RECOMMENDED ACTION: Approve and authorize the Chair, Board of Supervisors, to execute on behalf of the County, Contract Amendment Agreement #22-401-2 with Pittsburg Preschool Coordinating Council, effective April 1, 1993, to amend Standard Contract #22-401-1, to increase the payment limit by $30, 000, from $27 , 625, to a new Contract payment limit of $57, 625, and to extend the term of the Contract through May 31, 1994. This Contractor provides Case Management services for persons in Contra Costa County with HIV Disease. II. FINANCIAL IMPACT: This Contract is funded 100% by Federal funds through the Ryan White CARE Act, Title II, and HUD "Housing Opportunities for People with AIDS" . No County funds are required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: On June 9, 1992 , the Board approved Standard Contract #22-401-1 with Pittsburg Preschool Coordinating Council, Inc. , for the period from June 1, 1992 through June 30, 1993 , to provide Case Management services for persons in Contra Costa County diagnosed with HIV Disease. Approval of Contract Amendment Agreement #22-401-2 will allow the Contractor to provide additional services through May 31, 1994. AMENDMENT SPECIFICATIONS Number 22-401-2 In consideration of the Contractor's willingness to provide additional HIV/AIDS Case Management services as set forth in the Contract identified herein, County and Contractor agree to amend said Contract as specified below while all other parts of the Contract remain unchanged and in full force and effect. 1. Extension of Term. The Term of the Contract set forth in Paragraph 3. (Term) is hereby extended through May 31, 1994. 2. Payment Limit Increase. The Payment Limit set forth in Paragraph 4. (Payment Limit) is hereby increased by $30.000, from $27,625 to a new total Contract Payment Limit of $57.625. 3. Modification of Payment Provisions. Payment Provisions Paragraph 1. (Payment Amounts) subparagraph d. , is hereby modified to read as follows: "[R] d. $ 2.125 monthly for services performed during the months of June, 1992 through March, 1993; $ 4.268 monthly for services performed during the months of April, 1993 through June, 1993; $ 2.143 monthly for services performed during the months of July, 1993 through April, 1994; and $ 2.141 for the month of May, 1994, provided that final contract payment will be withheld from Contractor pending receipt of a properly documented final demand and approval by the Health Services Director, or his designee, of Contractor's Final Activity Report specified in Paragraph 3. (Progress Reports) of the Service Plan. " 4. Modification of Service Activities. Service Plan Paragraph 2. (Service Activities) is hereby modified to read as follows: "2. Service Activities. Contractor shall provide HIV/AIDS Case Management services for Contra Costa residents diagnosed with HIV Disease, in accordance with the Work Plan which is on file in the County's AIDS Program Office, 597 Center Avenue, Suite 200, Martinez, and which is incorporated herein by reference." 5. Submission of Progress Reports. Contractor's Final Activity Report as specified in Service Plan Paragraph 3. (Progress Report) shall be submitted to County's Health Services Director, or his designee, on or before June 15, 1994. Initials: Contractor County Dept.
