MICHAEL WALFORD, PUBLIC WORKS DIRECTOR DATE: February 8, 1994 SUBJECT: Approval of Contract Change Orders Nos. 15, 16, and 17 for the A.D. 1992-1 project, Project No. 4500-6X5181-92, Danville area Specific Request(s)or Recommendation(s)&Background&Justification I. Recommended Action: APPROVE Contract Change Orders Nos. 15, 16, and 17 in the approximate amounts of$2,100.00, $3,000.00, and $1,400.00, respectively, and AUTHORIZE the Public Works Director to execute them in order to compensate the contractor, Oliver de Silva, Inc., for work resulting from utility conflicts. 11. Financial Impact: The project contingency funds are sufficient to cover the change order costs. The source of funds is Assessment District 1992-1. 111. Reasons for Recommendations and Background: The cumulative total of Contract Change Orders Nos. 1 through 17 exceeds$25,000.00, requiring Board approval. The sequence and scope of work was altered because of encountering buried utilities. These unforseen conditions resulted in extra work. IV. Consequences of Negative Action: The lack of approval would preclude payment for work that was performed in good faith.
