SUBJECT: Approve Contract Amendment Agreement #29-350-7 with
Kaiser Foundation Hospitals
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute on
behalf of the County, Contract Amendment Agreement #29-350-7 with
Kaiser Foundation Hospitals, effective May 1, 1993, to extend
Contract #22-350-3 (as amended by Contract Amendment Agreements #29-
350-4, #29-350-5 and #29-350-6) through April 30, 1994, for County's
provision of professional home health services for Kaiser's
patients.
II. FINANCIAL IMPACT:
Under the terms of this Agreement, Kaiser pays the County a fee-for-
service in accordance with the fee schedule included in the
Agreement. As the Agreement has been amended over the years, the
fees have been increased to cover the County's cost of providing the
services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
The County's Home Health Agency has been providing home health
services for Kaiser Foundation Hospitals for a number of years.
Upon discharge, patients receiving care at certain Kaiser Foundation
Hospitals may be referred to County's Home Health Agency for
services such as skilled nursing; physical, occupational and speech
therapy; medical social work; and home health aide services in their
homes. The Agreement stipulates that the services be provided
within a 30-mile radius of a Kaiser Foundation Hospital or a Kaiser
Foundation Health Plan Office in Contra Costa and Solano Counties.
Amendment Agreement #29-350-7 extends the term of the original
Agreement, which was approved by the Board of Supervisors on June
20, 1989 and amended yearly thereafter, to April 30, 1994.
The Board Chair should sign four copies of the agreement, three of
which should then be returned to the Contracts and Grants Unit for
submission to the Contractor.

FOURTH AMENDMENT TO AGREEMENT BETWEEN
CONTRA COSTA COUNTY HEALTH SERVICES AND

FOR THE PROVISION OF HOME HEALTH SERVICES
THIS FOURTH AMENDMENT TO AGREEMENT is made and entered into as of the first
day of July 1993, by and between CONTRA COSTA COUNTY HEALTH SERVICES
(hereinafter referred to as "Contractor"), and KAISER FOUNDATION HOSPITALS, a
California nonprofit and public benefit corporation, (hereinafter referred to as "Hospitals").
WITNESSETH
A. Contractor and Hospitals have heretofore entered into a Agreement for the provision of
Home Health Services to Hospital's patients. The patients receiving treatment under this
Agreement are accepted for care only by Kaiser Foundation Hospitals and are referred
to Contractor as provided in the Agreement. All services are to be provided within a 30-
mile radius of a Kaiser Foundation Hospital or Kaiser Foundation Health Plan office in
Contra Costa and Solano Counties in California.
B. The parties hereto desire to amend the Agreement in the manner hereinafter set forth:
NOW THEREFORE, the parties agree as follows:
1. Effective July 1, 1993 Hospitals agrees to reimburse Contractor at the rates presented in
Appendix A, Schedule of Rates, attached hereto.
2. Effective July 1, 1993 all Kaiser Foundation Hospitals in the Northern California region
may participate under the terms of this Agreement.
3. Section 6.0 shall be amended to read: "All invoices will be submitted in duplicate and
submitted within ninety (90) days of completion of Covered Services visits to Member.
However, such ninety (90) day period may be extended when Contractor is not the
primary payer. In this event, Contractor may have up to one (1) year to reconcile

billings. After one (1) year, Hospitals will not be responsible for Contractor charges for
any reason whatsoever, including Contractor's error in billing or incorrectly billing
primary payer. Therefore, one (1) year after provision of Covered Services, Contractor
will assume responsibility for charges incurred and will not attempt to bill Hospitals nor
the Member.
In all other respects the Agreement is confirmed and continued in effect until terminated
by either party.

In consideration of services to be performed by Contractor, Hospitals agrees to reimburse
Contractor as follows:

Registered Nurse $139.00/visit
Home Health Aides $ 75.00/hr
Physical Therapists $134.00/visit
Speech Therapists $139.00/visit
Occupational Therapists $133.00/visit
Medical Social Workers $193.00/visit
The above rates include the visit, supplies, mileage and time spent in documentation, and
attendance at case conferences or any required meetings.
