Operations Committee on July 12, 1993 regarding what steps
John Muir Medical Center has taken to meet the terms of its
community obligation which was made a condition of its 20-year
designation as a trauma center.
3 . ACKNOWLEDGE receipt of this report from our Committee,
outlining the steps which have been taken to secure a new
operator for the Adolescent Drug Treatment Center in San
Ramon.
BACKGROUND:
As a part of our Committee' s on-going oversight of the subject of
drug and alcohol programming in the County and the implementation
of the Countywide Drug and Alcohol Action Plan, the Health Services
Director brought to our attention the fact that John Muir Medical
Center had located a new operator for its Adolescent Drug Treatment
Center in San Ramon.

On May 24, 1993, our Committee met with Chuck Deutschman, Substance
Abuse Program Coordinator for the Health Services Department; Beth
Schecter, Executive Director of New Connections, a drug treatment
program in the community and a drug program contractor with the
County; Bob Weis and Patricia Goodday, members of the Board of
Directors of New Connections; and Dave Stegman, Executive Director
of the San Ramon Valley Community Services Group and a member of
the Steering Committee for the Regional Coalition that sought a new
operator for the Adolescent Drug Treatment Center in San Ramon.
Mr. Stegman noted that members of the community were concerned once
they learned last year that the former operator of the Adolescent
Drug Treatment Center was pulling out because they felt they could
no longer afford to keep the facility open.
A community group was formed in an effort to identify another
operator who might be willing to try to make a go of the treatment
center financially. Mr. Stegman reported that an RFP was issued
nationwide in an effort to see what interest there was in operating
the program. Eleven applications were received in response to the
RFP, including one from New Connections. Four applicants were
interviewed and eventually the process selected a joint coalition
of the Hazelden organization (of Minneapolis) , which will operate
the inpatient facility, and New Bridges (of Berkeley) , which will
operate the outpatient and aftercare program. John Muir Medical
Center did not directly participate in the selection process . They
simply asked the Regional Coalition to see whether they could find
another operator for the program and, if so, to tell John Muir with
whom they should contract. A reception is being held on June 8,
1993 to celebrate the continuation of the Adolescent Drug Treatment
Center under its new operator.
Ms ., Schecter noted that New Connections was not selected as the
operator of the Adolescent Drug Treatment Center in San Ramon, but
is still concerned about the need for low-cost drug treatment for
youth in Contra Costa County. She shared with the Committee the
.attadhed_=le.tte.P dated February 3, 1993, to Supervisor McPeak,
suggesting the possibility that the Board of Supervisors would
endorse New Connection' s interest in approaching John Muir Medical
Center and suggesting that John Muir provide some financial support
to New Connections as a part of John Muir's obligation to provide
financial support to the community in exchange for receiving a 20-
year designation as a trauma center.
Mr. Weis noted that something had gone wrong with the selection
process for the operator of the Adolescent Drug Treatment Center,
because an in-county provider should have been given some
preference. Mr. Stegman responded that being an in-county provider
was not one of the criteria against which applications were
measured because this Center is seen as a regional resource and
whether the provider is from this county or not should not be of
great importance. Mr. Deutschman noted that about 50% of the youth
who have been treated at the Adolescent Drug Treatment Center were
from Contra Costa County.
Although no one present could be sure, New Connections believes
that some alternative way has been established to fund the Bay Area
Poison Control Center and that John Muir Medical Center has not had
to provide any funding toward .the Poison Control Center, which is
part of what they were originally asked to do by the Board of
Supervisors .
Mr. Deutschman referenced a letter dated March 15, 1993, from Mark
Finucane to Supervisor McPeak in which Mr. Finucane attempted to
correct the impression that John Muir had not made any payments to
the Poison Control Center.
Our Committee feels that we need to have an update as to exactly
what contributions John Muir Medical Center has made to the
community in connection with . the trauma center designation. In
addition, we want to clarify exactly how the Bay Area Poison
Control Center is currently being funded and whether any
alternative mechanisms have been developed for this purpose. Our

Committee believes that in the future John Muir Medical Center
should look to providing support for other drug and alcohol
rehabilitation programs, although we are not necessarily suggesting
that such support should be for New Connections in particular.
What further actions we will recommend to the Board of Supervisors
will depend on the nature of the reports we receive back from the
Health Services Director in response to the above recommendations.

this idea should come from the Board of Supervisors before we raise •
it with the administration at John Muir.
To that end we have prepared the attached document for discussion
with you and the other supervisors . We would appreciate it if you
would review this and give us your thoughts on it, as well as
procedural guidance on discussing it with the other supervisors.

