MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: March 22, 1994 SUBJECT: Approval of Contract Change Order No. 2 for the Avon Bridge Rehabilitation project, Project No. 0662-6134089-90, Avon area. Specific Requests or Recommendations &Background&Jusffication I. Recommended Action: APPROVE Contract Change Order No. 2 in the approximate amount of $42,000.00 and AUTHORIZE the Public Works Director to execute it in order to compensate the contractor, Valentine Corporation, for work specified, repairing damaged bridge rotation equipment and from changes in design and field conditions. II. Financial Impact: The project contingency funds are sufficient to cover the change order costs. The source of funds is Federal Funds (81%), Matching State Funds (1%), and Road Funds (18%). III. Reasons for Recommendations and Background: ound: The cumulative total of Contract Change Orders Nos. 1 and 2 exceeds $25,000.00, requiring Board Approval. Mechanical parts,which had been damaged, needed to be replaced to expedite bridge rotation. Disintegration of the bridge had to be stopped and repaired, unexpected field conditions necessitated design changes which resulted in extra work.. IV. Consequences of Negative Action: The lack of approval would preclude payment for work performed in good faith and still needed.
