKING AND WEIGHT GAIN
Keeping weight in control is often given as a reason to start and
continue smoking. And yes, the fear of gaining weight is a
common complaint after quitting. While the nicotine in cigarettes
may speed up smokers' metabolisms, causing them to use more
calories, the fact is most weight gain after quitting is temporary,
and no amount of weight gain is equal to the health hazards of
continuing to smoke. You can compensate for any shift to a lower
metabolic rate that may occur when you quit by either consuming
fewer calories or exercising more. In fact, by doing just that,
a large portion of smokers maintain or eventually lose weight
when they give up smoking. The fact is that some quitters never
see any weight change after they quit.
DON'T GIVE UP
Many people who have successfully quit smoking got off to
several false starts, but they eventually reached their goal. If
you have a relapse, and start smoking again, don't give up. As
the old saying goes; "If at first you don't succeed, try, try

SUBJECT: Smoking Cessation Programs
BACKGROUND
On November 2, 1993, we reported to your committee on the status of the Smoking
Cessation Program and were requested to provide a follow-up report. Since that
time, we have met with the Committee concerning arrangements for Smoking
Cessation classes and have issued a follow-up questionnaire to County employees.
The joint Labor/Management Healthy Lifestyle Committee met on December 21, 1993
to prepare proposed criteria and guidelines for County employees to attend lunch
time smoking cessation classes including having the County provide a shared portion
of the time needed to attend such classes. At that time, the Employee Wellness
Program presented the Committee with findings on additional research relating to
the implementation of worksite smoking cessation,programs.
Classes
The Employee Wellness Program advised the Committee that of the non-profit
organizations and. community agencies that we spoke with concerning the
coordination of worksite smoking cessation programs, we learned that facilitators
of the programs require a minimum of 1-1/2 to 2 hours per session (2 hours being
the preference) to effectively present a program. The majority of the smoking
cessation programs are 6 to 7 weeks in length and currently meet in the evenings.
Also, facilitators require a 15 to 20 person minimum to initiate a worksite program.
It is more cost effective to funnel employees into already existing programs. The
attached resource guide lists only some of the community and private resources
currently providing smoking cessation classes.
Survey Results
The results received from the Smoking Cessation Assistance Survey distributed with
the January 10, 1993 payroll, showed that there are very few County employees still
smoking (189 of the 1,229 responses received or about 2.5 percent of the County
population). A Smoking Cessation Program Questionnaire was then distributed to
all County employees in December, 1993 which was to determine the level of interest
of those employees who are still smoking in attending worksite smoking cessation
programs. Of the 62 responses returned, 31 employees indicated an interest in
attending a worksite smoking cessation program. Twenty of those respondents
work in scattered locations throughout East, West and Central County.

Comparing the results of both surveys and the information obtained from the
smoking cessation program facilitators, the Healthy Lifestyle Committee suggests
proceeding in the following manner.
♦ Coordinate lunch time Brown Bag seminars where representatives of various
private and community based smoking cessation programs could visit several
worksite locations throughout the County and present specific information on
their programs. This would allow employees to make an educated decision as
to the type of program they would like to try.
♦ Begin disseminating smoking cessation program and referral pamphlets.
♦ Begin tracking and awarding incentives prizes and special recognition awards
for those employees who are successful at their quest to kick-the-habit.
♦ Begin recruiting non-smoking volunteers to "sponsor" smokers who are
attempting to break the habit.
Since negotiations have concluded, the Healthy Lifestyle Committee will establish a
regular meeting schedule to formulate additional joint recommendations for future
wellness programs.
