SUBJECT: Approval of Contract Amendment Agreement #23-165-3 with Brian
Johnson, Inc.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:.
Approve and authorize the Health Services Director, or his designee
(Art Lathrop) , to execute on behalf of the County, Contract Amendment
Agreement #23-165-3; with Brian Johnson, Inc. , effective February 1,
1994 , to amend Short Form Service Contract #23-165 (as amended by
Contract Amendment Agreement #23-165-1 and extended by Contract
Extension Agreement #23-165-2) , to increase the Contract Payment Limit
by $24, 960, from $24, 960 to a new total Contract Payment Limit of
$49, 920.
II. FINANCIAL IMPACT:
This Contract is funded by CSA EM-1 (Measure H) funds. There is no
County General Fund impact. Board of Supervisor' s approval of this
Contract Amendment Agreement is necessary because the increase in the
Contract Payment Limit exceeds the amount the County' s Purchasing
Services Manager is authorized to approve in any twelve month period.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
This Contractor has been providing consultation and technical
assistance to the Department' s Emergency Medical Services (EMS)
regarding implementation of an Automated EMS Message Transmission
Network under a contract extension of Short Form Service Contract 23-
165 (effective January 15, 1993 through March 31, 1993) , as amended by
Contract Amendment Agreement #23-165-1.
Approval of Contract Amendment Agreement .#23-165-3 will allow the
Contractor to provide additional hours of service to complete phase II
of the Automated EMS Message Transmission Network, through June 30,
1994 .

AMENDMENT SPECIFICATIONS
Number 23-165-3
In consideration for Contractor's agreement to provide additional
units of service under the Contract identified herein (as amended
by Contract Amendment Agreement 123-165-1 and extended by Contract
Extension Agreement #23-165-2) , County agrees to increase the
Contract Payment Limit., County and Contractor agree, therefore, to
amend said Contract as. specified below, while all other parts of
the Contract remain unchanged and in full force and effect:
1. Payment Limit Increase. The Payment Limit set forth in
Contract Amendment Agreement #23-165-1, Paragraph 2 . (Payment Limit
Increase) is hereby increased by $24,960, from , 24,960 to a new
total Contract Payment Limit of $49,920.

2 . Service Unit Increase. The number of service units set
forth in Contract Amendment Agreement #23-165-1, Paragraph 3 .
(Service Unit Increase) , is hereby increased by 312 service units
from 312 to a new total of 624 service units.