REQUEST FOR SUPPORT FROM THE CONTRA COSTA COUNTY
BOARD OF SUPERVISORS
OVERVIEW
New Connections, a non-profit agency based in Concord, California, has been
providing substance abuse prevention and intervention services for youth and
families since 1972 . In 1990, the agency began offering adult chemical
dependency treatment services and became licensed by the State of California
in 1991 . The agency has been and is primarily funded by government monies,
67%, with the remaining 33% funded by the private sector.
Faced with the realities of the current economic conditions and declining
government budgets, New Connections has undertaken numerous efforts over the
past two years to shift the funding of the agency to more private sector
monies thus creating a more stable and better balanced agency.
NEEDS AND OPPORTUNITY
Recently, John Muir Medical Center, the Contra Costa County Trauma Center,
announced its intention to close the John Muir Adolescent Treatment Center
(ATC) based in San Ramon, Ca. in June, 1993. ATC is the only organization
providing adolescent chemical dependency treatment services in the County. No
other provider exists or has come forward at this time.
New Connections is in the process of developing an outpatient adolescent
chemical dependency treatment program modeled after our very successful adult
4krogram. Without sufficient funding of $100,000, we are unable to proceed
eyond the developmental stage. This amount would allow us to complete
program development and implement the program for one year, prior to fees
being accumulated. Our goal is to provide a low cost program with sliding
scale slots available.
REQUEST FOR BOARD OF SUPERVISOR SUPPORT
New Connections has learned that John Muir Medical Center, in return for
being granted a 20 year term on their Trauma Center contract, agreed to meet
the County's funding requirement for the Poison Control Hotline. It is our
understanding that to date, no such funding has been requested by the Poison
Control Center, nor has been paid by John Muir.
New Connections would like the Board of Supervisors ' active support in
discussing with John Muir the possibility of reallocating a portion of the
monies promised by John Muir for the Poison Control Hotline to the adolescent
treatment .program currently in the process of development.
By doing so, the Board would be first, meeting a Board identified County
need: adolescent chemical dependency treatment; actively helping to achieve
a greater balance in the funding of the New Connections nonprofit agency;
and, collaborating (a Board stated objective) with the private sector in
reducing the reliance of the County indigent population on governmental
assistance.

Last month, you forwarded to me a copy of a proposal from New Connections to reallocate
a portion of John Muir's payment for the Poison Control Hotline for an adolescent
treatment program.
I am very sympathetic to the need for an adolescent treatment program in Central County.
It has been identified by the Advisory Board as a high priority. The New Connections
model for substance abuse treatment has great potential for the adolescent population and
would be an important addition to the limited option of services currently available.
However, New Connections is incorrect that John Muir Medical Center has not provided
funding for the Poison Control Center. For FY 92-93, Muir agreed to make a cash
contribution of $31,200 plus conduct a direct mail fundraising campaign for the Poison
Control Center with a guarantee of $30,000. I understand that a first installment
representing half of the total $61,200 has been made to the Poison Control Center and the
remaining amount is being paid in monthly installments as scheduled. Muir's obligation to
provide Contra Costa County's share of funding for the Poison Control Center was a
condition of granting Muir the trauma center designation last year.
I would also like to point out that the $61,200 in cash and fundraising guarantee to the
Poison Control Center is in addition to other fees and programs provided by Muir. The
annual fee alone -- $75,000 -- is the highest in the state. Muir also agreed to pay $30,500
each year to fund a County injury prevention program. Not including the $30,000
fundraising guarantee for the Poison Control Center, Muir's cash payment requirements to
be the designated trauma center total $135,000 -- nearly double the second highest
designation fee charged by Sacramento County. Other counties' trauma fees range from
zero to $41,560 a year with an average of $17,800 a year.

Although I applaud New Connections attempt to find a local private sponsor for its
program,given the significant fees provided by John Muir, I think pursuing the funding from
John Muir as a condition of its trauma designation would not be very productive. However,
that notwithstanding, we will bring the subject up with representatives of John Muir outside
of the trauma center process. Mr. Deutschman has been directed to broach the subject with
Medical Center representatives.
