SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute on
behalf of the County, Contract Amendment Agreement #22-435-1 with
Catholic Charities (Diocese of Oakland) , effective April 1, 1993, to
amend Standard Contract #22-435 (effective August 1, 1992 through June
30, 1993) , to increase the payment limit by $27, 500, from $18, 000 to
a new Contract payment limit of $45,500, and to extend the term of the
Contract through November 30, 1993 . This Contractor provides housing
advocacy services for persons in Contra Costa County with HIV Disease
and their families.
II. FINANCIAL IMPACT:
This Contract is funded 100% by Federal funds through the CARE Act,
Title 1, and HUD "Housing Opportunities for People with AIDS" monies.
No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In October, 1992, the County Administrator approved, and the
Purchasing Agent executed, Standard Contract #22-435 with Catholic
Charities (Diocese of Oakland) to provide housing advocacy services
for persons in Contra Costa County with HIV Disease and their
families.
Approval of Contract Amendment Agreement #22-435-1 will allow the
Contractor to provide additional services through November 30, 1993.

AMENDMENT SPECIFICATIONS
Number 22-435-1
In consideration of the Contractor's willingness to expand HIV/AIDS housing advocacy services
as set forth in the Contract identified herein, County and Contractor agree to amend said
Contract as specified below while all other parts of the Contract remain unchanged and in
full force and effect.
1. Extension of Term. The Term of the Contract set forth in Paragraph 3. (Term) is hereby
extended through November 30, 1993.
2. Payment Limit Increase. The Payment Limit set forth in Paragraph 4. (Payment Limit) is
hereby increased from $27,500 to a new total Contract Payment Limit of $45,500.
3. Modification of Payment Provisions. Payment Provisions Paragraph 1. (Payment Amounts)
subparagraph d. , is hereby modified to read as follows:
"[R] d. $ 1.636 monthly for services performed during the months of August,
1992 through March, 1993;
$ 5.074 monthly for services performed during the months of April,
1993 through June, 1993;
$ 3.438 monthly for services performed during the months of July, 1993
through October, 1993; and
$ 3.438 for the month of November, 1993; provided that final contract
payment will be withheld from Contractor pending receipt of a
properly documented final demand and approval by the Health
Services Director, or his designee, of Contractor's final
activity report specified in Paragraph 3. (Progress Reports)
of the Service Plan."
4. Modification of Service Activities. Service Plan Paragraph 2. (Service Activities) is
hereby modified to read as follows:
"2. Service Activities. Contractor shall provide HIV/AIDS housing advocacy services
for Contra Costa residents diagnosed with HIV Disease, and their families, in accordance with
the Work Plan which is on file in the County's AIDS Program Office, 597 Center Avenue, Suite
200, Martinez, and which is incorporated herein by reference."
5. Submission of Progress Reports. Contractor's Final Activity Report as specified in
Service Plan Paragraph 3. (Progress Report) shall be submitted to County's Health Services
Director, or his designee, on or before December 15. 1993.
